1. Inspeção
<ol><li>Peça ao paciente para andar, e observe mancar e dor.</li>

<li>Inchaço ou derrame: Sugerido quando o joelho lesionado é visivelmente maior do que o joelho não envolvido. </ol>2. Palpação
<ol><li>Palpação para inchaço e derrame Cerque os dedos ao redor da rótula para avaliar o inchaço. Inchaço na frente da rótula sugere bursite pré-patelar ou infecção, enquanto inchaço atrás da rótula sugere uma derrame articular e provavelmente um desordem interna significativa. A quantidade de efusão tem algum significado diagnóstico e pode ser aproximadamente classificada de 0 a 3+ onde 0 significa sem derrame, 1+ significa a derrame de traços como visto na osteoartrite ou lágrima meniscal antiga, 2+ significa a efusão moderada como visto no ligamento cruzado posterior (LPC) ou ruptura meniscal, e 3+ significa efusão grande que pode indicar ruptura do ligamento cruzado anterior (LCA) ruptura ou luxação patelar.</li>
	<li>Palpação para ternura A localização da dor pode sugerir quais estruturas podem ser feridas. Coloque o paciente na posição supina e palpa as seguintes áreas com as pontas dos dedos:<ol>
<li>Tubérculo tibial: Sinta pela saliência áspera na superfície anterior da tíbia proximal (abaixo da frente do joelho na linha média). É o local de inserção do tendão patelar. A ternura aqui sugere a doença de Osgood Schlatter em uma criança ou adolescente.</li>
		<li>Patela: Palpa com as pontas dos dedos sobre e ao redor da patela (o osso grande na frente da articulação do joelho). Ternura ou dor ao redor ou sob a patela sugere dor patelal; sobre o topo da patela sugere bursite pré-patela; sob a patela sugere dor patelal ou lesão chondral. </ol>3. Faixa de movimento (ROM)
A ROM da articulação do joelho é geralmente avaliada passivamente, com o paciente deitado supino. O joelho deve ser verificado para as seguintes moções:
<ol><li>Extensão (0°)<ol>
<li>Peça ao paciente para endireitar totalmente a perna. A falta de extensão do joelho sugere bloqueio mecânico (muitas vezes de um menisco rasgado, corpo solto ou efusão grande).</li>
		<li>Para detectar uma falta mais sutil de extensão, tenha o paciente deitado propenso a ter coxas apoiadas pelo final de uma mesa de exame, e observe a diferença na altura do calcanhar.</li>
	</ol>
</li>
	<li>Flexão (130°): Peça ao paciente para deitar supino e dobrar o joelho o máximo possível, trazendo o calcanhar o mais próximo possível das nádegas. A flexão pode ser limitada por derrame articular ou espasmo quadríceps.</li>
	<li>Mecanismo extensor: Peça ao paciente para endireitar ativamente o joelho na posição sentada. É essencial verificar se há extensão ativa do joelho para avaliar a integridade do mecanismo extensor (músculo quadríceps e tubérculo, patela e tubérculo tibial).</li>
	<li>Crepitus: Sinta por crepitus (moagem palpável produzida pelo movimento) colocando a palma da mão sobre o joelho, pois o joelho é erificado ativamente ou passivamente e depois estendido. Este sinal raramente é clinicamente significativo, a menos que esteja associado à dor, derrame ou limitação de movimento.</li>
</ol>4. Teste ligamentar
<ol><li>Teste o ligamento colateral medial (LMC) e o ligamento colateral lateral (LCL) para dor e frouxidão.<ol>
<li>Coloque o paciente na posição supina.</li>
		<li>Estender passivamente (endireitar) o joelho do paciente e, em seguida, flexioná-lo ligeiramente (a 20°).</li>
		<li>Teste o MCL aplicando o estresse de valgo segurando o tornozelo do paciente com uma mão, enquanto empurra contra o lado lateral do joelho com a outra mão<ol>
<li>Marque a lesão do MCL por dor e frouxidão. A frouxidão ao estresse valgus aplicado com o joelho em plena extensão sugere lesão à LCA e/ou PCL, bem como ao MCL. A LCA e o PCL ficam tensos quando o joelho está em plena extensão e, portanto, devem agir para evitar a frouxidão valgus nesta posição. Dobrar o joelho a 20° afrouxa esses ligamentos e permite testes isolados do MCL.</li>
		</ol>
</li>
		<li>Teste a LCL exercendo um estresse de varo segurando o tornozelo com uma mão, enquanto empurra contra o lado medial do joelho com a outra mão, também com o joelho estendido e depois flexionado para 20°.<ol>
<li>Use a mesma classificação para uma lesão mcl para descrever uma lesão à LCL. A LCL é muito menos comumente ferida do que o MCL. Se for notada uma frouxidão significativa para o estresse do varo, suspeite de lesão mais grave no canto extrateral do joelho. Tal lesão pode acompanhar uma luxação do joelho e garante encaminhamento ortopédico urgente.</li>
