4. إجراء Seldinger

Subclavian central venous catheter placement is used to gain central venous access through the subclavian vein. Central venous access is needed in many clinical situations for hemodynamic monitoring, medication delivery, and blood sampling. The subclavian route is one of three main sites used for central venous cannulation, along with the internal jugular and femoral veins.

The subclavian approach has several advantages. It is associated with lower infection and thrombosis rates than internal jugular and femoral central lines. It can also be placed quickly using anatomic landmarks. This makes it useful in trauma settings, especially when cervical collars block access to the internal jugular vein.

The main drawback is the risk of pneumothorax, which is air in the space around the lung. This risk exists because the dome of the lung lies just superficial to the subclavian vein. If the subclavian artery is punctured by mistake, the clavicle can make compression difficult and limit access to the vessel.

Successful placement depends on understanding the anatomy and following the Seldinger procedure. First, the subclavian vein is cannulated with an 18-gauge thin-walled needle. A guide wire is then advanced through the needle into the vessel. After the needle is removed, a dilator is passed over the wire to widen the skin and soft tissue, and the catheter is advanced over the wire into position. Finally, the guide wire is removed and the catheter is sutured in place.

Central venous catheter kits come in several forms from different manufacturers. A catheter may have a single lumen, a double lumen, or a triple lumen. A triple-lumen CVC is often used when multiple medications must be given at the same time, but the placement steps are the same for each type.
