Observation and Inspection

Observatie vindt constant plaats tijdens de klinische ontmoeting. Veel van de genoemde punten in de procedure worden meestal gelijktijdig uitgevoerd en wanneer zich kansen voordoen. De procedure benadrukt de componenten van observatie, maar is niet bedoeld om een voorkeursvolgorde voor te stellen.
1. Algemene inspectie
<ol>
	<li>Let op de algemene gezondheidstoestand van de patiënt. Is de verschijning van de patiënt consistent met de vermelde leeftijd? Ziet de patiënt er fit en gezond uit of zwak en broos?</li>
	<li>Let op hun bewustzijnsniveau (bijv. wakker, alert of slaperig).</li>
	<li>Let op tekenen van pijn. Let op gezichtsuitdrukkingen, beschermende bewegingen, diaforese, etc.</li>
	<li>Let op tekenen van ademhalingsproblemen. Kan de patiënt in complete zinnen spreken zonder moeite? Is de patiënt "tripoding" (voorover leunend met de armen gesteund)? Worden zichtbare accessoire ademhalingsspieren gebruikt?</li>
	<li>Let op tekenen van emotionele stress. Fladdert de patiënt overdreven, vertoont algemene psychomotorische vertraging of huilend? Is oogcontact geschikt?</li>
	<li>Let op kleding, sieraden, tatoeages, verzorging, hygiëne en andere kenmerken die inzicht kunnen geven in de medische, sociale en emotionele situatie van de patiënt.</li>
	<li>Let op eventuele tekenen van pathologie die bij algemene observatie kunnen worden waargenomen, zoals huidlaesies, abnormale vetverdeling, gehoorproblemen, spieratrofie, geurtjes, etc.</li>
</ol>
2. Organspecifieke observatie
Tijdens de rest van het lichamelijk onderzoek wordt actieve observatie gedaan met een onderzoek van elk orgaansysteem. Voor sommige orgaansystemen vereist inspectie het gebruik van apparatuur (bijv. otoscoop of oftalmoscoop). Raadpleeg de video's voor elk orgaansysteem voor specifieke details.
3. Huidonderzoek
Gedetailleerde inspectie is de belangrijkste component van het huidonderzoek. Een volledig huidonderzoek omvat inspectie van alle voorste, achterste en laterale lichaamsoppervlakken en slijmvliezen. Inspectie van bepaalde gebieden vereist manipulatie voor onderzoek. Deze gebieden omvatten het haar, de hoofdhuid, de achterliggende delen van de oren, de uitwendige gehoorgangen, de neusgaten, de oksels, de nagels, de conjunctiva van de oogleden, de orale mucosa, de onderste delen van de borsten, de huid onder een pannus, de oppervlakken van de geslachtsdelen, de vaginale mucosa en de bilnaad.

