Obserwacja zachodzi jako ciągły proces podczas spotkania klinicznego. Wiele elementów wymienionych w procedurze jest zazwyczaj wykonywanych jednocześnie i gdy pojawiają się możliwości. Procedura uwypukla elementy obserwacji, ale nie ma na celu zasugerowania preferowanej sekwencji.
1. Ogólna ankieta
<ol>
	<li>Zwróć uwagę na ogólny stan zdrowia pacjenta. Czy wygląd pacjenta jest zgodny z podanym wiekiem? Czy pacjent wydaje się sprawny i zdrowy, czy słaby i wątły?</li>
	<li>Zwróć uwagę na ich poziom świadomości (np. przebudzony, czujny lub senny).</li>
	<li>Obserwuj oznaki bólu. Zwróć uwagę na mimikę twarzy, ostrożne ruchy, pocenie się itp.</li>
	<li>Obserwuj, czy nie ma oznak niewydolności oddechowej. Czy pacjent może bez trudu mówić pełnymi zdaniami? Czy pacjent jest "na trójnogu" (pochyla się do przodu z podpartymi ramionami)? Czy używane są widoczne mięśnie pomocnicze układu oddechowego?</li>
	<li>Obserwuj oznaki niepokoju emocjonalnego. Czy pacjent nadmiernie się wierci, wykazuje uogólnione spowolnienie psychomotoryczne lub płacze? Czy kontakt wzrokowy jest odpowiedni?</li>
	<li>Zwróć uwagę na odzież, biżuterię, tatuaże, pielęgnację, higienę i wszelkie inne cechy, które mogą zapewnić wgląd w medyczną, społeczną i emocjonalną sytuację pacjenta.</li>
	<li>Zwróć uwagę na wszelkie oznaki patologii, które mogą być widoczne podczas ogólnej obserwacji, takie jak zmiany skórne, nieprawidłowy rozkład tkanki tłuszczowej, deficyty słuchu, zanik mięśni, zapachy itp.</li>
</ol>
2. Obserwacja specyficzna dla narządów
Podczas pozostałej części badania fizykalnego, przeprowadzana jest aktywna obserwacja z badaniem każdego układu narządu. W przypadku niektórych układów narządowych badanie wymaga użycia sprzętu (np. otoskopu lub oftalmoskopu). Zapoznaj się z filmami dla każdego układu narządów, aby uzyskać szczegółowe informacje.
3. Badanie skóry
Szczegółowa inspekcja jest głównym elementem badania skóry. Pełne badanie skóry obejmuje kontrolę wszystkich przednich, tylnych i bocznych powierzchni ciała oraz błon śluzowych. Inspekcja niektórych obszarów wymaga manipulacji w celu przeprowadzenia badania. Obszary te obejmują włosy, skórę głowy, wyrostki sutkowate, tylne małżowiny uszne, zewnętrzne kanały słuchowe, nozdrzyska, pachy, paznokcie, spojówkę powiekową, błonę śluzową jamy ustnej, dolne aspekty piersi, skórę pod łuską, powierzchnie narządów płciowych, błonę śluzową pochwy i szczelinę pośladkową.

Typowe objawy obejmują obszary hipo- lub hiperpigmentacji, bladość (spojówka powiekowa, dłonie, podeszwy i łożyska paznokci), sinica (łożyska paznokci, wargi i okolice ust) oraz żółtaczka (twardówka, skóra i błony śluzowe).</li>
	<li>W każdym badanym miejscu zwróć również uwagę na stopień nawilżenia (tj. suchość lub tłustość), turgor i teksturę skóry.</li>

