Percutaneous Cricothyrotomy

1. Posizionamento del paziente e preparazione per la procedura

Il contesto di questa procedura è spesso la situazione veramente emergente. Come per tutte le procedure emergenti, la vera tecnica sterile viene sacrificata per la rapidità. Ad esempio, è improbabile che la situazione che richiede una cricotirotomia emergente consenta di ingoiare e glorificare sterili.
<ol><li>Stare in testa al letto e identificare l'anatomia del paziente palpando la prominenza laringea e muovendo le dita inferiormente nella depressione sottostante, la membrana cricotiroidea.</li>

Una procedura chirurgica delle vie aeree è indicata quando altre forme di intubazione endotracheale hanno fallito e la ventilazione sta peggiorando o non è possibile. La cricotirotomia è preferita alla tracheotomia a causa del minor rischio di complicanze, dell'anatomia prevedibile della membrana cricotiroidea e della rapidità comparativa con cui la procedura può essere eseguita, anche da professionisti meno esperti. Nel caso della cricotirotomia percutanea, il praticante identifica prima la membrana cricotiroidea da punti di riferimento fisici e fa una piccola incisione verticale della pelle. Un ago a parete sottile da 18 calibri (attaccato a una siringa) perfora la membrana e le vie aeree vengono identificate positivamente quando l'aria viene aspirata nella siringa. Un filo guida viene quindi alimentato attraverso l'ago. I set standard di cricotirotomia includono un catetere delle vie aeree (simile a un tubo tracheostomico) con un dilatatore rigido all'interno del suo lume. La combinazione catetere/dilatatore viene alimentata sul filo guida e il catetere/dilatatore viene posizionato all'interno delle vie aeree. Il dilatatore e il filo guida vengono successivamente rimossi e il catetere viene collegato a un dispositivo a valvola a sacco per la ventilazione.

Le due forme di questa procedura sono la cricotiroidotomia aperta o chirurgica - discussa in un altro video - e la cricotiroidotomia percutanea, di cui parleremo qui. Dopo che il tubo è in posizione, rimuovere il filo guida, collegare il tubo al dispositivo di ventilazione e fissarlo in posizione con le cravatte. Emergency cricothyrotomy provides airway access when endotracheal intubation has failed and ventilation is getting worse or is not possible. This is the feared &quot;can’t intubate, can’t ventilate&quot; situation. In that setting, cricothyrotomy is the surgical airway procedure of choice.

Cricothyrotomy is often chosen over tracheotomy because it has a lower risk of complications. It also uses the cricothyroid membrane, whose anatomy is more predictable. The procedure can be performed more quickly, even by less experienced practitioners.

Cricothyrotomy has traditionally been done in an open form. A percutaneous cricothyrotomy uses the standard Seldinger technique, which places a device into the body with an introducer needle and a guide wire. This approach can be especially useful when the landmarks are harder to identify.

The procedure begins by locating the cricothyroid membrane using physical landmarks and making a small vertical skin incision. A thin-walled 18-gauge needle attached to a syringe is used to pierce the membrane. Air in the syringe confirms that the airway has been entered.

Next, a guide wire is passed through the needle. Standard cricothyrotomy sets include an airway catheter, similar to a tracheostomy tube, with a stiff dilator inside it. The catheter and dilator are advanced over the guide wire into the airway, then the dilator and guide wire are removed, and the catheter is connected to a bag-valve device for ventilation.
