Observation and Inspection

L'osservazione avviene come un processo costante durante l'incontro clinico. Molti degli elementi elencati nella procedura vengono in genere eseguiti contemporaneamente e quando si presentano opportunità. La procedura evidenzia le componenti dell'osservazione, ma non ha lo scopo di suggerire una sequenza preferita.
1. Indagine generale
<ol><li>Nota lo stato generale di salute nel paziente. L'aspetto del paziente è coerente con l'età dichiarata? Il paziente appare in forma e sano o debole e fragile?</li>
	<li>Nota il loro livello di coscienza(ad esempio,sveglio, vigile o sonnolento).</li>
	<li>Osservare per i segni di dolore. Nota espressioni facciali, movimenti sorvegliati, diaforesi, ecc.</li>
	<li>Osservare per segni di difficoltà respiratoria. Il paziente può parlare in frasi complete senza difficoltà? Il paziente è "treppiede" (sporgendosi in avanti con le braccia sostenute)? Vengono utilizzati muscoli accessori visibili della respirazione?</li>
	<li>Osserva i segni di disagio emotivo. Il paziente si agita eccessivamente, mostra un rallentamento psicomotorio generalizzato o piange? Il contatto visivo è appropriato?</li>
	<li>Prendi nota di abbigliamento, gioielli, tatuaggi, toelettatura, igiene e qualsiasi altra funzionalità che possa fornire informazioni sulla situazione medica, sociale ed emotiva del paziente.</li>
	<li>Notare eventuali segni di patologia che possono essere evidenti all'osservazione generale, come lesioni cutanee, distribuzione anormale del grasso, deficit uditivi, atrofia muscolare, odori, ecc.</li>
</ol>2. Osservazione organo-specifica
Durante il resto dell'esame fisico, l'osservazione attiva viene eseguita con un esame di ciascun sistema di organi. Per alcuni sistemi di organi, l'ispezione richiede l'uso di attrezzature(ad esempio,otoscopio o oftalmoscopio). Fare riferimento ai video per ciascun sistema di organi per dettagli specifici.
3. Esame della pelle
L'ispezione dettagliata è la componente principale dell'esame cutaneo. Un esame completo della pelle include l'ispezione di tutte le superfici anteriori, posteriori e laterali del corpo e delle mucose. L'ispezione di determinate aree richiede la manipolazione per l'esecuzione dell'esame. Queste aree includono i capelli, il cuoio capelluto, i processi mastoidi, i padiglioni auricolari posteriori, i canali uditivi esterni, le narici, l'ascella, le unghie, la congiuntiva palpebrale, la mucosa orale, gli aspetti inferiori del seno, la pelle sottostante un panno, le superfici dei genitali, la mucosa vaginale e la schisi glutea.
<ol><li>Nota il colore della pelle o della mucosa in ogni sito esaminato. <ol><li>Osservare l'aspetto e il comportamento del paziente, tra cui postura, abbigliamento, espressioni facciali, attività motoria, manierismi, caratteristiche fisiche e reazioni alle domande poste durante l'esame.</li>
	<li>Nota la fluidità, la velocità e il volume del discorso.</li>
	<li>Valutare l'effetto del paziente, compresa la gamma, l'appropriatezza, l'intensità e l'abilità. La valutazione oggettiva dell'effetto dell'esaminatore deve essere confrontata con il rapporto soggettivo dell'umore del paziente, che si ottiene tramite indagine diretta.</li>
	<li>Valuta il processo di pensiero del paziente, che è composto da elementi, come il livello di organizzazione, la presenza di tangenzialità, associazioni sciolte e "volo di idee".</li>
	<li>Valutare il contenuto del pensiero e le percezioni del paziente, anche se questi in genere non sono completati dalla sola osservazione e possono essere necessarie domande specifiche. Il contenuto del pensiero comprende ossessioni, ansie, fobie, pre-occupazione somatica, delusioni e idee di persecuzione, influenza e riferimento. Le percezioni includono allucinazioni, de-realizzazione e de-personalizzazione.</li>
	<li>Nota la funzione cognitiva del paziente. Indizi di anomalie di attenzione, orientamento, memoria, giudizio e intuizione possono emergere se l'esaminatore è in sintonia per cercarli, anche se l'uso di domande specifiche e strumenti convalidati può essere necessario per quantificare i deficit.</li>
	<li>Utilizzare domande specifiche per valutare la suicidalità e l'omicidio.</li>
</ol>5. Osservazioni accessorie
<ol><li>Alcuni luoghi di esame offrono l'opportunità di conoscere i supporti sociali, gli interessi e lo stile di vita di un paziente. Quando si visita un paziente in uno spazio che occupa per più di poche ore(ad esempio,stanza d'ospedale, casa di cura, casa), notare la presenza (o l'assenza) di decorazioni, biglietti di benessere, foto di famiglia, libri, ecc. Per ottenere una comprensione della vita del paziente al di fuori del ruolo del paziente.</li>
	<li>Quando familiari o amici sono presenti con i pazienti, osservare le dinamiche interpersonali. Questa opportunità di osservazione offre importanti informazioni sul paziente. Il familiare parla per il paziente? Il paziente guarda il familiare prima di rispondere alle domande?</li>
	<li>Presta attenzione al modo in cui ti senti in presenza del paziente, in quanto ciò può rivelarsi diagnosticamente utile, specialmente in termini di malattia psichiatrica. Mentre i fornitori devono essere consapevoli delle insidie del controtransfert, se la sensazione che il paziente sta innescando in te non è tipica o facilmente spiegata, potrebbe esserci una spiegazione sottostante nella salute mentale del paziente. Ad esempio, una sensazione insolita di tristezza nel clinico durante l'incontro può portare il clinico a prendere in considerazione una diagnosi di disturbo depressivo maggiore.</li>
</ol>

