2. Componenten van het onderzoek

3. Algemene overwegingen
<ol>
	<li>Om zichzelf langzaam in de persoonlijke ruimte van de patiënt te verplaatsen en de comfort van de patiënt met de aanraking van de clinicus te peilen, beginnen veel providers het onderzoek met de handen. Voer een zachte palpatie van de nagelbedden uit met uw vingertoppen (om capillaire vulling en bleekheid te beoordelen) en palpatie van de radiale pols met uw vingerkussentjes om de eerste contact op een niet-bedreigende manier te initiëren.</li>
	<li>Gebruik de vingerkussentjes bij het uitvoeren van de meeste palpatie, met de volgende opmerkelijke uitzonderingen:
	<ol>
		<li>Gebruik de vingertoppen om de nagelbedden, leverrand en cervix te palperen.</li>
		<li>Gebruik de ulnaire oppervlakte van de handen bij het beoordelen van tactiele fremitus.</li>
		<li>Gebruik de palmaire oppervlakte van de vingers en/of handen voor het beoordelen van de borstuitzetting, precordium voor PMI/heffen/opheffen/gejuich, het uitvoeren van lichte en diepe palpatie van de buik en krachttesten tegen weerstand.</li>
		<li>Gebruik de dorsale oppervlakte van de handen voor het beoordelen van de relatieve temperatuur van de huid (meestal in vergelijking met een ander deel van het lichaam van de patiënt).</li>
	</ol>
	</li>
	<li>Wees u bewust van de druk die wordt gebruikt voor palpatie, die varieert op basis van de structuren die worden onderzocht. Bijvoorbeeld, overmatige druk kan een pols afsluiten, ongemak veroorzaken en de bruikbaarheid beperken. Onvoldoende druk kan het vermogen tot palpatie van diepe structuren beperken (bijvoorbeeld aorta).</li>
	<li>Wees bewust van de duur wanneer druk wordt uitgeoefend, wat varieert op basis van de structuren die worden onderzocht. Bijvoorbeeld, gespannen pittig oedeem van de onderste ledematen kan worden gemist als de onderzoeker niet gedurende ten minste een paar seconden constante druk uitoefent. Evenzo kan een blancherende uitslag niet als zodanig worden geïdentificeerd zonder voldoende duur van druk. Omgekeerd veroorzaakt overmatige duur van palpatie bij een patiënt met peritoneale tekenen onnodig ongemak zonder de diagnostische opbrengst te verhogen.</li>
	<li>Overweeg om gebieden met bekend ongemak naar het einde van het onderzoek te palperen, waarbij u duidelijk maakt dat dit in het belang van hun comfort wordt gedaan. Bijvoorbeeld, als een patiënt acute pijn in de rechter knie heeft, kan het onderzoek beginnen aan de linker onderste ledemaat, gevolgd door onderzoek van de rechter enkel en heup, waardoor de patiënt minder beschermd is wanneer de pijnlijke rechter knie wordt onderzocht.</li>
	<li>Toon empathie terwijl u erkent dat bepaalde delen van het onderzoek de patiënt ongemak kunnen veroorzaken. Het lichamelijk onderzoek vereist het gebruik van alle zintuigen van de zorgverlener om informatie over de patiënt te verkrijgen. Het tastzintuig wordt gebruikt om diagnostische informatie te verkrijgen door middel van palpatie.
De specifieke delen van de hand van de onderzoeker die voor palpatie worden gebruikt, verschillen op basis van het lichaamsdeel dat wordt onderzocht. Vanwege hun dichte sensorische innervatie zijn de vingerkussentjes nuttig voor fijn onderscheid (bijv. het definiëren van de randen van massa's, lymfeklieren) (Figuur 1). Het dorsale oppervlak van de hand geeft een grof gevoel van relatieve temperatuur (Figuur 2). De palmaire oppervlakken van de vingers en handen zijn het meest bruikbaar voor het onderzoeken van grote lichaamsdelen (bijv. buik) (Figuur 3). Trilling wordt het best waargenomen met het ulnaire oppervlak van de handen en 5de vingers (bijv. tactiel fremitus) (Figuur 4).
Terwijl palpatie fundamenteel is voor het diagnostische aspect van het lichamelijk onderzoek, is het ook belangrijk om de rol te erkennen die aanraking speelt bij het communiceren van zorg en comfort tijdens de patiëntenontmoeting. Patiënten ervaren aanraking door een zorgverlener over het algemeen op een positieve manier, en hun percepties van een zorgverlener kunnen worden beïnvloed door het bekwame gebruik van aanraking tijdens klinische ontmoetingen.1 Fysiek contact is geassocieerd met veranderingen in hormonale en neurotransmitterniveaus, specifiek afname van cortisol en toename van serotonine.2
Daarom heeft de arts door het zorgvuldige gebruik van palpatie, en aanraking in het algemeen, tijdens het lichamelijk onderzoek de mogelijkheid om belangrijke diagnostische informatie te verkrijgen, terwijl rapport wordt ontwikkeld en genezing wordt bevorderd.

Was je handen met zeep en water of breng een topische desinfecterende oplossing aan. Verwarm je handen met warm water of door ze tegen elkaar te wrijven. Als er specifieke infectiecontrolemaatregelen nodig zijn, draag dan de beschermende uitrusting en leg de patiënt uit waarom het nodig is. Onthoud: jasjes, handschoenen en maskers kunnen een barrière vormen voor het opbouwen van een relatie met de patiënt.Om de comfort van de patiënt met de aanraking van de arts te peilen, kun je het onderzoek beginnen met de handen. Voer eerst palpatie uit van de nagelbedden met je vingertoppen om capillaire vulling en bleekheid te beoordelen. Palpeer vervolgens de radiale pols met je vingerpads. Ga dan naar het anatomische gebied van belang, in dit geval het buikgebied. Gebruik drapeertechnieken om de toegang te optimaliseren, terwijl je de bescheidenheid van de patiënt in balans houdt. Vraag vervolgens de patiënt om het gebied van ongemak aan te wijzen. Palpation uses touch to collect diagnostic information during the physical exam. It is one of the ways a healthcare provider uses the senses to learn more about a patient. The part of the hand used for palpation changes depending on the body area being examined.

Finger pads are best for fine detail. Their dense sensory nerves help the examiner feel small features, such as the borders of masses or lymph nodes. The back of the hand, also called the dorsal surface, gives a rough sense of relative temperature. This makes it useful for checking warmth.

The palmar surfaces of the fingers and hands are better for surveying larger areas of the body, such as the abdomen. When vibration needs to be felt, the ulnar surface of the hand and the fifth finger work best. This is useful for checking tactile fremitus, which is vibration felt through the chest wall.

Palpation also has a human side. Touch can communicate care and comfort during a patient encounter. Patients generally view healthcare provider touch positively, and skilled touch can shape how they perceive the provider. Physical contact has also been linked to lower cortisol and higher serotonin. Used well, palpation helps with diagnosis, rapport, and healing.
