Purpose. To assess the consistency of anterior segment measurements obtained using a Sirius Scheimpflug/Placido photography-based topography system (CSO, Italy) and IOLMaster partial coherence interferometry (Carl Zeiss Meditec, Germany) in eyes with cataracts. Methods. A total of 90 eyes of 90 patients were included in this prospective study. The anterior chamber depth (ACD), keratometry (K), corneal astigmatism axis, and white to white (WTW) values were randomly measured three times with Sirius and IOLMaster. Concordance between them was assessed by calculating 95% limits of agreement (LoA). Results. The ACD and K taken with the Sirius were statistically significantly higher than that taken with the IOLMaster; however, the Sirius significantly underestimated the WTW values compared with the IOLMaster. Good agreement was found for Km and ACD measurements, with 95% LoA of -0.20 to 0.54?mm and -0.16 to 0.34?mm, respectively. Poor agreement was observed for astigmatism axis and WTW measurements, as the 95% LoA was -23.96 to 23.36° and -1.15 to 0.37?mm, respectively. Conclusion. With the exception of astigmatism axis and WTW, anterior segment measurements taken by Sirius and IOLMaster devices showed good agreement and may be used interchangeably in patients with cataracts.

Parkinson disease (PD) is a neurodegenerative process that leads to a selective loss of dopaminergic neurons, mainly in the basal ganglia of the brain. Numerous studies have analyzed the ability of optical coherence tomography (OCT) to detect retinal nerve fiber layer (RNFL) thickness abnormalities and changes in PD, but the results have not always been consistent. Therefore, we carried out a meta-analysis to evaluate the RNFL thickness measured with OCT in PD.

We present a 49-year-old woman with nanophthalmos, white cataract, and malignant glaucoma in her left eye. The corrected distance visual acuity (CDVA) in that eye was hand motion, and the intraocular pressure (IOP) fluctuated between 28 mm Hg and 45 mm Hg with antiglaucoma medications. An unsutured sclerectomy combined with zonulectomy, hyaloidectomy, and anterior vitrectomy was performed. At 6 months postoperatively, the IOP was maintained at 12 mm Hg, the anterior chamber was deep, and the Snellen CDVA was 20/80. No intraoperative or postoperative complications were observed. The outcome in this case suggests that sclerectomy combined with phacoemulsification, zonulectomy, hyaloidectomy, and anterior vitrectomy is effective for the management of nanophthalmic patients with white cataract and malignant glaucoma.

Optic neuropathy is common in mitochondrial disorders, but poorly characterized in Friedreichs ataxia (FRDA), a recessive condition caused by lack of the mitochondrial protein frataxin. We investigated 26 molecularly confirmed FRDA patients by studying both anterior and posterior sections of the visual pathway using a new, integrated approach. This included visual field testing and optical coherence tomography (OCT), pattern visual evoked potentials (P-VEPs) and diffusion-weighted imaging. The latter was used to study optic radiation by calculating water apparent diffusion coefficients (ADC). All patients suffered optic nerve involvement with their disorder. Different patterns of visual field defects were observed and a variably reduced retinal nerve fiber layer thickness was seen by OCT in all cases. P-VEPs were abnormal in approximately half of the patients. Decreased visual acuity and temporal optic disc pallor were present in advanced stages of the disease, but only five patients were symptomatic. Two of these patients suffered a sudden loss of central vision, mimicking Lebers hereditary optic neuropathy (LHON), and of the other three symptomatic patients two were noted to be compound heterozygotes. ADC values of optic radiations in patients were significantly higher than controls (P < 0.01). Retinal nerve fiber layer thickness at OCT and P-VEPs correlated with age at onset and ICARS total score. ADC values correlated with age at onset, disease duration, GAA triplet expansion size, ICARS total score and P-VEPs. Visual pathway involvement is found consistently in FRDA, being previously underestimated, and we here document that it also involves the optic radiations. Occasional LHON-like cases may occur. However, optic neuropathy in FRDA substantially differs from classic mitochondrial optic neuropathies implying a different pathophysiology of visual system degeneration in this mitochondrial disease.

Purpose. Recent investigations suggested that unaffected carriers of Leber hereditary optic neuropathy (LHON) may show subclinical visual alterations. Structural changes have also been detected by optical coherence tomography (OCT), which revealed a temporal thickening of the retinal nerve fiber layer (RNFL). These changes may reflect compensatory effects such as mitochondria accumulation within the RNFL axons. This study aimed to investigate whether the RNFL of LHON carriers shows greater than expected thickness variations, which may reflect transient subclinical changes, over the course of years. Methods. Using Stratus OCT, the RNFL thickness was measured yearly from 2005 to 2008 in 24 Brazilian LHON carriers, all with homoplasmic 11778/ND4 mtDNA mutation. An Italian sample of 20 healthy subjects served as a control. Data were compared also to a previously published sample (n=59) of glaucomatous eyes. Results. The LHON carriers showed test-retest standard deviations that were larger than normal controls in the temporal (p=0.004), superior (p<0.0001), and inferior quadrants (p=0.019). Compared to the glaucoma cases, no statistical differences were observed. Conclusions. The RNFL thickness in LHON carriers, when measured at different time points, has higher variability than in normal subjects. Transitory RNFL swelling may be caused either by compensatory mechanisms (increased mitochondrial biogenesis) or by axonal stasis preceding decompensation of retinal ganglion cells. In both situations, these changes may represent the origin of the visual alterations previously detected in LHON carriers. Alternatively, increased variability of RNFL thickness may be influenced by the LHON microangiopathy, as retinal blood vessels contribute to the OCT RNFL thickness measurements.
