- La nociception, généralement appelée "douleur",est le processus par le biais duquel un stimulus douloureuxest transmis par le système nerveux périphérique et centralpour prévenir d'un mal ou d'une blessure potentiels. 
Lorsqu'un signal nuisible est détecté, les nocicepteurs,terminaisons nerveuses libres, sont activés. 
Les mastocytes avoisinants libère des composés chimiquescomme les histamines,et les macrophages sécrètent des cytokines,de petites protéines au rôle capitaldans la signalisation immunitaire. 
Simultanément, les axones des nocicepteurstransmettent le signal à la corne dorsalede la moelle épinièrepar le biais de deux fibres de douleur différentes. 
Les premières à être activéessont les fibres A-delta, à myéline fine,qui conduisent la douleur aiguë, immédiate, bien localisée,à très grande vitesse, pour permettre au corpsde se retirer du stimulus douloureux. 
L'autre type d'axone sont les fibres C,qui conduisent le signal lentement, car sans myéline,et transmettent une douleur brûlante, prolongée. 
Depuis la moelle épinière, les signaux douloureuxpassent vers le tronc cérébral,puis vers le thalamus et le cortex somatosensitifoù les signaux de douleur sont interprétéspour déterminer la localisation de la blessure. 
D'autres zones du cerveau traitent la douleur,notamment les structures cortico-limbiques,y compris l'amygdale et le cortex préfrontalqui sont liés aux souvenirs émotionnels et à la cognition. 
Pour finir, les multiples interactions au sein du cerveaucontribuent à la perception subjectivedont les individus font l'expérience.

Nociception

La nociception — la capacité de ressentir la douleur — est essentielle à la survie et au bien-être général d’un organisme. Les stimuli nocifs tels que la douleur perçante d’un objet pointu, la chaleur d’une flamme non recouverte, ou le contact avec des produits chimiques corrosifs sont d’abord détectés par les récepteurs sensoriels, appelés récepteurs nociceptifs, situés sur les terminaisons nerveuses. Les nocicepteurs expriment les canaux ioniques qui convertissent les stimuli nocifs en signaux électriques. Lorsque ces signaux atteignent le cerveau par l’intermédiaire de neurones sensoriels, ils sont perçus comme de la douleur. Ainsi, la douleur aide l’organisme à éviter les stimuli nocifs.
 
 Le système immunitaire joue un rôle essentiel dans la pathologie de la douleur. En rencontrant des stimuli nocifs, les cellules immunitaires telles que les mastocytes et les macrophages présents sur le site des lésions libèrent des substances chimiques inflammatoires tels que les cytokines, les chimiokines, les histamines et les prostaglandines. Ces substances chimiques attirent d’autres cellules immunitaires telles que les monocytes et les cellules T jusqu’au site de la blessure. Ils stimulent également les nocicepteurs, entraînant une hyperalgie — une réponse plus intense à un stimulus précédemment douloureux — ou une allodynie — une réponse douloureuse à un stimulus normalement inoffensif tel que le toucher léger. Une telle sensibilisation à la douleur aide à protéger le site blessé pendant la guérison. 
 
 Dans certains cas, la douleur dépasse son rôle de système d’alerte grave si la sensibilisation ne parvient pas à se résoudre avec le temps. La douleur chronique — douleur persistante ou récurrente qui dure plus de trois mois &mash; accompagne souvent des affections inflammatoires comme la polyarthrite rhumatoïde. Les anti-inflammatoires non stéroïdiens (AINS) comme l’aspirine et l’ibuprofène réduisent la douleur en inhibant la synthèse des molécules inflammatoires prostaglandines. Cependant, les AINS et les opioïdes actuellement utilisés pour lutter contre la douleur présentent des effets secondaires graves et un risque de dépendance. Par conséquent, la compréhension des mécanismes à la base de la pathologie de la douleur peut aider à mettre au point des médicaments plus efficaces pour supprimer la perception de douleur avec des effets secondaires négatifs moins graves.

Pain signaling begins when the body detects a harmful stimulus. Sharp objects, open flames, and corrosive chemicals can trigger this response. Sensory receptors called nociceptors, which sit on nerve endings, detect these noxious stimuli and use ion channels to turn them into electrical signals.

Those electrical signals travel through sensory neurons to the brain, where they are experienced as pain. This warning system helps an organism avoid damage and stay safe. Pain is therefore an important part of survival and overall well-being.

The immune system also shapes pain after an injury. Mast cells and macrophages at the injury site release inflammatory chemicals such as cytokines, chemokines, histamines, and prostaglandins. These signals attract more immune cells, including monocytes and T cells, and they also stimulate nociceptors.

This immune response can increase pain sensitivity. Hyperalgesia is a stronger response to a stimulus that was already painful. Allodynia is pain caused by something that is usually not painful, such as light touch. These changes help protect the injured area during healing.

Pain can become a problem when sensitization does not fade. Chronic pain is persistent or recurring pain that lasts longer than three months. It often occurs with inflammatory diseases such as rheumatoid arthritis. NSAIDs, including aspirin and ibuprofen, reduce pain by blocking the production of prostaglandins. However, NSAIDs and opioids can cause serious side effects and may lead to addiction, so researchers continue to study pain mechanisms to find safer drugs.
