Observation and Inspection

1. Pesquisa geral
<ol><li>Observe o estado geral de saúde do paciente. A aparência do paciente é consistente com a idade declarada? O paciente parece apto e saudável ou fraco e frágil?</li>
	<li>Observe seu nível de consciência (por exemplo,acordado, alerta ou sonolento).</li>
	<li>Observe sinais de dor. Note expressões faciais, movimentos protegidos, diaforese, etc.</li>
	<li>Observe sinais de problemas respiratórios. O paciente pode falar em frases completas sem dificuldade? O paciente é "tripé" (inclinando-se para a frente com os braços apoiados)? Os músculos acessórios visíveis da respiração estão sendo usados?</li>
	<li>Observe sinais de sofrimento emocional. O paciente está se mexendo excessivamente, exibindo desaceleração psicomotora generalizada ou chorando? O contato visual é apropriado?</li>
	<li>Anote roupas, joias, tatuagens, arrumação, higiene e quaisquer outras características que possam fornecer insights sobre a situação médica, social e emocional do paciente.</li>
	<li>Observe quaisquer sinais de patologia que possam ser evidentes na observação geral, como lesões cutâneas, distribuição anormal de gordura, déficits auditivos, atrofia muscular, odores, etc.</li>
</ol>2. Observação específica de órgãos
Durante o restante do exame físico, a observação ativa é feita com um exame de cada sistema de órgãos. Para alguns sistemas de órgãos, a inspeção requer o uso de equipamentos (por exemplo,otoscópio ou oftalmóscópio). Consulte os vídeos de cada sistema de órgãos para obter detalhes específicos.
3. Exame de pele
A inspeção detalhada é o principal componente do exame de pele. Um exame completo da pele inclui a inspeção de todas as superfícies do corpo anterior, posterior e lateral e membranas mucosas. </ol>4. Exame de estado mental
<ol><li>Observe a aparência e o comportamento do paciente, incluindo postura, vestido, expressões faciais, atividade motora, maneirismos, características físicas e reações às perguntas feitas durante o exame.</li>
	<li>Note a fluência, taxa e volume de fala.</li>
	<li>Avalie o afeto do paciente, incluindo o alcance, adequação, intensidade e capacidade. A avaliação objetiva do afeto do examinador deve ser comparada ao relatório subjetivo de humor do paciente, obtido via inquérito direto.</li>
	<li>Avalie o processo de pensamento do paciente, composto por elementos, como nível de organização, presença de tangencialidade, associações soltas e "fuga de ideias".</li>
	<li>Avalie o conteúdo e as percepções do paciente, embora estes normalmente não sejam concluídos apenas por observação, e podem ser necessários questionamentos específicos. O conteúdo do pensamento abrange obsessões, ansiedades, fobias, pré-ocupação somática, delírios e ideias de perseguição, influência e referência. As percepções incluem alucinações, desrealização e despersonalização.</li>
	<li>Note a função cognitiva do paciente. Pistas sobre anormalidades de atenção, orientação, memória, julgamento e discernimento podem surgir se o examinador estiver sintonizado para procurá-las, embora o uso de perguntas específicas e instrumentos validados possa ser necessário para quantificar déficits.</li>
	<li>Use perguntas específicas para avaliar o suicídio e a homicida.</li>
</ol>5. Observações acessórias

As observações gerais começam no primeiro ponto de contato com qualquer paciente e continuam durante todo o encontro clínico, mesmo durante a conversa com o paciente. A inspeção é mais direcionada a objetivos e é limitada ao que se pode observar visualmente ao examinar partes específicas do corpo, como pele, olhos ou orelhas, às vezes com a ajuda de um equipamento especializado. Aqui, ilustramos as observações gerais que um clínico deve considerar realizar durante cada encontro clínico, seguidas de algumas considerações relacionadas às etapas de inspeção visual. Clinical observation is the starting point of a physical examination and begins at the first contact with a patient. Observation is a broad term for using the senses carefully to gather information. Inspection is a narrower term. It refers to what can be seen with the eyes, especially on the surface of the body, rather than to behavior.

Skilled clinicians use more than sight when they examine a patient. They also rely on touch, through palpation and percussion, and on hearing, through auscultation and percussion. Smell can add useful clues as well. It may point to poor personal hygiene, substance use, or metabolic diseases.

Taste is mostly a historical part of medicine. For many centuries, diabetes mellitus was even diagnosed by the sweet taste of urine. Over time, clinicians also develop a sixth sense, often called gut instinct. This skill comes from deliberate practice on thousands of patients over many years. It is based largely on bedside observations and can help predict serious illness.
