Respiratory Exam II: Percussion and Auscultation

1. Posizionamento
<ol><li>Assicurarsi che il paziente sia spogliato fino alla vita.</li>
	<li>Posizionare il paziente sul tavolo d'esame con un angolo da 30 a 45 gradi e avvicinarsi dal lato destro. L'esame della parte posteriore del polmone richiede che il paziente sia inclinato in avanti o seduto sul bordo del letto.</li>
</ol>2. Percussioni
<ol><li>Percuss sia posteriormente che anteriormente, a partire dalla schiena.</li>
	<li>Posizionare la mano non dominante con il dito medio (dito plessimetrico) premuto e iperesteso saldamente sulla zona centrale della schiena destra o sinistra del paziente (livelli più bassi di polmoni posteriormente). Più il dito viene premuto sulla parete toracica, più forte tende ad essere la nota di percussione.</li>
	<li>Assicurarsi che le altre dita e il palmo non siano premuti contro il petto del paziente.</li>
	<li>Utilizzare la punta del dito medio (dito plessore) della mano dominante per picchieggiare saldamente sul terzo superiore (falange centrale o distale) del dito plessimetrico della mano non dominante almeno due volte (si consiglia di tenere le unghie corte). Il suono dovrebbe essere cavo, rappresentando un polmone pieno d'aria.</li>
	<li>Ripeti la percussione a quattro e cinque livelli, confrontando ogni livello polmonare fianco a fianco, lavorando fino alla parete toracica, a partire dai bordi polmonari inferiori. Alla scadenza, il bordo inferiore dei polmoni è a livello della sesta costola sulla linea midclavicolare e l'ottava costola sulla linea midassiallare anteriormente, approssimativamente a livello del processo spinoso T10 posteriormente.</li>
	<li>Percuss anteriormente e posteriormente, posizionando il dito sul petto negli spazi intercostali.</li>
	<li>Apprezza la qualità dei suoni delle percussioni. I risultati normali sulla percussione toracica sono:<ol>
<li>Nota di percussione risonante: ascoltata su un normale polmone pieno d'aria.</li>
		<li>Nota di percussione opaca (il suono udito sui tessuti solidi): sopra il fegato nel torace anteriore inferiore destro e sopra il cuore nel torace anteriore sinistro. Quando la percussione dei polmoni suscita questo suono, è indicativo di consolidamento.</li>
		<li>Nota di percussione timpanica (un suono simile a un tamburo quando si percusse su organi cavi): sopra lo spazio del Traube, un'area sovrasente la bolla gastrica e delimitata dalla sesta costola, dalla linea ascellare anteriore e dal margine costale sinistro. L'effusione pleurica sinistra produce un suono di percussione sordo nello spazio di Traube.</li>
	</ol>
</li>
	<li>Si noti la presenza di suoni di percussione patologici. Una nota di percussione "noiosa e sassosa" o piatta suona più noiosa del suono sordo "standard". Assomiglia alla nota di percussione ascoltata sopra la coscia ed è indicativo di un versamento pleurico. Una nota di percussione iper-risonante è un suono di percussione patologico indicativo di polmoni iper-gonfiati da BPCO avanzata, enfisema o pneumotorace.</li>
</ol>3. Auscultazione
<ol><li>Posizionare il paziente: chiedere al paziente di piegarsi in avanti o sedersi in posizione verticale per esaminare posteriormente. Chiedere al paziente di incrociare le braccia o mettere le mani sulle spalle opposte aiuta anche a ottenere la massima esposizione ai campi polmonari.</li>
	<li>Posizionare il diaframma dello stetoscopio sul petto del paziente e chiedere al paziente di fare respiri profondi dentro e fuori attraverso la bocca.</li>
	<li>Auscultate a cinque livelli posteriormente e anteriormente, confrontando fianco a fianco.</li>
	<li>I normali suoni del respiro sono chiamati suoni del respiro vescicolare, che sono suoni a bassa profondità più forti sull'ispirazione e più morbidi alla scadenza. Dovrebbero essere simmetrici posteriormente.</li>
	<li>Si noti la presenza e la posizione di suoni anormali (avventizi) di respiro extra, come crepitii, respiro sibilante, rhonchi, stridor o sfregamento per attrito pleurico (Tabella 1).</li>
	<li>Nota le seguenti caratteristiche di eventuali suoni anomali del respiro (se presenti): volume, qualità, durata e se si verificano durante l'ispirazione o l'espirazione(cioè,tempi nel ciclo respiratorio). Molti suoni del respiro anormali si sentono meglio dopo aver chiesto al paziente di tossire.</li>
	<li>Valutare per la broncofonia, una maggiore trasmissione del suono sul polmone consolidato, quando si chiede al paziente di dire "99" o "1-2-1". L'egofonia è quando un suono "E" cambia in una "A" su un polmone consolidato.</li>
	<li>Valutare per sussurrare pectoriloquy. Mentre si ausculta con lo stetoscopio, chiedere al paziente di sussurrare "99" o "1-2-1". Nel polmone consolidato, il suono sarà effettivamente ascoltato meglio e più chiaramente con lo stetoscopio.</li>