	<li>Let op de kleur van de huid of mucosa op elke onderzochte plaats. Veelvoorkomende bevindingen zijn gebieden met hypo- of hyperpigmentatie, bleekheid (conjunctiva van de oogleden, handpalmen, voetzolen en nagelriemen), cyanose (nagelriemen, lippen en perioraal) en geelzucht (sclerae, huid en slijmvliezen).</li>
	<li>Let op elke onderzochte plaats ook op de mate van hydratatie (droogheid of vettigheid), turgor en textuur van de huid.</li>
</ol>
4. Geestelijke statusonderzoek
<ol>
	<li>Observeer de verschijning en het gedrag van de patiënt, inclusief houding, kleding, gezichtsuitdrukkingen, motorische activiteit, manieren, fysieke kenmerken en reacties op de vragen die tijdens het onderzoek worden gesteld.</li>
	<li>Let op de vloeiendheid, snelheid en volume van de spraak.</li>
	<li>Beoordeel het affect van de patiënt, inclusief het bereik, de geschiktheid, intensiteit en het vermogen. De objectieve beoordeling van het affect door de onderzoeker moet worden vergeleken met het subjectieve rapport van de stemming van de patiënt, dat wordt verkregen via directe vraagstelling.</li>
	<li>Evalueer het denkproces van de patiënt, dat bestaat uit elementen zoals het organisatieniveau, de aanwezigheid van tangentialiteit, losse associaties en "vluchten van ideeën."</li>
	<li>Evalueer de denkinhoud en percepties van de patiënt, hoewel deze doorgaans niet alleen door observatie worden voltooid, en specifiek ondervragen kan vereist zijn. Denkinhoud omvat obsessies, angsten, fobieën, somatische preoccupatie, waanvoorstellingen en ideeën van vervolging, invloed en verwijzing. Percepties omvatten hallucinaties, de-realisatie en de-personalisatie.</li>
	<li>Let op de cognitieve functie van de patiënt. Aanwijzingen voor afwijkingen van aandacht, oriëntatie, geheugen, oordeel en inzicht kunnen naar voren komen als de onderzoeker erop let, hoewel het gebruik van specifieke vragen en gevalideerde instrumenten nodig kan zijn om tekortkomingen te kwantificeren.</li>
	<li>Gebruik specifieke vragen om te beoordelen op suïcidaliteit en homocidaliteit.</li>
</ol>
5. Aanvullende observaties
<ol>
	<li>Bepaalde onderzoekslocaties bieden mogelijkheden om meer te weten te komen over de sociale ondersteuning, interesses en levensstijl van een patiënt. Wanneer u een patiënt bezoekt in een ruimte die hij/zij meer dan een paar uur bezet (bijv. ziekenhuiskamer, verpleeghuis, thuis), let op de aanwezigheid (of afwezigheid) van decoraties, get-well kaarten, familiefoto's, boeken, etc. om een beter begrip te krijgen van het leven van de patiënt buiten de rol van patiënt.</li>
	<li>Wanneer familieleden of vrienden aanwezig zijn bij patiënten, observeer de interpersoonlijke dynamiek. Deze gelegenheid voor observatie biedt belangrijke informatie over de patiënt. Spreekt het familielid voor de patiënt? Kijkt de patiënt naar het familielid voordat hij/zij antwoordt op vragen?</li>
	<li>Let op hoe u zich voelt in de aanwezigheid van de patiënt, aangezien dit diagnostisch nuttig kan zijn, vooral in termen van psychiatrische aandoeningen. Hoewel zorgverleners zich bewust moeten zijn van de valkuilen van tegenoverdracht, als het gevoel dat de patiënt bij

