General Approach to the Physical Exam

L’examen du corps est essentielle à l’exercice de la médecine. Depuis l’Empire romain, les médecins ont décrit le lien entre les modifications en fonction des différentes parties du corps et états pathologiques spécifiques et ont cherché à davantage de connaissances scientifiques pour améliorer le diagnostic de chevet. Cependant, dans cette ère moderne de plus en plus la technologie dans le domaine du diagnostic médical, il est important d’examiner le rôle que l’examen physique joue aujourd'hui. Il est erroné de croire qu’un examen physique détient toutes les réponses, et on a beaucoup écrit sur l’utilité douteuse de certaines manœuvres précédemment tenu en haute estime. Il est tout aussi erroné pour suggérer que l’examen physique joue qu’un rôle dans la rencontre de patients moderne.
L’examen physique demeure un précieux outil de diagnostic ; Il y a beaucoup de diagnostics qui ne peuvent être effectués que par un examen physique. Un diagnostic fait par les laboratoires ou l’imagerie se fait rarement en l’absence de conclusions détectables au chevet du malade. L’examen physique est fondamental dans la pratique de la médecine depuis des siècles. Malgré les progrès substantiels de l’instrumentation médicale, l’examen physique reste un outil de diagnostic précieux, et son importance ne peut être surestimée. Lorsque les médecins évaluent les antécédents et effectuent un examen physique, ils recueillent des informations menant à une hypothèse ferme, ce qui favorise une approche plus judicieuse de la prescription de tests et de l’analyse de ces tests. Cela a le potentiel de réduire les risques pour les patients et les coûts des soins de santé. L’examen physique peut être subdivisé en les composantes suivantes : enquête générale, mesure des signes vitaux, examen du fonctionnement neurologique et de l’état mental, examen de la tête, des yeux, des oreilles, du nez, de la gorge, de la poitrine, des poumons, des ganglions lymphatiques, cardiovasculaire, abdominal, musculo-squelettique, cutané, génito-urinaire et rectal. Un chaperon peut être nécessaire si un examen sensible comme des examens génito-urinaires, rectaux ou mammaires doit être effectué. Expliquez au patient l’examen physique qui sera effectué et demandez-lui la permission de procéder à l’examen. « Maintenant que j’ai expliqué ce que je vais faire, puis-je procéder à l’examen ? » Chaque examen consiste en des manœuvres utilisant les techniques d’inspection, de percussion, de palpation et d’auscultation, chacune d’entre elles étant explorée en détail dans des vidéos distinctes de cette collection. Vous devez vous efforcer de minimiser le repositionnement du patient en regroupant les manœuvres qui doivent être effectuées dans une position particulière. Une fois l’examen terminé, demandez au patient de remettre des vêtements réguliers. Il est optimal d’attendre que le patient soit à nouveau habillé avant de donner des conseils et un avis. Par la suite, en pesant comment la présence ou l’absence de signes particuliers affectent la probabilité de la maladie et en consolidant les informations obtenues à partir des antécédents du patient et de l’examen physique, on peut décider d’initier un traitement ou de prescrire des tests supplémentaires de manière délibérée et judicieuse. Physical examination remains an important part of medicine. Doctors have used the body exam for centuries to connect changes in body function with disease. Even with modern imaging and lab tests, the bedside exam still helps clinicians understand what may be happening in the patient.

Physical examination is not a replacement for tests, and it does not answer every question. Some maneuvers that were once highly valued may have limited usefulness. Still, many conditions can only be found through physical examination, and diagnoses made with labs or imaging usually begin with findings from the bedside.

As a provider performs the history and physical, they form and check ideas, or hypotheses, about the patient’s condition. These ideas help guide which tests to order and which questions to ask next. That can make testing more focused, reduce risk for the patient, and help lower health care costs.

The physical exam also supports the therapeutic relationship. By using careful and respectful touch, the clinician can show attention and sensitivity. This process can build rapport and may help promote healing during the patient encounter.
