Nocycepcja, ogólnie określana jako ból, to proces, w którym szkodliwy bodziec jest przenoszony przez obwodowy i ośrodkowy układ nerwowy w celu ostrzeżenia przed potencjalną szkodą lub urazem.

Po wykryciu szkodliwego sygnału aktywowane są nocyceptory, czyli wolne zakończenia nerwowe. Pobliskie komórki tuczne uwalniają substancje chemiczne, takie jak histamina, podczas gdy makrofagi wydzielają cytokiny, małe białka, które są kluczowymi czynnikami sygnalizacji immunologicznej.

Jednocześnie aksony nocyceptorów przekazują sygnał do rogu grzbietowego rdzenia kręgowego, głównie za pośrednictwem dwóch różnych włókien bólowych.

Jako pierwsze aktywowane są cienko zmielinizowane włókna A-delta, które z bardzo dużą prędkością przewodzą natychmiastowy ostry i dobrze zlokalizowany ból, aby umożliwić organizmowi wycofanie się ze szkodliwego bodźca.

Innym rodzajem aksonów są włókna C, które przewodzą powoli, ponieważ są niezmielinizowane i przenoszą długotrwały palący ból.

Z rdzenia kręgowego sygnały bólowe krzyżują się i przemieszczają do pnia mózgu, a następnie do wzgórza i kory somatosensorycznej, gdzie sygnały bólowe są interpretowane w celu określenia lokalizacji urazu.

Dodatkowe obszary mózgu przetwarzają ból, w szczególności struktury korowo-limbiczne, w tym ciało migdałowate i kora przedczołowa, które odnoszą się do wspomnień emocjonalnych i funkcji poznawczych. Ostatecznie wielorakie interakcje w mózgu przyczyniają się do subiektywnego postrzegania, którego doświadczają poszczególne osoby.

Nociception

Nocycepcja — zdolność odczuwania bólu — jest niezbędna dla przetrwania organizmu i ogólnego dobrostanu. Szkodliwe bodźce, takie jak przeszywający ból spowodowany ostrym przedmiotem, ciepło otwartego płomienia lub kontakt z żrącymi substancjami chemicznymi, są najpierw wykrywane przez receptory czuciowe, zwane nocyceptorami, zlokalizowane na zakończeniach nerwowych. Nocyceptory wytwarzają kanały jonowe, które przekształcają szkodliwe bodźce w sygnały elektryczne. Kiedy te sygnały docierają do mózgu za pośrednictwem neuronów czuciowych, są odbierane jako ból. W ten sposób ból pomaga organizmowi uniknąć szkodliwych bodźców.

Układ odpornościowy odgrywa zasadniczą rolę w patologii bólu. Po napotkaniu szkodliwych bodźców komórki odpornościowe, takie jak komórki tuczne i makrofagi, obecne w miejscu urazu, uwalniają zapalne substancje chemiczne, takie jak cytokiny, chemokiny, histaminy i prostaglandyny. Te substancje chemiczne przyciągają inne komórki odpornościowe, takie jak monocyty i komórki T, do miejsca uszkodzenia. Pobudzają także nocyceptory, powodując hiperalgezję — bardziej intensywną reakcję na wcześniej bolesny bodziec lub alodynię — bolesną reakcję na zwykle nieszkodliwy bodziec, taki jak lekki dotyk. Takie uwrażliwienie na ból pomaga chronić uszkodzone miejsce podczas gojenia.

W niektórych przypadkach ból przestaje pełnić rolę ostrego systemu ostrzegawczego, jeśli uczulenie nie ustępuje z czasem. Ból przewlekły — uporczywy lub nawracający ból trwający dłużej niż trzy miesiące — często towarzyszy stanom zapalnym, takim jak reumatoidalne zapalenie stawów. Niesteroidowe leki przeciwzapalne (NLPZ), takie jak aspiryna i ibuprofen, zmniejszają ból poprzez hamowanie syntezy cząsteczek zapalnych prostaglandyn. Jednakże NLPZ i opioidy stosowane obecnie w leczeniu bólu mają poważne skutki uboczne i mogą powodować uzależnienie. Dlatego zrozumienie mechanizmów leżących u podstaw patologii bólu może pomóc w opracowaniu skuteczniejszych leków tłumiących odczuwanie bólu przy mniej poważnych negatywnych skutkach ubocznych.

Pain signaling begins when the body detects a harmful stimulus. Sharp objects, open flames, and corrosive chemicals can trigger this response. Sensory receptors called nociceptors, which sit on nerve endings, detect these noxious stimuli and use ion channels to turn them into electrical signals.

Those electrical signals travel through sensory neurons to the brain, where they are experienced as pain. This warning system helps an organism avoid damage and stay safe. Pain is therefore an important part of survival and overall well-being.

The immune system also shapes pain after an injury. Mast cells and macrophages at the injury site release inflammatory chemicals such as cytokines, chemokines, histamines, and prostaglandins. These signals attract more immune cells, including monocytes and T cells, and they also stimulate nociceptors.

This immune response can increase pain sensitivity. Hyperalgesia is a stronger response to a stimulus that was already painful. Allodynia is pain caused by something that is usually not painful, such as light touch. These changes help protect the injured area during healing.

Pain can become a problem when sensitization does not fade. Chronic pain is persistent or recurring pain that lasts longer than three months. It often occurs with inflammatory diseases such as rheumatoid arthritis. NSAIDs, including aspirin and ibuprofen, reduce pain by blocking the production of prostaglandins. However, NSAIDs and opioids can cause serious side effects and may lead to addiction, so researchers continue to study pain mechanisms to find safer drugs.
