Bone Remodeling

- [Instructeur] Zelfs in het volwassen skelet zijn botten actieve weefsels die voortdurend veranderingen in hun architectuur ondergaan. Dit proces, dat botremodellering wordt genoemd, bestaat uit gelijke fasen van botresorptie, de verwijdering ervan, en het aanbrengen van nieuw weefsel. Wanneer rijpe botcellen, osteocyten, mechanische stress voelen, signaleren ze cellen naar de plaats van het bot, bijvoorbeeld de uiteinden van het dijbeen, die om de zes maanden worden vervangen.

Tijdens de resorptie, houdt één celtype, de osteoclasten, zich vast aan het oppervlak. Door het afscheiden van lysosomale enzymen en waterstofprotonen, kunnen ze de organische componenten afbreken, waardoor erosieholtes ontstaan terwijl ze de oude of beschadigde matrix verteren. Calcium wordt ook afgegeven aan het bloed en speelt een rol in de hormonale feedback loops.

Zodra het materiaal is opgelost, vernietigt de osteoclast zichzelf door middel van apoptose, wat verdere vernietiging van het bot voorkomt. Om de omkering te starten, verschijnen mononucleaire cellen op het oppervlak in voorbereiding op de volgende stap. In de vormingsfase, bewegen de osteoblasten zich in de holte en deponeren nieuw botweefsel via de ossificatie van de organische matrix die bekend staat als osteoïde.

Dit deel bevat collageenvezels die bijdragen aan de structuur en flexibiliteit. Na de matrixsynthese kunnen de osteoblasten zich differentiëren in afgeplatte botbekledende cellen of als osteocyten in het bot begraven worden.

Botremodellering is een continu en evenwichtig proces van botresorptie door osteoclasten en botvorming door osteoblasten. Bij volwassenen helpt het de botmassa en calciumhomeostase te behouden. Hoewel mechanische stress de omzet kan stimuleren als onderdeel van het normale onderhouds- en reparatieproces, reguleren verschillende hormonen ook de botremodellering.

<h4>Hormonale controle van botremodellering</h4>

Parathyroïdhormoon (PTH) handhaaft de homeostatische controle van de calciumspiegels in het bloed door de botresorptie te reguleren. PTH komt vrij uit de bijschildklieren als reactie op een laag calciumgehalte in het bloed. Het stimuleert osteoblasten om immuunmoleculen te produceren die de differentiatie van voorlopercellen tot osteoclasten bevorderen. Activering van osteoclasten bevordert de botresorptie, waardoor de gemineraliseerde botmatrix wordt afgebroken en calcium in het bloed vrijkomt. Wanneer de calciumspiegels in het bloed zijn hersteld, voorkomt een negatieve feedbacklus verdere afgifte van PTH.

<h4>Osteoporose</h4>

Osteoporose is een ziekte waarbij de botresorptie groter is dan de botvorming, wat resulteert in een verminderde botdichtheid. Osteoporose komt vaker voor bij vrouwen, vooral na de menopauze. Dit komt door de cruciale rol die het vrouwelijke geslachtshormoon – oestrogeen – speelt bij de hermodellering van botten. Oestrogeen beperkt de vorming van osteoclasten en bevordert hun vernietiging via apoptose. Dit zorgt ervoor dat de botvorming hoger is dan de botresorptie. De oestrogeenspiegels dalen echter ernstig bij vrouwen in de menopauze. Daarom overtreft de botresorptie de creatie ervan, wat leidt tot verlies van botsterkte en een verhoogd risico op fracturen.

Bone turnover is a continuous process in which old bone is broken down and new bone is built. Osteoclasts carry out bone resorption, and osteoblasts carry out bone formation. In adults, this balanced cycle helps maintain bone mass and calcium homeostasis. Mechanical stress can also stimulate turnover as part of normal maintenance and repair.

Parathyroid hormone, or PTH, helps control blood calcium levels by increasing bone resorption when calcium is low. It is released from the parathyroid glands in response to low calcium in the blood. PTH stimulates osteoblasts to produce immune molecules that promote the differentiation of precursor cells into osteoclasts. Once osteoclasts become active, they break down the mineralized bone matrix and release calcium into the blood. When calcium levels return to normal, a negative-feedback loop reduces further PTH release.

Osteoporosis develops when bone resorption becomes greater than bone formation, which lowers bone density. It is more common in women, especially after menopause. Estrogen, the female sex hormone, helps limit the formation of osteoclasts and also promotes their destruction by apoptosis, or programmed cell death. When estrogen levels drop after menopause, bone resorption can outpace bone creation. This weakens bone strength and raises the risk of fractures.
