Observation and Inspection

Observation se présente comme un processus constant au cours de la rencontre clinique. Beaucoup de raisons indiquées dans la procédure sont généralement faites en même temps et lorsque les occasions se présentent. Cette procédure met en évidence les éléments d’observation, mais n’est pas prévue de proposer une séquence préférée.
1. étude d’ensemble

	<li>Notez l’état général de santé du patient. Apparence du patient est compatible avec l’âge déclaré ? Le patient semble forme et en bonne santé ou faible et frêle ?</li>
	<li>Notez leur niveau de conscience (p. ex., éveillé, alerte ou somnolente).</li>
	<li>Observer les signes de la douleur. Notez les expressions faciales, mouvements surveillés, diaphorèse, etc..</li>
	<li>Observer des signes de détresse respiratoire. Le patient peut parler en phrases complètes sans difficulté ? Est le patient « tripoding » (penché vers l’avant avec les bras pris en charge) ? Sont visibles muscles accessoires de la respiration est-elle utilisées ?</li>
	<li>Observer des signes de détresse émotionnelle. Le patient s’agiter excessivement, présentant un ralentissement psychomoteur généralisée, ou pleurer ? Contact avec les yeux est-elle appropriée ?</li>
	<li>Prenez note de vêtements, bijoux, tatouages, toilettage, hygiène et toutes les autres fonctionnalités qui peuvent donner un aperçu de la situation médicale, sociale et émotionnelle du patient.</li>
	<li>Noter tout signe de pathologie qui ne peut être démontrée sur l’observation générale, telles que des lésions cutanées, répartition anormale des graisses, déficits de l’audience, atrophie musculaire, odeurs, etc..</li>

2. organ-specific observation
Durant le reste de l’examen physique, observation active s’effectue par un examen de chaque système d’organe. Pour certains systèmes d’organe, inspection nécessite l’utilisation d’équipement (p. ex., otoscope ou ophtalmoscope). Consultez les vidéos pour chaque système d’organe pour des détails spécifiques.
3. examen de la peau

	<li>Certains lieux d’examen offre des possibilités de découvrir les soutiens sociaux, intérêts et mode de vie du patient. Lors de la visite d’un patient dans un espace qu’ils occupent pendant plus de quelques heures (p. ex., chambre d’hôpital, maison de repos, domicile), noter la présence (ou l’absence) de décorations, cartes de prompt rétablissement, photos de famille, livres, etc. pour mieux comprendre la vie du patient en dehors du rôle de patient.</li>

Cette opportunité d’observation offre des informations importantes concernant le patient. Membre de la famille parle pour le patient ? Fait le regard du patient pour le membre de la famille avant de répondre aux questions ?</li>
	<li>Faites attention à la façon dont vous vous sentez en présence du patient, car cela peut s’avérer diagnostique utile, notamment en termes de maladie psychiatrique. Alors que les fournisseurs doivent être conscients des pièges du contre-transfert, si le sentiment que le patient déclenche en vous n’est pas typique ou facilement explicables, il peut y avoir une explication sous-jacente en santé mentale du patient. Par exemple, une sensation inhabituelle de tristesse dans le clinicien au cours de la rencontre peut conduire le médecin à poser un diagnostic de trouble dépressif majeur.</li>

Observation et contrôle est fondamentales pour l’examen physique et commence au premier point de contact avec un patient. Alors que l’observation et contrôle sont souvent utilisés de façon interchangeable, l’observation est un terme général qui fait référence à l’utilisation prudente de son sens pour obtenir des informations. L’inspection est un acte limité à ce que l'on peut observer visuellement et en se référant à l’examen physique, fait généralement référence aux conclusions sur la surface du corps, plutôt qu’aux comportements. Cliniciens qualifiés utilisent tous leurs sens pour aider à acquérir une compréhension de leurs patients, en s’appuyant sur la vision, touch (percussion et palpation) et audition (percussion et auscultation) principalement. L’observation et l’inspection sont fondamentales pour tout examen clinique. Les observations générales commencent dès le premier point de contact avec n’importe quel patient et se poursuivent tout au long de la rencontre clinique, même si la conversation est simple avec le patient. L’inspection est davantage axée sur un objectif et se limite à ce que l’on peut observer visuellement tout en examinant des parties spécifiques du corps comme la peau, les yeux ou les oreilles, parfois à l’aide d’un équipement spécialisé. Nous illustrons ici les observations générales qu’un clinicien devrait envisager d’effectuer lors de chaque rencontre clinique, suivies de quelques considérations liées aux étapes d’inspection visuelle. Tout d’abord, passons en revue quelques étapes d’observation générales qu’un clinicien doit garder à l’esprit lorsqu’il rencontre un patient. Ces observations peuvent être faites à tout moment pendant l’examen. Au cours de la conversation initiale, lorsqu’un patient explique sa maladie, notez l’état de sa santé physique et demandez-vous : « L’apparence du patient correspond-elle à l’âge indiqué ? De plus, recherchez des signes de détresse émotionnelle comme une agitation excessive ou un contact visuel inadéquat. Faites également attention à la façon dont vous vous sentez en présence du patient, car cela peut s’avérer utile pour le diagnostic. Un sentiment inhabituel de tristesse chez le clinicien peut conduire à envisager un diagnostic de trouble dépressif majeur. De plus, au cours de la conversation, observez les signes de douleur en regardant les expressions faciales et en notant si le patient présente des mouvements prudents ou des signes autonomes comme la diaphorèse. Notez également l’hygiène, les vêtements, le maquillage, etc. du patient pour des indices supplémentaires qui pourraient aider au diagnostic. De plus, recherchez des signes de détresse respiratoire. Notez si le patient parle en phrases complètes sans aucune difficulté. Observez si le patient est en train de « trépied », c’est-à-dire qu’il se penche en avant avec les bras soutenus. Remarquez si des muscles accessoires visibles de la respiration sont utilisés, ce qui est courant dans les cas de détresse respiratoire. Prises ensemble, ces simples observations peuvent fournir un aperçu substantiel de l’état physique et mental du patient et peuvent aider au diagnostic de sa maladie. Maintenant que vous connaissez les observations générales, examinons l’inspection, qui est essentielle à un examen physique complet et spécifique à un organe. Tout d’abord, expliquez au patient le but de l’inspection et obtenez son consentement. Comme mentionné précédemment, il s’agit d’observer visuellement les surfaces du corps pour vérifier toute anomalie. Clinical observation is the starting point of a physical examination and begins at the first contact with a patient. Observation is a broad term for using the senses carefully to gather information. Inspection is a narrower term. It refers to what can be seen with the eyes, especially on the surface of the body, rather than to behavior.

Skilled clinicians use more than sight when they examine a patient. They also rely on touch, through palpation and percussion, and on hearing, through auscultation and percussion. Smell can add useful clues as well. It may point to poor personal hygiene, substance use, or metabolic diseases.

Taste is mostly a historical part of medicine. For many centuries, diabetes mellitus was even diagnosed by the sweet taste of urine. Over time, clinicians also develop a sixth sense, often called gut instinct. This skill comes from deliberate practice on thousands of patients over many years. It is based largely on bedside observations and can help predict serious illness.
