Cardiac Exam I: Inspection and Palpation

1. Introduzione

</ol>3. Esame periferico
<ol><li>Chiedere al paziente di tenere le mani alzate e valutare quanto segue:<ol>
<li>Ricarica capillare: premere sulla miniatura del paziente con il primo dito mentre si tiene l'altro lato del dito del paziente con il pollice. 	</ol></li>
	<li>Palpare delicatamente l'impulso carotideo con le prime due dita e valutare il volume e il carattere dell'impulso. Un impulso a lenta risalita è un segno di stenosi aortica.</li>

		<li>Usando un righello posizionato verticalmente misura la distanza dall'angolo di Louis alla carta e calcola il JVP aggiungendo 5 cm (la distanza dall'angolo di Louis all'atrio destro) a quel numero.</li>
		<li>Un altro modo per visualizzare la vena giugulare interna è premere delicatamente nel quadrante superiore destro dell'addome, appena sotto il margine costale. Questa manovra induce il cosiddetto reflusso epatogiugulare (spostamento del sangue dai vasi addominali all'atrio destro). Normalmente, un aumento transitorio di JVP può essere osservato prima di una diminuzione. L'aumento sostenuto della JVP si osservata nell'insufficienza cardiaca congestizia e in altre condizioni, come il rigurgito tricuspide e la pericardite costrittiva.</li>
	</ol></li>
</ol>4. Ispezione toracica
<ol><li>Ispezionare il torace del paziente anteriormente e posteriormente per eventuali cicatrici visibili. Cerca qualsiasi evidenza di una cicatrice sternotomia mediana (un segno di chirurgia di bypass coronarico o chirurgia della valvola aortica). </ol>5. Palpazione

Si noti che il battito apicale sarà spostato lateralmente nei casi di cardiomegalia. Quindi, usa il palmo della mano per palpare le quattro zone del cuore nel precordio e la parete toracica superiore sinistra e destra. Nota eventuali vibrazioni o ronzii sotto la mano, che potrebbero indicare brividi. Per completare la palpazione del torace, posizionare la mano sul bordo sternale sinistro.
