Fracture of the lateral process of the talus: a report of two cases.

Fracture of the lateral process of the talus is a rare lesion commonly described in snowboarding injuries. Nevertheless, several conditions can lead to this injury, which is often misdiagnosed as a severe ankle sprain because of the difficulty in detecting it on standard radiographic views. Computed tomography is very helpful for the assessment of this injury. The type of treatment will depend on the size and degree of the displacement of the fracture. This injury can also lead to subtalar joint osteoarthritis. We report 2 cases of fracture of the lateral process of the talus. In the first case, a young male sustained a combined inversion and dorsiflexion strain of his right foot when he fell from a ladder. In the second case, a woman broke the lateral process of her right talus by the same mechanism when she was sledding. In the 2 situations, we opted for an open reduction and internal fixation using 2.4-mm cannulated screws. Both patients were allowed walking with partial weightbearing with a walker boot (VACOped(®)) for 6 weeks. At 1 year, both showed a consolidated fracture and had regained their preinjury level of activity.

Perfusion bioreactors are known to exert shear stresses on cultured cells, leading to cell differentiation and enhanced extracellular matrix deposition on scaffolds. The influence of the scaffold's porous microstructure is investigated for a polycaprolactone (PCL) scaffold with a regular microarchitecture and a silk fibroin (SF) scaffold with an irregular network of interconnected pores. Their complex 3D geometries are imaged by micro-computed tomography and used in direct pore-level simulations of the entire scaffold-bioreactor system to numerically solve the governing mass and momentum conservation equations for fluid flow through porous media. The velocity field and wall shear stress distribution are determined for both scaffolds. The PCL scaffold exhibited an asymmetric distribution with peak and plateau, while the SF scaffold exhibited a homogenous distribution and conditioned the flow more efficiently than the PCL scaffold. The methodology guides the design and optimization of the scaffold geometry.

Bias dependence of tunneling magnetoresistance in magnetic tunnel junctions with asymmetric barriers.

The transport properties of magnetic tunnel junctions (MTJs) are very sensitive to interface modifications. In this work we investigate both experimentally and theoretically the effect of asymmetric barrier modifications on the bias dependence of tunneling magnetoresistance (TMR) in single crystal Fe/MgO-based MTJs with (i) one crystalline and one rough interface, and (ii) with a monolayer of O deposited at the crystalline interface. In both cases we observe an asymmetric bias dependence of TMR and a reversal of its sign at large bias. We propose a general model to explain the bias dependence in these and similar systems reported earlier. The model predicts the existence of two distinct TMR regimes: (i) a tunneling regime when the interface is modified with layers of a different insulator, and (ii) a resonant regime when thin metallic layers are inserted at the interface. We demonstrate that in the tunneling regime, negative TMR is due to the high voltage which overcomes the exchange splitting in the electrodes, while the asymmetric bias dependence of TMR is due to the interface transmission probabilities. In the resonant regime, inversion of TMR could happen at zero voltage depending on the alignment of the resonance levels with the Fermi surfaces of the electrodes. Moreover, the model predicts a regime in which TMR has different signs at positive and negative bias, suggesting possibilities of combining memory with logic functions.

The burden of obsessive compulsive disorder (OCD) may lead persons with diagnosed or undiagnosed OCD, and their relatives, to search for good quality information on the Web. This study aimed to evaluate the quality of Web-based information on English-language sites dealing with OCD and to compare the quality of websites found through a general and a medically specialized search engine.

The morbidity of bone graft harvesting from the iliac crest has been widely discussed in the literature. For some authors, it is considered to be low and for others relatively high. We report on a case of a fracture of the iliac wing after graft harvesting from the anterior iliac crest despite good surgical technique. This complication is well known and most of these fractures heal uneventfully if treated conservatively. However, if anatomical and technical considerations are respected, the patient could be spared this inconvenience. Based on a literature review, we discuss the procedures potential complications and how to avoid them in an update.

Modern computing technology is based on writing, storing and retrieving information encoded as magnetic bits. Although the giant magnetoresistance effect has improved the electrical read out of memory elements, magnetic writing remains the object of major research efforts. Despite several reports of methods to reverse the polarity of nanosized magnets by means of local electric fields and currents, the simple reversal of a high-coercivity, single-layer ferromagnet remains a challenge. Materials with large coercivity and perpendicular magnetic anisotropy represent the mainstay of data storage media, owing to their ability to retain a stable magnetization state over long periods of time and their amenability to miniaturization. However, the same anisotropy properties that make a material attractive for storage also make it hard to write to. Here we demonstrate switching of a perpendicularly magnetized cobalt dot driven by in-plane current injection at room temperature. Our device is composed of a thin cobalt layer with strong perpendicular anisotropy and Rashba interaction induced by asymmetric platinum and AlOx interface layers. The effective switching field is orthogonal to the direction of the magnetization and to the Rashba field. The symmetry of the switching field is consistent with the spin accumulation induced by the Rashba interaction and the spin-dependent mobility observed in non-magnetic semiconductors, as well as with the torque induced by the spin Hall effect in the platinum layer. Our measurements indicate that the switching efficiency increases with the magnetic anisotropy of the cobalt layer and the oxidation of the aluminium layer, which is uppermost, suggesting that the Rashba interaction has a key role in the reversal mechanism. To prove the potential of in-plane current switching for spintronic applications, we construct a reprogrammable magnetic switch that can be integrated into non-volatile memory and logic architectures. This device is simple, scalable and compatible with present-day magnetic recording technology.

