Abstract

Introduction
利什曼病是全世界流行的寄生虫传染病,在发展中国家面临重大挑战,被世界卫生组织1、2号确认为最重要的被忽视的热带病之一。利什曼病的特点是皮面、黏膜和/或内脏表现。皮内利什曼病通常是由L.亚马逊西斯,L.墨西哥,L.巴西兰西斯,L.Guyensis,L.主要,L.热带和L.aethiopica3。这种形式的疾病往往是自我修复在人类由于诱导保护性细胞免疫反应。然而,细胞免疫反应可能失败,并且疾病可以进展到传播皮性利什曼病4,5。由于利什曼病物种和宿主遗传背景的多样性,没有可用的疫苗。治疗方案也有限,因为目前可用的大多数药物要么昂贵,有毒,要么可能需要长期治疗8,9。此外,有报告称,对现有的治疗方法有抗药性10,11。
利什曼病的病原体是原生动物寄生虫利什曼病。寄生虫在其生命周期中呈现两种截然不同的形态形式:在沙蝇中发现的鞭状体;在沙蝇中发现的鞭状体;和乳腺,在哺乳动物宿主巨噬细胞的寄生虫中发现细胞内形式12,13。尽管脊椎动物宿主的巨噬细胞具有防御机制,但阿马斯蒂戈特的入侵、生存和复制能力仍受到许多研究,研究14、15、16、17。因此,一些研究小组一直在描述体外巨噬细胞感染测定,以评估特定环境因素以及寄生虫和宿主基因对寄生虫感染性的影响。这种测定提供了几个优点,如能够适应高通量格式的研究,获得结果的时间相对较短,以及减少牺牲的实验室动物数量。然而,体外测定的发现是有限的,因为它们并不总是在体内研究14,19,20,21复制。体内检测为宿主-寄生虫相互作用提供了系统生理概述,外体测定不能完全模仿。例如,免疫学研究可以通过免疫组织化学测定从收集的足垫组织部分,甚至从波普利特淋巴结进行分析恢复的免疫细胞22。
动物在生物和生物医学研究中经常被用作人类疾病的模型,以更好地了解疾病的基本生理机制23。在利什曼病的情况下,接种的路线、地点或剂量影响疾病结果24,25,26,27。此外,人类和小鼠的易感性和抗药性受到宿主和寄生虫4、5、22、28、29、30、31的遗传背景的高度调节。BALB/c小鼠极易受到L.亚马逊斯皮感染,随着寄生虫传播到淋巴结、脾脏和肝脏32,疾病进展迅速。由于该疾病可能进展到皮质转移,感染可能是致命的。相比之下,C57BL/6小鼠在L.亚马逊感染测定33中经常出现慢性病变,具有持久性寄生虫负荷。因此,L. amazonensis感染这种特殊的小鼠物种一直被认为是研究人类长期形式皮性利什曼病的极佳模型,因为它比BALB/c小鼠感染模型5,34更好地模仿疾病进展。

Protocol
圣保罗大学生物科学研究所动物护理和使用委员会(CEUA 342/2019)核准了所有实验程序,并根据圣保罗州保护和使用实验室动物的建议和政策(Lei Estadual 11.977, de 25/08/2005)和巴西政府(莱联邦11.794,de 08/10/2008)。第 1-5 节中描述的所有步骤都应无菌地在层流柜内执行。在处理活的利什曼病寄生虫时,应使用个人防护设备。
1. 在体外分化L. 亚马孙普罗马斯蒂戈特到轴,20,35,36,37,38
注: L. 亚马逊斯(MHOM/BR/1973/M2269) (La) 寄生虫用于此测定.根据研究目的,感染性寄生虫形式可以通过使用密度梯度(如前14所述)纯化元循环形式,或者通过根据以下方案将原虫分化成轴状的乳腺体。
- 在含有10 mL的培养基(亲培养基)(pH = 7.0)的25厘米2细胞培养瓶中生长拉丙酮。
- 在25°C孵育3天。
注:使用在体外通过不到10倍的前列腺培养物,以避免寄生虫39、40、41、42、43、44的毒性和失调变化。 - 移液器 5 mL 对数生长阶段前乳酸培养成新的 25 厘米2瓶。