L'osservazione e l'ispezione sono fondamentali per l'esame fisico e iniziano al primo punto di contatto con un paziente. Mentre l'osservazione e l'ispezione sono spesso usate in modo intercambiabile, l'osservazione è un termine generale che si riferisce all'uso attento dei sensi per ottenere informazioni. L'osservazione e l'ispezione sono fondamentali per qualsiasi esame clinico. Le osservazioni generali iniziano dal primo punto di contatto con qualsiasi paziente e continuano per tutto l'incontro clinico, anche durante la semplice conversazione con il paziente. L'ispezione è più mirata ed è limitata a ciò che si può osservare visivamente mentre si esaminano parti specifiche del corpo come la pelle, gli occhi o le orecchie, a volte con l'aiuto di un'attrezzatura specializzata. Qui, illustriamo le osservazioni generali che un clinico dovrebbe considerare di eseguire durante ogni incontro clinico, seguite da alcune considerazioni relative alle fasi dell'ispezione visiva. Innanzitutto, esaminiamo alcuni passaggi di osservazione generale che un medico dovrebbe tenere a mente quando incontra qualsiasi paziente. Clinical observation is the starting point of a physical examination and begins at the first contact with a patient. Observation is a broad term for using the senses carefully to gather information. Inspection is a narrower term. It refers to what can be seen with the eyes, especially on the surface of the body, rather than to behavior.

Skilled clinicians use more than sight when they examine a patient. They also rely on touch, through palpation and percussion, and on hearing, through auscultation and percussion. Smell can add useful clues as well. It may point to poor personal hygiene, substance use, or metabolic diseases.

Taste is mostly a historical part of medicine. For many centuries, diabetes mellitus was even diagnosed by the sweet taste of urine. Over time, clinicians also develop a sixth sense, often called gut instinct. This skill comes from deliberate practice on thousands of patients over many years. It is based largely on bedside observations and can help predict serious illness.