Avulsion fracture of the peroneus longus tendon insertion at the base of the first metatarsal: report of a case.

Isolated avulsion fracture of the peroneus longus tendon insertion at the base of the first metatarsal is very rare. Similar to most avulsion fractures that result from excessive strain at a tendon or ligament insertion, this type of injury is caused by the strong tension exerted by the peroneus longus tendon. The mechanisms leading to this lesion and treatment options are not clearly defined. We present the case of an isolated minimally displaced intra-articular avulsion fracture at the plantar lateral base of the first metatarsal. Faced with a painful non-union following conservative treatment we considered excision of the bony fragment and first tarsometatarsal arthrodesis. This leads to a favourable functional outcome.

Over the past few years, several questionnaires have been developed to measure mindfulness. The Mindful Attention Awareness Scale (MAAS) was created to specifically capture attention and awareness in daily life (Brown & Ryan, 2003). In this article, we present a French adaptation of the MAAS. In the 1st study, we explored the psychometric properties of this adaptation. In the 2nd study, we investigated its relation to cognitive emotion regulation and depressive symptomatology using path analysis. As in the original version of the MAAS, the French adaptation has a strong 1-factor structure. Moreover, there was a negative relationship between the MAAS and the severity of depressive symptoms, both directly and indirectly. The indirect pathway was mediated by the nonadaptive cognitive emotion regulation strategy of self-blame and the adaptive cognitive emotion regulation strategy of positive reappraisal. In conclusion, this questionnaire represents a valid mindfulness measure for French-speaking clinicians and researchers.

We report the direct measurement of the nonadiabatic component of the spin torque in domain walls. Our method is independent of both the pinning of the domain wall in the wire as well as of the Gilbert damping parameter. We demonstrate that the ratio between the nonadiabatic and the adiabatic components can be as high as 1, and explain this high value by the importance of the spin-flip rate to the nonadiabatic torque. In addition to their fundamental significance these results open the way for applications by demonstrating a significant increase of the spin torque efficiency.

[Hyperthymic and cyclothymic temperaments: attenuated forms of bipolar disorder?].

A temperament is described as a temporally stable dimension, biologically determined. Several temperaments have been described (hyperthymic, depressive, cyclothymic, irritable and anxious) and could represent premorbid characteristics of an affective disorder. Some temperaments could thus correspond to chronic sub-affective states expressed as attenuated forms of mood disorder. Several studies suggest that a large proportion of bipolar patients have a hyperthymic or cyclothymic temperament. Moreover, temperaments seem to influence the clinical caracteristics of bipolar disorders. Future investigations will evaluate whether temperaments represent a predisposing factor, an attenuated form of bipolar disorders or a distinct entity.

The Oxford II medial unicompartmental knee arthroplasty: an independent 10-year survival study.

Unicompartmental knee arthroplasty (UKA) is being used increasingly. We report the outcome of a series of 48 Oxford II meniscal bearing UKAs carried out for osteoarthritis of the medial compartment (38 knees in 32 patients) and for spontaneous avascular necrosis of the medial femoral condyle (10 knees in 10 patients). Using the endpoint of revision for any cause, the outcome for every knee was established. Ten knees have been revised (20.8%). At ten years there were 215 knees still at risk and the cumulative survival rate was 77.7% (95% CI: 56.8 to 90.2). Our study reveals a higher failure rate than previous reports on this prosthesis. The technique of implantation of the Oxford knee is demanding; the learning curve is thus long and could explain, at least partially, our moderate results.

Hoarding or compulsive hoarding, a severe form of collectionism, is currently described in various psychiatric disorders, including the famous Diogenes syndrome. The lack of remission despite a correct treatment in a patient that we have followed in the brief therapy center in Geneva, has led us to carry out an extensive Medline review, which revealed several interesting points. It seems that excessive hoarding, with a prevalence of 2,3 to 4,6%, has received increasing interest and there is an actual debate of distinguishing hoarding from other disorders. Thus, this field requires further attention to better understand its phenomenology. This case report describes an excessive hoarding disorder confirming the complexity of this trouble and the possibility to consider it as an independent diagnosis.

In some cases the language used in the PubMed abstracts makes matching that content to a JoVE video difficult. In other cases, there happens not to be any content in our video library that is relevant to the topic of a given abstract. In these cases, our algorithms are trying their best to display videos with relevant content, which can sometimes result in matched videos with only a slight relation.