2. C57BL/6 脚垫感染L. 亚马孙病
注:在圣保罗大学生物医学研究所动物中心获得并维持雌性C57BL/6小鼠(6-8周大)。动物们得到了食物和水。
- 通过将PBS中稀释的寄生虫悬浮液的等分分转移到Neubauer腔室(即细胞计),计算轴动物的培养物。计数非旗状寄生虫,它们代表乳腺状。
注: 或者,Trypan 蓝色 (1:1) 可用于计数可存活的乳腺的数量(即,那些没有沾染 Trypan 蓝色)。 - 根据所需的接种剂量和拟接种的接种量(建议在 PBS 的 50 μL 中 1 x 106轴质乳腺)稀释 PBS 中的轴乳酸精。
- 用27G针头装入结核菌内注射器,并带有制备的寄生虫悬浮液。

3. 鼠标脚垫病变发展
- 每周测量一次病变的进展,使用卡钳测量左侧(受感染)和右侧(未感染)脚垫的厚度。
- 每周计算左右后脚垫之间的厚度差,以评估病变进展。
- 绘制 Y 轴上的计算差异和 X 轴上的感染时间,并计算统计显著性。
注:根据动物护理和使用委员会的建议,在感染的病变变得溃疡之前,必须牺牲动物,因为皮肤溃疡可能导致继发感染。如补充图1所示,用本方案中使用的寄生虫剂量(106轴病)和宿主菌株(C57BL/6)来呈现溃疡的迹象大约需要10周。在发现溃疡的迹象之前,应该牺牲小鼠进行病变提取。
4. 小鼠脚垫病变提取和寄生虫限制稀释
- 准备一个96孔板(平底),通过添加180μL的亲介质测定46,47。
注:对每种动物使用四个板道(板的一半),代表四重检测(即动物1=道A-D;动物2=车道E-H)。 - 每个病变在玻璃组织研磨管中加入 1 mL 的亲介质,并称量管。
注:管必须保持无菌,因此,如果称量在层流柜外,请使用无菌盖,用合上盖称量管。 - 按照动物护理和使用委员会的建议,在CO2室中牺牲动物。用70%乙醇喷洒,对动物进行消毒。
- 将动物的脚踩在脚后跟,取出受感染的脚垫,在脚垫上喷洒70%的乙醇进行消毒。确保剪刀和钳子的灭菌,保持他们浸泡在70%乙醇。
- 将受感染的脚垫放入无菌培养皿中,并使用无菌钳和手术刀进行解剖,以收集所有软组织。丢弃骨骼。
注:建议在解剖步骤中均匀性,以避免有偏置的组织恢复。 - 将收集的组织转移到玻璃组织研磨管与亲培养,然后再次称重管,以确定病变重量。
注意:避免将钳子和手术刀直接放入介质中,因为小体积可能被移除,从而导致组织重量被低估。病变的重量是通过从仅含有亲培养基的管的重量中减去含有亲培养基的管的重量来计算的。 - 使用研磨机使组织均匀化 10 倍,实现完全的组织破坏。
- 让混合物沉淀10分钟,然后收集20μL的上清液。
- 在步骤 4.1 中准备的 96 井板1 st通道的 1 个柱中加载 20 μL 的上清液。在接下来的三个通道中重复此操作,使每种动物都有四重点(即,对于动物 1,向每个井添加 20 μL,并标记 A1、B1、C1 和 D1)。

5. 病变寄生虫负载测定
- 使用倒置显微镜分析孵育7天后的板块,以确定每个通道的最后一口含寄生虫的孔。
注:通过对介质的颜色变化和浊度的视觉分析,还可以指示寄生虫的生长或细胞密度。 - 通过按病变重量划分稀释系数,计算每个通道的寄生虫组织负荷。
注:在特定通道中,如果寄生虫只存在于第1-5列中(即井A1-A5对寄生虫生长呈阳性),这意味着第5列仅含有1种寄生虫,第4列含有10种寄生虫,等等,倍数为10。第1列将包含104种寄生虫,它表示上清液(步骤4.7中的非稀释样品)中最初20μL中的寄生虫数量。由于这 20 μL 用 180 μL 的亲介质稀释,初始管包含 5 x 105寄生虫: (1 x 104) / (0.02 mL) = 5 x 105寄生虫/1 mL。然后,通过将寄生虫的初始浓度除以病变重量(5 x 105)/ 病变重量(参见步骤 4.6),可以计算出该病变中的寄生虫负荷。 - 使用日志 Y 刻度绘制每个动物的四重结果的平均值。
6. 统计分析
- 使用每个实验组至少五个动物作为复制,将数据表示为平均 = 标准偏差。
- 使用未配对的双尾测试执行统计分析,认为 p 值 < 0.05 显著。

