General Approach to the Physical Exam

1. Voor de patiëntonderzoek

2. De kamer betreden
<ol>

3. Privacy van de patiënt
<ol>

4. Overweeg uw aanpak
<ol>
	<li>Bepaal welke aspecten van het onderzoek u van plan bent uit te voeren. Dit kan variëren van een uitgebreid onderzoek, zoals u zou doen tijdens een preventief bezoek, tot een meer gerichte onderzoek op basis van klachten van de patiënt en uw klinische verdenking van specifieke ziekten.</li>
	<li>Of het nu een gericht of een uitgebreid onderzoek is, het onderzoek moet op een overlegde, actieve manier worden uitgevoerd, met constante aandacht voor wat men zoekt, in plaats van uitsluitend via een gewoontematige/automatische procedure. Het bereiken van de toestand waarin men zich kan concentreren op de bevindingen, in plaats van alleen op het proces, vereist veel oefening.</li>
	<li>Plan uw onderzoeksvolgorde om efficiëntie en patiëntcomfort te optimaliseren. U moet ernaar streven het herpositioneren van de patiënt te minimaliseren door manoeuvres te groeperen die in een bepaalde positie moeten worden uitgevoerd. Het is handig om een plan in gedachten te hebben voordat u met het onderzoek begint.</li>
</ol>
5. Kleding van de patiënt
<ol>
	<li>Zorg ervoor dat de patiënt geschikt gekleed is voor het geplande onderzoek. Zorg indien nodig voor een jas en sluier voor de patiënt.</li>
</ol>
6. Andere omgevingsoverwegingen
<ol>
	<li>Pas de hoogte van de stoel en onderzoekstafel naar behoefte aan om uw mogelijkheden om manoeuvres uit te voeren te optimaliseren.</li>
	<li>Pas verlichting en omgevingsgeluid indien mogelijk aan.</li>
	<li>De conventionele aanpak van het onderzoek plaatst de onderzoeker aan de rechterkant van de patiënt.</li>
</ol>
7. Componenten van het onderzoek
<ol>
	<li>Het lichamelijk onderzoek is onderverdeeld in de volgende regionale/anatomische componenten: algemeen overzicht; vitale functies; hoofd, ogen, oor, neus, keel (HOONK); nek; borstkas; cardiovasculair; rug; buik; ledematen; neurologisch; musculoskeletaal; huid; borst; genito-urinaire; rectum; lymfeklieren; mentale status. Er is aanzienlijke overlapping tussen de componenten.</li>
	<li>Elk componentonderzoek bestaat uit manoeuvres die de technieken van inspectie, percussie, palpatie en auscultatie gebruiken, die elk in detail worden behandeld in afzonderlijke video's. Nieuwere modaliteiten, zoals beddiagnostisch echografie, worden steeds vaker opgenomen in het lichamelijk onderzoek.</li>

8. Klinisch redeneren
<ol>
	<li>De onderzoeker moet actief afwegen hoe de aanwezigheid of afwezigheid van bepaalde bevindingen de ziektewaarschijnlijkheid beïnvloedt. Ervaren clinici doen dit in real time tijdens het onderzoek.</li>

Het onderzoek van het lichaam is fundamenteel voor de praktijk van de geneeskunde. Sinds de Romeinse tijd hebben artsen de verbinding beschreven tussen veranderingen in de functie van specifieke delen van het lichaam en specifieke ziektetoestanden en hebben ze geprobeerd de wetenschappelijke kennis te verbeteren om de diagnose aan het bed te verbeteren. In dit moderne tijdperk van toenemende technologie binnen de medische diagnostiek, is het echter belangrijk om de rol te overwegen die lichamelijk onderzoek tegenwoordig speelt. Licheamelijk onderzoek blijft een waardevol diagnostisch hulpmiddel; er zijn veel diagnoses die alleen door lichamelijk onderzoek kunnen worden gesteld. Een diagnose gemaakt door laboratorium of beeldvorming wordt zelden gedaan in afwezigheid van bevindingen die aan het bed kunnen worden waargenomen. Terwijl de zorgverlener een anamnese en lichamelijk onderzoek uitvoert, genereren en testen ze actief hypothesen om de toestand van de patiënt te verklaren. De informatie die men verzamelt, vervangt mogelijk niet de noodzaak van testen, maar het hebben van stevige hypothesen stelt de zorgverlener in staat om tests wijzer te bestellen en betere vragen te stellen aan die tests. Ondanks aanzienlijke vooruitgang in medische instrumentatie, blijft lichamelijk onderzoek een waardevol diagnostisch hulpmiddel en de betekenis ervan kan niet worden overschat. Terwijl artsen de geschiedenis beoordelen en een lichamelijk onderzoek uitvoeren, verzamelen ze informatie die leidt tot een stevige hypothese, wat een meer oordeelkundig benaderen bevordert bij het bestellen van tests en de analyse van die tests. Dit heeft op zijn beurt het potentieel om het risico voor de patiënt en de kosten van de gezondheidszorg te verminderen. Het lichamelijk onderzoek kan worden onderverdeeld in de volgende componenten: algemene studie, meting van vitale functies, onderzoek van de neurologische functie en mentale status, onderzoek van het hoofd, ogen, oren, neus, keel, borst, longen, lymfeklieren, cardiovasculair, buik, musculoskeletal, huid, urogenitaal en rectaal. Een chaperonne kan noodzakelijk zijn als een gevoelig onderzoek zoals urogenitaal, rectaal of borstonderzoek moet worden uitgevoerd.Leg de patiënt uit welk lichamelijk onderzoek gaat worden uitgevoerd en vraag om toestemming om door te gaan met het onderzoek. "Nu ik heb uitgelegd wat ik ga doen, mag ik doorgaan met het onderzoek?" Elke onderzoek bestaat uit manoeuvres waarbij de technieken van inspectie, percussie, palpatie en auscultatie worden gebruikt, die elk in detail worden onderzocht in afzonderlijke video's van deze collectie. U moet streven naar minimalisering van het verplaatsen van de patiënt door manoeuvres te groeperen die in een bepaalde positie moeten worden uitgevoerd. Physical examination remains an important part of medicine. Doctors have used the body exam for centuries to connect changes in body function with disease. Even with modern imaging and lab tests, the bedside exam still helps clinicians understand what may be happening in the patient.

Physical examination is not a replacement for tests, and it does not answer every question. Some maneuvers that were once highly valued may have limited usefulness. Still, many conditions can only be found through physical examination, and diagnoses made with labs or imaging usually begin with findings from the bedside.

As a provider performs the history and physical, they form and check ideas, or hypotheses, about the patient’s condition. These ideas help guide which tests to order and which questions to ask next. That can make testing more focused, reduce risk for the patient, and help lower health care costs.

The physical exam also supports the therapeutic relationship. By using careful and respectful touch, the clinician can show attention and sensitivity. This process can build rapport and may help promote healing during the patient encounter.
