Das menschliche Gehirn ist vom Kreislaufsystem durch eine Schutzbarriere isoliert, die als Blut-Hirn-Schranke bezeichnet wird.
 Sie besteht aus Endothelzellen, die die Kapillaren des Zentralnervensystems auskleiden, Die Blut-Hirn-Schranke besteht auch aus einer Basalmembran und spezialisierten Nervenzellen, den so genannten Astrozyten, die in engem Kontakt mit den Kapillaren stehen.
 Diese Komponenten tragen dazu bei, unerwünschte Substanzen am Überqueren zu hindern.
 Diese Barriere steuert den chemischen Austausch zwischen Blut und der interstitiellen Flüssigkeit im Gehirn über verschiedene Arten von Proteinen, die Verbindungskomplexe bilden.
 Im Allgemeinen können nur fettlösliche Verbindungen über die Barriere diffundieren, während wasserlösliche Verbindungen über aktiven oder passiven Transport hindurchkommen.

<h4>Überblick</h4>
 
 Die Blut-Hirn-Schranke (BHS) bezeichnet das spezialisierte Gefäßsystem, welches das Gehirn mit Nährstoffen im Blut versorgt und gleichzeitig die Bewegung von Ionen, Molekülen, Krankheitserregern und anderen Substanzen streng reguliert. Sie besteht aus eng miteinander verbundenen Endothelzellen auf der einen Seite und Astrozytenfüßchen auf der anderen Seite. Zusammen bilden sie eine semipermeable Schranke, die das Gehirn schützt. Daher stellt sie auch eine einzigartige Herausforderungen bei der Verabreichung von Medikamenten dar.
 
 <h4>Zelluläre Komponente</h4>
 
 Die BHS besteht aus einer Vielzahl von zellulären Komponenten, wozu auch die erwähnten Endothelzellen und Astrozyten gehören. Diese Zellen teilen sich eine gemeinsame Basallamina und regulieren gemeinsam den Durchgang der Stoffe zwischen dem Kreislaufsystem und der interstitiellen Flüssigkeit, die das Gehirn umgibt.
 
 Spezialisierte Endothelzellen formen die Wände der Hirnkapillaren. Sie sind durch extrem enge und komplexe interzelluläre Verbindungen miteinander verbunden. Diese Verbindungen bilden eine selektive physikalische Barriere, die eine einfache Diffusion der meisten Substanzen, einschließlich mittelgroßer bis großer Moleküle wie Glucose und Insulin, verhindert.
 
 Ein zweiter Zelltyp, die Astrozyten, sind ein Typ der Gliazellen des Zentralnervensystems, der die Endothelzellfunktion, den Blutfluss und den Ionenausgleich im Gehirn durch Interaktion und enge Verbindung mit der zerebralen Gefäßversorgung beeinflussen kann. Astrozyten stellen eine direkte Verbindung zwischen dem Gefäßsystem und den Neuronen her. Sie verlängern ihre Prozesse, die sogenannten Endfüße, die sich an einem Ende um die Blutgefäße wickeln, während sie am anderen Ende in engen Kontakt mit den Neuronen an den Synapsen treten.
 
 <h4>Chemischer Austausch</h4>
 
 Die Fähigkeit einer Substanz, die BHS zu durchqueren und die Effizienz, mit der dieser Austausch stattfindet, hängt von den chemischen und molekularen Eigenschaften jedes Moleküls oder Ions ab. Im Allgemeinen diffundieren kleine lipidlösliche Bestandteile wie Sauerstoff und Kohlendioxid schnell und einfach durch die Endothelschicht. Im Gegensatz dazu erfordern größere oder wasserlösliche Komponenten oft einen selektiveren Prozess, der einen passiven oder aktiven Transport durch eine Endothelzelle beinhalten kann. Der Austausch dieser Substanzen erfolgt in der Regel langsamer oder gar nicht. Das ist von der relativen Häufigkeit und Effizienz der spezifischen molekularen Rezeptoren und Transportproteine auf der Oberfläche der Endothelzellen und anderen Faktoren abhängig.
 
 <h4>Die BHS schützt das Gehirn</h4>
 
 Der durch die BHS ermöglichte neurale Schutz ist für die Gesundheit und Funktion des Gehirns von entscheidender Bedeutung. Eine Dysregulation der BHS kann zu schweren neurologischen Erkrankungen wie Multipler Sklerose, Infektionen und Ischämie aufgrund einer unzureichenden Blutversorgung führen. Umgekehrt kann sich die Aktivität der BHS nachteilig auf die Behandlung einiger neurologischer Erkrankungen auswirken, da die Aktivität der BHS die Passage von neuroaktiven Arzneimitteln in das zentrale Nervensystem verhindern oder erheblich reduzieren kann. Aus diesem Grund müssen Medikamente die neurologischen Ziele haben so konzipiert sein, dass die BHS passieren können.

The blood-brain barrier is a specialized system of blood vessels that helps control what enters the brain. It delivers nutrients from the blood while tightly regulating ions, molecules, pathogens, and other substances. This semipermeable barrier protects the brain and also makes it harder to deliver some therapies.

The blood-brain barrier includes several cellular parts, especially endothelial cells and astrocytes. These cells share a common basement membrane and regulate movement between the circulation and the fluid around the brain. Specialized endothelial cells form the walls of cerebral capillaries. They are joined by very tight intercellular junctions, which create a selective physical barrier. This barrier blocks simple diffusion of most substances, including molecules as large as glucose and insulin.

Astrocytes are glial cells in the central nervous system. They influence endothelial cell function, blood flow, and ion balance in the brain. Astrocytes connect closely with the cerebral vasculature. Their processes, called endfeet, wrap around blood vessels on one end and make contact with neurons at synapses on the other. This gives them a direct link between blood vessels and nerve cells.

What crosses the barrier depends on a molecule's chemical and molecular properties. Small lipid-soluble substances, such as oxygen and carbon dioxide, move across by rapid simple diffusion. Larger or water-soluble substances usually need a more selective process. They may move through an endothelial cell by passive or active transport. In many cases, this exchange is slower, or it may not occur at all, depending on receptors and transport proteins on the endothelial surface.

The blood-brain barrier is essential for brain health and normal function. When it does not work properly, severe neurological diseases can occur, including multiple sclerosis, infection, and ischemia from poor blood supply. The barrier can also make treatment difficult because it can reduce the passage of neuroactive drugs into the central nervous system. For that reason, medicines aimed at the brain must be designed to pass through the blood-brain barrier.