Observatie en inspectie is fundamenteel voor lichamelijk onderzoek en begint bij het eerste contact met een patiënt. Hoewel observatie en inspectie vaak door elkaar worden gebruikt, is observatie een algemene term die verwijst naar het zorgvuldig gebruik van iemands zintuigen om informatie te verkrijgen. Inspectie is een handeling beperkt tot wat men visueel kan waarnemen en verwijst bij lichamelijk onderzoek doorgaans naar bevindingen op het oppervlak van het lichaam, in plaats van naar gedragingen. Bekwame clinici gebruiken al hun zintuigen om hen te helpen hun patiënten te begrijpen, voornamelijk vertrouwend op zicht, aanraking (percussie en palpatie) en gehoor (percussie en auscultatie). Reuk kan ook belangrijke diagnostische informatie bieden tijdens de patiëntenontmoeting (bijv. persoonlijke hygiëne, middelengebruik of metabole ziekten). Gelukkig is het gevoel voor smaak grotendeels een historisch overblijfsel in de geneeskunde, hoewel het interessant is op te merken dat diabetes mellitus gedurende vele eeuwen werd gediagnosticeerd door de zoete smaak van de urine. Observatie en inspectie zijn fundamenteel voor elk klinisch onderzoek. Algemene observaties beginnen bij het eerste contact met een patiënt en gaan door tijdens het hele klinische onderzoek, zelfs tijdens een gesprek met de patiënt. Inspectie is meer doelgericht en beperkt tot wat men visueel kan waarnemen bij het onderzoeken van specifieke lichaamsdelen zoals huid, ogen of oren, soms met behulp van gespecialiseerde apparatuur.Hier illustreren we de algemene observaties die een kliniek zou moeten overwegen om uit te voeren tijdens elk klinisch onderzoek, gevolgd door enkele overwegingen met betrekking tot de visuele inspectiestappen.Laten we eerst enkele algemene observatiestappen doornemen waar een arts rekening mee moet houden bij het ontmoeten van een patiënt. Deze observaties kunnen op elk moment tijdens het onderzoek worden gemaakt.Tijdens de eerste gesprek wanneer een patiënt hun ziekte uitlegt, let op de toestand van hun fysieke gezondheid en vraag uzelf af: "Is de verschijning van de patiënt consistent met de opgegeven leeftijd? Ziet de patiënt er fit en gezond uit of zwak en broos? Is de patiënt wakker en alert, of slaperig?" Tegelijkertijd, beoordeel hun mentale status en affect, en onderzoek of hun gedachten georganiseerd zijn.Tijdens het gesprek kan een arts ook de gedachten en percepties van de patiënt evalueren. Kijk ook naar tekenen van emotionele stress zoals overmatig scharrelen of onvoldoende oogcontact. Let ook op hoe u zich voelt in de aanwezigheid van de patiënt, omdat dit diagnostisch nuttig kan zijn. Een ongewoon gevoel van verdriet bij de arts kan leiden tot het overwegen van een diagnose van een ernstige depressieve stoornis. Bovendien, observeer tijdens het gesprek tekenen van pijn door naar gezichtsuitdrukkingen te kijken en door op te merken of de patiënt beschermende bewegingen of autonome tekenen zoals diaforesis vertoont. Let ook op de hygiëne, kleding en make-up van de patiënt voor aanvullende aanwijzingen die kunnen helpen bij de diagnose. Kijk ook naar tekenen van ademhalingsproblemen. Let op of de patiënt in volle zinnen spreekt zonder enige moeilijkheid. Observeer of de patiënt "tripoding" is, wat voorwaarts leunen met de armen ondersteund is. Merk op of zichtbare accessoire ademhalingsspieren worden gebruikt, wat gebruikelijk is bij gevallen van ademhalingsproblemen.Al deze eenvoudige observaties kunnen samen substantiële inzicht geven in de fysieke en mentale toestand van de patiënt en kunnen helpen bij de diagnose van hun ziekte.Nu u weet wat algemene observaties zijn, laten we kijken naar inspectie, die cruciaal is voor een uitgebreid en orgaanspecifiek lichamelijk onderzoek.Leg eerst de patiënt de doeleinden van de inspectie uit en verkrijg hun toestemming. Zoals eerder vermeld, omvat dit het visueel observeren van lichaamsoppervlakken om te controleren op eventuele afwijkingen. Tijdens een uitgebreid huidonderzoek is inspectie van alle voorste, laterale en achterste lichaamsoppervlakken en slijmvliezen noodzakelijk. Let op de kleur van de huid of mucosa op elke onderzochte plaats. Veelvoorkomende bevindingen zijn gebieden van hypo- of hyperpigmentatie, bleekheid, cyanose, geelzucht. Controleer ook de mate van hydratatie, turgor en textuur.Meestal bestaat inspectie erin om scherp naar verschillende delen van het lichaamsoppervlak te kijken met het blote oog. Soms is speciale apparatuur nodig voor de inspectie van structuren die ontoegankelijk zijn voor het blote oog. Bijvoorbeeld, een otoscoop is noodzakelijk voor de inspectie van trommelvliezen. Sommige delen van het lichaam vereisen inspectie met manipulatie. Bijvoorbeeld, voor hoofdhuidonderzoek, moet een arts mogelijk door het haar van de patiënt manipuleren om het oppervlak bloot te leggen. Clinical observation is the starting point of a physical examination and begins at the first contact with a patient. Observation is a broad term for using the senses carefully to gather information. Inspection is a narrower term. It refers to what can be seen with the eyes, especially on the surface of the body, rather than to behavior.

Skilled clinicians use more than sight when they examine a patient. They also rely on touch, through palpation and percussion, and on hearing, through auscultation and percussion. Smell can add useful clues as well. It may point to poor personal hygiene, substance use, or metabolic diseases.

Taste is mostly a historical part of medicine. For many centuries, diabetes mellitus was even diagnosed by the sweet taste of urine. Over time, clinicians also develop a sixth sense, often called gut instinct. This skill comes from deliberate practice on thousands of patients over many years. It is based largely on bedside observations and can help predict serious illness.