		</ol>
</li>
	</ol>
</li>
	<li>Avalie a ACL realizando os seguintes testes:<ol>
<li>Teste de Lachman: Com o supino do paciente e o joelho em 20° de flexão (como descrito acima), segure a tíbia proximal com uma mão e aplique uma força anteriormente direcionada na tíbia enquanto estabiliza a coxa com a outra mão colocada no lado lateral do fêmur. Este é o exame definitivo para avaliar para uma lágrima de LCA.</li>
		<li>Teste anterior da gaveta: Com o paciente deitado supino e o joelho em 90° de flexão, coloque as mãos ao redor do aspecto superior da tíbia, com os polegares na frente do joelho, e puxe a tíbia para a frente. A sensibilidade deste exame é limitada porque com o joelho flexionado a 90°, os ligamentos colaterais são tensos e restringem o movimento anterior.</li>
		<li>Mudança de pivô: Com o supino do paciente, segure a perna pelo pé e gire a perna internamente enquanto flexiona o joelho após 20° e aplique um leve estresse valgo (empurrando para fora do joelho medialmente). Procure a tíbia para avançar. Este teste é útil na avaliação das restrições secundárias da articulação do joelho, e geralmente será positivo apenas quando o joelho estiver muito frouxo. Este teste é muitas vezes doloroso, o que limita sua utilidade.</li>
	</ol>
</li>
	<li>Teste de PCL<ol>
<li>Teste posterior da gaveta: posicione o paciente e coloque as mãos quanto ao teste anterior da gaveta e empurre a tíbia posterior. Deslocamento posterior significativo da tíbia sugere uma lesão de LPC.</li>
		<li>Sinal de sag: posicione o supino do paciente com o joelho flexionado a 90° e o pé plano sobre a mesa do exame. Observe se a tíbia cair para trás em relação ao fêmur. Isso sugere uma ruptura pcl com uma frouxidão posterior significativa.</li>
	</ol>
</li>
</ol>5. Testes meniscos
Os testes feitos para avaliar lesões meniscal são muitas vezes não específicos com uma alta taxa de falsos positivos. Os testes meniscos mais comuns descritos incluem:
<ol><li>Teste de salto: Com o paciente deitado supino, force o joelho a uma extensão completa empurrando para baixo na rótula. Um menisco ferido causará dor significativa à medida que é beliscado com extensão do joelho.</li>
	<li>Ternura de linha conjunta: Palpate ao longo das linhas articulares medial e lateral, onde está localizado o menisco. A ternura aqui sugere lesão ao menisco ou osteoartrite.</li>
	<li>Extensão do joelho propensa: Peça ao paciente para ficar propenso com os dois joelhos pendurados na extremidade da mesa de exame e, em seguida, procure uma diferença na altura do calcanhar. Isso pode indicar um bloqueio mecânico à extensão do joelho causado por um menisco rasgado e deslocado.</li>
	<li>Passeio de pato: Peça ao paciente para entrar em um agachamento completo e, em seguida, caminhar na posição agachada. Se for capaz de fazer isso, é improvável que o paciente tenha uma lesão significativa na cartilagem ou ligamento.</li>
	<li>Teste de McMurray<ol>
<li>Com o paciente deitado supino, coloque uma mão no joelho (proporcionando estresse varo) e a outra mão no calcanhar.</li>
		<li>Realize o teste flexionando e estendendo o joelho do paciente enquanto gira interna e externamente a tíbia. Um clunk significativo com esta manobra pode indicar uma ruptura meniscal deslocada. Este teste deve ser feito com cautela, pois pode causar um menisco rasgado para deslocar e travar a articulação do joelho. A utilidade deste teste é limitada por sua alta taxa de falsos positivos.</li>
	</ol>
</li>
	<li>Teste de compressão de Apley<ol>
<li>Que o paciente fique propenso na mesa de exame com o joelho dobrado a 90°.</li>
		<li>Flexione passivamente e estenda o joelho do paciente enquanto gira a tíbia interna e externamente com uma mão, e aplique simultaneamente uma carga axial na articulação do joelho empurrando para baixo no calcanhar com a outra mão.</li>
		<li>Observe por um clunk significativo durante esta manobra, o que pode indicar um menisco rasgado. A utilidade deste teste também é severamente limitada por uma alta taxa de falso positivo.</li>
	</ol>
</li>
</ol>6. Testes de patela
Vários testes podem ser feitos para avaliar a disfunção patelar, dor ou luxação. Os testes de patela são realizados com o paciente deitado na posição supina.