4. Badanie stanu psychicznego
<ol>
	<li>Obserwuj wygląd i zachowanie pacjenta, w tym postawę, ubiór, mimikę twarzy, aktywność motoryczną, maniery, cechy fizyczne i reakcje na pytania zadawane podczas badania.</li>
	<li>Zwróć uwagę na płynność, tempo i głośność mowy.</li>
	<li>Oceń afekt pacjenta, w tym zakres, odpowiedniość, intensywność i zdolność. Obiektywna ocena afektu przez badającego powinna być porównana z subiektywnym raportem nastroju pacjenta, który uzyskuje się poprzez bezpośrednie dochodzenie.</li>
	<li>Oceń proces myślowy pacjenta, który składa się z takich elementów, jak poziom organizacji, obecność styczności, luźne skojarzenia i "lot pomysłów".</li>
	<li>Oceń treść myśli i spostrzeżenia pacjenta, chociaż zazwyczaj nie są one uzupełniane przez samą obserwację i mogą być wymagane konkretne pytania. Treść myśli obejmuje obsesje, lęki, fobie, zaabsorbowanie somatyczne, urojenia oraz idee prześladowań, wpływów i odniesień. Percepcje obejmują halucynacje, de-realizację i depersonalizację.</li>
	<li>Zwróć uwagę na funkcje poznawcze pacjenta. Wskazówki dotyczące nieprawidłowości uwagi, orientacji, pamięci, osądu i wglądu mogą się pojawić, jeśli egzaminator jest dostrojony do ich szukania, chociaż użycie konkretnych pytań i zatwierdzonych instrumentów może być konieczne do ilościowego określenia deficytów.</li>

Ogólne obserwacje rozpoczynają się w pierwszym punkcie kontaktu z każdym pacjentem i są kontynuowane przez całe spotkanie kliniczne, nawet podczas samej rozmowy z pacjentem. Inspekcja jest bardziej ukierunkowana na cel i ogranicza się do tego, co można zaobserwować wizualnie podczas badania określonych części ciała, takich jak skóra, oczy czy uszy, czasami za pomocą specjalistycznego sprzętu. W tym miejscu ilustrujemy ogólne obserwacje, które lekarz kliniczny powinien rozważyć podczas każdego spotkania klinicznego, a następnie kilka kwestii związanych z etapami kontroli wizualnej. Najpierw omówmy kilka ogólnych kroków obserwacji, o których klinicysta powinien pamiętać podczas spotkania z każdym pacjentem. Spostrzeżeń tych można dokonać w dowolnym momencie badania. Jednocześnie oceń ich stan psychiczny i afekt oraz zbadaj, czy ich myśli są zorganizowane. Podczas rozmowy lekarz może również ocenić treść myśli i spostrzeżenia pacjenta. Ponadto szukaj oznak niepokoju emocjonalnego, takich jak nadmierne wiercenie się lub nieodpowiedni kontakt wzrokowy. Zwróć również uwagę na to, jak się czujesz w obecności pacjenta, ponieważ może to okazać się przydatne diagnostycznie. Nietypowe uczucie smutku u klinicysty może prowadzić do rozważenia rozpoznania dużego zaburzenia depresyjnego. Niektóre obszary ciała wymagają oględzin z manipulacją. Na przykład w przypadku badania skóry głowy lekarz może być zmuszony do manipulowania włosami pacjenta, aby odsłonić powierzchnię. Clinical observation is the starting point of a physical examination and begins at the first contact with a patient. Observation is a broad term for using the senses carefully to gather information. Inspection is a narrower term. It refers to what can be seen with the eyes, especially on the surface of the body, rather than to behavior.

Skilled clinicians use more than sight when they examine a patient. They also rely on touch, through palpation and percussion, and on hearing, through auscultation and percussion. Smell can add useful clues as well. It may point to poor personal hygiene, substance use, or metabolic diseases.

Taste is mostly a historical part of medicine. For many centuries, diabetes mellitus was even diagnosed by the sweet taste of urine. Over time, clinicians also develop a sixth sense, often called gut instinct. This skill comes from deliberate practice on thousands of patients over many years. It is based largely on bedside observations and can help predict serious illness.