La percussione e l'auscultazione dovrebbero sempre essere eseguite in sequenza ogni volta che si esegue un esame respiratorio completo. Imparare a percuss correttamente richiede tempo e pratica (la pratica può essere fatta su te stesso o su altre superfici, come un tavolo). Imparare la tecnica corretta per la percussione e l'auscultazione del sistema respiratorio è vitale e viene fornito con la pratica su pazienti reali. Imparare la tecnica corretta per la percussione e l'auscultazione dell'apparato respiratorio è vitale per la diagnosi al letto del paziente delle malattie polmonari. Il polmone sinistro ha solo due lobi, superiore e inferiore, separati dalla fessura obliqua sinistra. Poiché i polmoni sono per lo più pieni d'aria che inspiriamo, le percussioni eseguite sulla maggior parte dell'area polmonare producono un suono risonante, che è un suono basso e vuoto. Pertanto, qualsiasi ottusità o iperrisonanza è indicativa di patologia polmonare, come il versamento pleurico o lo pneumotorace, rispettivamente. Anche i suoni respiratori uditi attraverso lo stetoscopio durante l'auscultazione sono peculiari. I due suoni che si avvertono durante la respirazione normale sono bronchiale e vescicolare. Il suono bronchiale, che è più tubolare e cavo, si sente attraverso le grandi vie aeree nella parte anteriore del torace. Considerando che, il suono vescicolare, che è morbido, acuto e frusciante, può essere udito sulla maggior parte dell'area del tessuto polmonare. I suoni anomali del respiro includono crepitii noti anche come rantoli, che sono indicativi di liquido nelle piccole vie aeree. D'altra parte, i rantoli o i ronchi suggeriscono una costrizione o un gonfiore delle vie aeree, che causa un'ostruzione parziale delle vie aeree. Gli sfregamenti pleurici si verificano quando le superfici pleuriche infiammate scivolano l'una contro l'altra durante la respirazione e, infine, lo stridore è causato dall'ostruzione delle vie aeree superiori. Con questa conoscenza di dove e cosa cercare durante la percussione respiratoria e l'auscultazione, discutiamo le fasi procedurali a partire dalla percussione. Chiedi al paziente di sedersi dritto o di piegarsi in avanti. Inizia con la percussione della superficie posteriore. Posiziona la mano non dominante con il dito medio premuto e iperesteso saldamente sulla zona centrale della schiena del paziente. Usa la punta del dito medio della mano dominante per picchiettare con decisione sulla terza falange superiore del dito medio della mano che preme almeno due volte. Ripeti l'operazione a quattro o cinque livelli, confrontando da un lato all'altro. Eseguire la stessa procedura sulla parete toracica anteriore, lavorando dai bordi polmonari inferiori. Sia anteriormente che posteriormente, assicurarsi che il dito medio della mano che preme sia posizionato negli spazi intercostali e non sulle costole. Apprezza la qualità del suono delle percussioni. Battere su un normale polmone pieno d'aria dovrebbe produrre una nota di percussione risonante. Al contrario, la percussione su tessuti solidi come il fegato o il cuore dovrebbe produrre una nota sorda. E le percussioni su spazi vuoti, come lo spazio del Traube, dovrebbero produrre una nota timpanica, che è un suono simile a un tamburo. Infine, passiamo all'auscultazione, che consiste nell'ascoltare i suoni del respiro utilizzando uno stetoscopio. Per iniziare, istruire il paziente a piegarsi in avanti o sedersi in posizione eretta per esaminare posteriormente. Richiedere al paziente di mettere le mani sulle spalle opposte per ottenere la massima esposizione ai campi polmonari. Posiziona il diaframma sulla zona centrale della schiena del paziente e chiedigli di fare respiri profondi dentro e fuori dalla bocca. Auscultare a cinque livelli posteriormente, quindi ripetere la stessa procedura anteriormente, confrontando da un lato all'altro. I normali suoni respiratori dovrebbero essere simmetrici sia posteriormente che anteriormente; Qualsiasi deviazione è un possibile indicatore di una malattia polmonare. Le ultime tre fasi dell'auscultazione sono test che mirano a identificare il consolidamento polmonare. Il primo di questi test è valutare la broncofonia. Chiedi al paziente di dire "99", mentre ausculta la zona del torace. Un aumento della trasmissione del suono indica un polmone consolidato. Il secondo è valutare l'egofonia. Chiedi al paziente di dire "E". Quando un suono "E" cambia in una "A" attraverso lo stetoscopio, è un'indicazione di un polmone consolidato. Infine, valuta il pectoriloquio sussurrato. Chiedi al paziente di sussurrare "99". In caso di polmone consolidato, il suono sarà effettivamente udito meglio e più chiaramente attraverso lo stetoscopio. Tutti questi passaggi devono essere eseguiti anche posteriormente in punti diversi per coprire l'intera area polmonare. Al termine dell'esame, ringraziare il paziente e fargli cambiare le spalle. Distinguere tra i suoni uditi durante questa parte dell'esame può occasionalmente sembrare soggettivo, ma la valutazione diventa più chiara e più facile con la pratica, portando a una "diagnosi spot" per molte condizioni polmonari. Respiratory exam skills include percussion and auscultation of the lungs. These techniques help identify lung problems during a physical exam. Percussion uses tapping to assess the chest, and auscultation uses a stethoscope to listen for breath sounds.

Percussion is a useful skill that is often skipped in routine clinical practice. When it is done correctly, it can help a physician detect underlying lung pathology. Auscultation can also give an almost immediate clue to several acute pulmonary conditions, including chronic obstructive pulmonary disease (COPD), asthma, pneumonia, and pneumothorax.

The lung zones used for auscultation match the areas of the lungs beneath the chest wall. The right lung has three lobes: superior, middle, and inferior. The left lung has two lobes: superior and inferior. The superior lobe of the left lung also has a separate projection called the lingula, which is part of the lung map used during exam.