<ol><li>Teste de apreensão: Empurre a rótula em direção lateral. O teste é positivo quando o paciente fica apreensivo que a patela pode deslocar-se recorrentemente. Isso sugere uma luxação prévia da patela.</li>
	<li>Teste de moagem de patela: Empurre para baixo na rótula e moer para frente e para trás. Este teste pode agravar a dor da disfunção patelar e tem uma taxa de falso positivo muito alta.</li>
	<li>Ângulo Q (ângulo do quadríceps): Com o paciente deitado supino, estime o ângulo formado por uma linha que conecta a coluna ilílica superior anterior (ASIS) do quadril e o centro da patela, com uma linha abaixo da tíbia anterior (do centro da patela ao tubérculo tibial). Ângulos superiores a 10° no masculino e 15° no feminino predispõem a problemas patelares. A pelve mais ampla observada nas mulheres aumenta o ângulo Q e predispõe o paciente à dor patelar.</li>
</ol>

O exame do joelho é melhor feito após uma abordagem stepwise com o paciente em posições sentadas e supinas. O paciente deve ser observado para mancar ou dor ao entrar e sair da sala de exame. É importante que o paciente remova roupas suficientes para que a anatomia superficial do joelho possa ser vista e comparada com o lado não envolvido. Lembre-se de lavar bem as mãos antes de tocar no paciente. Peça ao paciente para se sentar na mesa de exame e, em seguida, deite-se em decúbito dorsal. Comece avaliando o inchaço ao redor da rótula. Ordenhe por cima e por baixo do joelho e, em seguida, empurre a patela para baixo com o dedo para apreciar qualquer fluido acima e abaixo dela. Repita o mesmo do outro lado. Por fim, palpe o aspecto medial e lateral do joelho. Comece no côndilo femoral medial, o local de fixação do ligamento colateral medial, que se estende do côndilo até a tíbia medial proximal. Em seguida, verifique se há sensibilidade na bursa da pata anserina localizada logo abaixo da linha articular medial anterior. Por fim, palpe a banda iliotibial que vai do quadril, ao longo do lado lateral, e se prende à tíbia no tubérculo de Gerdy. Após a conclusão da parte de inspeção e palpação do exame, passe a testar a amplitude de movimento do joelho. Enquanto estiver em decúbito dorsal, faça com que o paciente endireite totalmente a perna e levante-a levemente para testar a extensão. Normalmente, isso deve revelar um 0? ângulo. Em seguida, verifique a flexão do joelho. Passivamente, dobre o joelho do paciente, trazendo o calcanhar o mais próximo possível das nádegas. O alcance para este movimento deve ser de cerca de 130?. Se limitado, pode indicar derrame articular ou espasmo do quadríceps. Em seguida, peça ao paciente que faça a transição para a posição sentada e peça que endireite o joelho. Isso é para testar a integridade do mecanismo extensor. Por fim, sinta a crepitação - o ranger palpável produzido pelo movimento - colocando a palma da mão sobre o joelho, pois ela é passiva ou ativamente, flexionada e estendida. Esse sinal raramente é clinicamente significativo, a menos que esteja associado a dor, derrame ou limitação de movimento. Depois de realizar todas as manobras mencionadas acima, avalie a estrutura do joelho usando uma variedade de testes especiais. O primeiro deles é o Teste de Ligamento. Essas manobras avaliam a dor e a frouxidão nos ligamentos colaterais medial e lateral e nos ligamentos cruzados anterior e posterior, abreviados como MCL, LCL, LCA e LCP, respectivamente. Para os testes MCL e LCL, primeiro endireite o joelho do paciente levantando-o no tornozelo com uma mão e, em seguida, use a outra mão para flexioná-lo a cerca de 20?. Para avaliar o ligamento colateral medial, mantenha o joelho na posição flexionada e aplique estresse em valgo empurrando o tornozelo para fora. Em seguida, faça o mesmo com o joelho estendido. Para