Representative Results
利什曼原生动物寄生虫在无脊椎动物和脊椎动物宿主的生命周期中以两种发育形式存在:原生动物,在雌性沙蝇的流明中发现的增殖形式;和乳腺,在哺乳动物宿主细胞的寄生虫中发现的增殖形式。前列腺有一个长长的体,宽约1.5微米,长20微米,通常从前肢出现一个旗杆。Amastigots 的体形为圆形或卵形体,长度为 2-6 μm,宽度为 1.5-3 μm,并具有明显的旗杆12、13(图 1A)。在血餐期间,无脊椎动物宿主,一种家族精神失常的造血昆虫,获得感染利什曼病的巨噬细胞。一旦这些细胞到达沙蝇消化管,乳腺被释放,并分化为前循环原体(图1A)。这些形式是非传染性的,并通过二元分裂而密集繁殖,并殖民昆虫载体的消化管。然后,前循环形式分化为元循环形式,一种感染性和快速移动的形式,呈现更薄的身体和细长的旗子(图1A)。代谢形式侵入食道的前部分和沙蝇的经证实的三叶草,因此,在其下一次血餐中,回流确保这些感染形式进入新的脊椎动物宿主。在脊椎动物宿主的食道中,寄生虫被巨噬细胞吞噬,分化成寄生虫体内的乳腺,其中乳腺由二元分裂繁殖,完成利什马尼亚12、13的生命周期。
轴病条件可以在体外模拟不同的宿主环境,保持寄生虫的形态和生存能力。此前,对乳腺的轴电子条件进行了描述,模拟了巨噬菌体的寄生虫的寄生虫环境,并触发了体外体外分化成乳腺37型的亲子卵体。这些条件模拟酸性环境(pH = 5.5)和脊椎动物宿主(34°C)温度升高。图1B说明了通过改变文化中的这些条件而分化为原状的亲子。这些斧头乳腺的生存能力可以通过Trypan蓝色染色来分析,这种方法基于活细胞拥有完整的膜,排除了某些染料,而死细胞不48。或者,我们分析了轴状乳腺的生存能力,验证它们在转移到中性pH值并在25°C下孵育时转化回原体的能力(图1B)。
在这里,我们提出了一种体内感染方法,以评估不同利什曼病菌株的毒性。图2A代表体内感染测定,显示感染野生型(La-WT)和利什曼病铁调节器1敲除(La-LIR1-/-)L.亚马逊纯化代谢素的C57BL/6小鼠脚垫的皮质病变发展。LIR1调节利什曼病中细胞内铁水平,调解铁出口,防止细胞内积累至毒性水平14。观察受感染的足垫与未感染脚垫的厚度差异的进展,我们能够证明La-LIR1-/-受感染的小鼠比La-WT感染小鼠的病变更小(图2A)。这些发现表明,LIR1对体内的亚马孙菌发生至关重要。这表明这种方法在评估利什曼病驱动性皮病差异方面的重要性和有效性。图2B显示了感染后73天未感染(右)和受感染(左)脚垫和病变发展,显示了La-WT和La-LIR1------受感染小鼠肿胀和病变进展的差异。
利什曼病感染的进展不仅包括代表炎症反应的病变发展,还包括寄生虫的细胞内复制。为了评估寄生虫的复制,病变的寄生虫负荷是通过提取受感染的病变来确定的,随后在96井板中进行限制稀释测定(图3A)。图3B显示了感染73天后La-WT和La-LIR1-----受感染小鼠脚垫病变的寄生虫负荷分析。 从极限稀释测定,我们发现106倍寄生虫在La-LIR1---------------------------WT感染小鼠的病变中,表明缺乏LIR1可以防止14的细胞内复制。
评估病变发育和寄生虫负荷的优点之一是检测寄生虫细胞内复制和宿主炎症反应的可能差异。我们观察到这两种表型之间的差异使用补充LIR1(La-LIR1AB),这是La-LIR1-/-与LIR1ORF集成回核糖体位14。当La-LIR1AB被注射到小鼠的脚垫中时,我们观察到与La-LIR1-/----------WT感染相比的中等大小的病变,但La-WT表型(补充图2)的完全寄生虫负荷救援。 这些结果表明,La-LIR1AB寄生虫能够在长期体内感染中像La-WT寄生虫一样复制。然而,小鼠炎症反应没有拉-WT感染那么严重,因为病变明显较小。

