La resistenza agli antibiotici nei batteri si manifesta quando i microrganismi sviluppano la capacità di resistere a farmaci progettati per ucciderli o inibire la loro crescita, rendendo inutili trattamenti un tempo efficaci. Questo fenomeno, guidato dal cambiamento genetico e dalla selezione durante l'esposizione agli antibiotici, rappresenta una minaccia profonda per la medicina moderna. I meccanismi includono enzimi inattivanti di farmaci (ad esempio, β-lattamasi), pompe di efflusso che espellono antibiotici, mutazioni che alterano i bersagli antibiotici, riduzione dell'assorbimento di farmaci e acquisizione di geni di resistenza tramite trasferimento genico orizzontale. La resistenza può svilupparsi de novo tramite mutazione o essere rapidamente diffusa tramite elementi genetici mobili come plasmidi e trasposoni. Antibiotic resistance in bacteria happens when microbes evolve the ability to survive drugs that once killed them or slowed their growth. Genetic change and antibiotic exposure drive this process. Over time, useful treatments can stop working.

Bacteria can resist antibiotics in several ways. Some make drug-inactivating enzymes, such as beta-lactamases. Others use efflux pumps to push antibiotics out of the cell. Resistance can also come from mutations that change the drug target, from lower drug uptake, or from new resistance genes gained through horizontal gene transfer, which is the movement of DNA between organisms.

Resistance may appear through new mutations or spread quickly through mobile genetic elements like plasmids and transposons. Misuse and overuse of antibiotics increase selection pressure, which helps resistant strains survive and spread. This makes antibiotic-resistant infections harder to treat and often forces doctors to use more toxic or more expensive drugs. These infections can also lead to longer hospital stays, higher healthcare costs, and more illness and death.

MRSA, short for methicillin-resistant Staphylococcus aureus, is a well-known example of this process. It acquired the mecA gene through the SCCmec element and makes an altered penicillin-binding protein called PBP2a. Because PBP2a does not bind beta-lactam drugs well, MRSA resists penicillins, cephalosporins, and carbapenems. MRSA first spread in hospitals as HA-MRSA and later in communities as CA-MRSA. It can cause skin, soft-tissue, bloodstream, and pneumonia infections, and it is often treated with vancomycin, daptomycin, or linezolid, although these options have limits.

Other resistant bacteria show the same problem in different forms. Vancomycin-resistant enterococci can gain van genes, such as vanA, and those genes can transfer to S. aureus to create rare but dangerous VRSA strains. Streptococcus pneumoniae can become resistant to penicillin through changes in penicillin-binding proteins and to macrolides through methyltransferase genes. These changes can make pneumonia, meningitis, and otitis media harder to treat and may require higher-tier antibiotics such as cephalosporins or fluoroquinolones.

Antibiotic resistance grows through genetic adaptation and gene exchange. Its impact reaches major pathogens such as MRSA, VRE, VRSA, and drug-resistant S. pneumoniae. Careful antibiotic use, resistance surveillance, infection control, and new treatments are important ways to slow this public health threat.