本议定书中描述的体内感染测定允许任何研究人员评估体内皮性利什曼病,考虑在系统场景中的宿主-寄生虫相互作用。这些测定方法已被许多组使用22,24,27,29,31,32,34,49,在这里我们编译了一个分步协议,以标准化这种方法,同时考虑一些组可能有的基础设施限制。该协议也可以用来评估转基因利什曼病寄生虫的毒性,通过体内生物成像50,51,52。与任何其他实验程序一样,此测定有局限性和关键步骤需要执行,例如要求经过培训的人员能够舒适地与小鼠一起工作,并具备执行次平面注射的经验,以避免意外感染。标准化协议对于避免有偏见的结果和在不同研究组之间产生可比结果极为重要。

在自然循环中,利什曼病通过受感染的沙蝇在血餐期间的叮咬而向脊椎动物宿主传播。沙蝇通常接种几百种利什曼病在昆虫的唾液中形成的利什曼病皮乳酸。在实验性体内感染中,最常见的接种地点是动物的足垫22。内皮注射到耳朵或腹内注射是接种的替代位点,取决于研究的目的,因为每个位点呈现不同的噬菌细胞类型24,56。因此,一些研究小组利用实验室感染的沙蝇来感染动物的耳皮,以模仿自然传播56、57、58、59。然而,该协议提出了一些限制,如沙飞菌群的维护,这需要大多数研究团体没有的设施。
描述体内C57BL/6感染La-LIR1-/-的原始作品已经使用了纯化的代谢原体形成14。 然而,利什曼病基因操纵可以损害promastigots分化成轴状的乳腺14或成元循环感染形式20。因此,根据利什曼病菌株,研究人员应确定最适当的方法,以获得可行的感染寄生虫形式,为他们的研究。这里描述的与原体培养不同的轴突的议定书可以是一种更容易的替代,在许多情况下产生可比的结果19,20,35,37,38,64。这种方法避免了使用其他方法,通常导致感染性寄生虫的产量降低,例如用特定但不可用的抗体65孵育原体,用于元循环前列腺素纯化,或者通过密度梯度依赖于代谢液的LPG表达18,66。通过确定马斯特金家族基因的表达水平,可以评估分化协议的效率64,67。Amastins是保护基因家族的成员,在利什曼病生命周期68期间进行差别调节,与寄生虫毒性和发病机制67、69、70相关。其他标记也可用于区分乳腺体和亲子形。例如,gp63在乳腺中受到降序,因为它的作用是保护昆虫的消化酶71。
在开发标准化的体内感染协议时,小鼠菌株的选择是另一个需要考虑的关键步骤。小鼠对利什曼病感染的易感性和抗药性主要受遗传背景29、30、55的调节。在这个协议中,选择C57BL/6菌株是因为它对L.亚马逊西斯的免疫反应与人类72,73的混合Th1-Th2反应密切相关。实验性鼠感染L.亚马逊病已被描述为导致中度病变在C57BL/6小鼠相比,其他小鼠菌株28,34,74。然而,根据寄生虫菌株,病变大小的差异只有在易感小鼠菌株中才能检测到,如BALB/c36。感染的时间过程亦须加以考虑,并与所选择的小鼠应变29、26、60、61、62、63、75有关。应始终避免溃疡足垫,因为它可能代表继发感染,并且经常在长时间感染时观察到,特别是在易感小鼠菌株的实验中。指定一天中开始感染的时间是另一个需要考虑的步骤。正如之前与L.亚马逊森的研究所证明的,寄生虫接种的时间影响病变的发展,因为宿主-寄生虫相互作用在第75天的黑暗时间以昼夜释放的褪黑激素的形式受到昼夜传播的影响。