verificar o LCL, troque as posições das mãos e aplique estresse em varo empurrando o tornozelo medialmente com o joelho estendido e depois flexionado. Em ambos os casos, pontue a lesão ligamentar por dor e frouxidão, conforme mostrado nesta tabela. Os próximos dois testes, ou seja, o teste de Lachman e o teste de Gaveta Anterior, examinam o LCA. Para o teste de Lachman, peça ao paciente que relaxe as pernas e, em seguida, flexione o joelho para cerca de 20? ângulo. E enquanto puxa para frente na tíbia proximal com uma mão, empurre o fêmur para baixo com a outra. Com esta manobra, você está sentindo um ponto final à medida que o LCA fica estressado. Em seguida, realize o Teste de Gaveta Anterior. Peça ao paciente para dobrar o joelho e colocar o pé apoiado na mesa. Em seguida, sente-se no pé para que ele não se mova. Coloque as mãos logo abaixo do joelho com os dois dedos indicadores na linha articular posterior e puxe para verificar se a tíbia se move para frente, o que não deveria.Após duas manobras - o teste da gaveta posterior e a busca do sinal de afundamento - avalie o LCP. Para o teste da gaveta posterior, faça com que o paciente dobre o joelho como no teste da gaveta anterior, depois coloque os dois polegares em ambos os lados do tendão da patela, puxe para trás e empurre posteriormente a parte superior da tíbia. Deslocamento significativo pode sugerir uma lesão do LCP. Por fim, execute o teste do sinal de Sagitação. Com o joelho do paciente flexionado para 90? e pé apoiado na mesa de exame; Observe se a tíbia cede ou cai para trás em relação ao fêmur. É uma boa abordagem para comparar entre os lados. A presença desse sinal pode sugerir uma ruptura do LCP com frouxidão posterior significativa. Após o teste do ligamento, execute as seguintes manobras que verificam se há lesão meniscal. No entanto, deve-se ter em mente que esses testes geralmente são inespecíficos e requerem confirmação adicional. O primeiro desses testes é o teste Bounce. Com o paciente em decúbito dorsal, levante a perna na altura do tornozelo e "balance" suavemente o joelho, forçando-o a se estender totalmente. Esta manobra causará dor se o menisco for rompido. Para avaliar a sensibilidade da linha articular, palpe ao longo das linhas articulares medial e lateral, onde o menisco está localizado. Qualquer sensibilidade aqui pode sugerir uma lesão no menisco ou osteoartrite. Em seguida, peça ao paciente para fazer a transição para a posição prona com os dois joelhos pendurados na extremidade da mesa de exame. Este é o teste de extensão do joelho em decúbito ventral. A diferença na altura do calcanhar pode indicar um bloqueio mecânico para a extensão do joelho causado por um menisco rompido e deslocado. Posteriormente, realize o teste de McMurray. Primeiro, dobre o joelho do paciente e flexione-o totalmente, depois gire e estenda-o internamente. Repita o mesmo girando-o externamente. Um som de estalo significativo durante este teste pode indicar uma ruptura do menisco deslocado. Esta manobra deve ser feita com cautela, pois pode-se fazer com que um menisco rompido se desloque e trave a articulação do joelho. Em seguida, execute o teste de compressão de Apley. Com o paciente deitado, flexione passivamente o joelho a 90?, gire a tíbia internamente, aplique carga axial e flexione e estenda o joelho continuamente para verificar se há algum som de batida. Repita a manobra enquanto gira a tíbia externamente. Claudicação significativa pode indicar um menisco rompido. Para o último teste meniscal, chamado de Teste de Caminhada do Pato. Solicite que o paciente fique em uma posição de agachamento completo e depois caminhe. É improvável que um paciente tenha uma lesão significativa na cartilagem ou nos ligamentos se for capaz de fazer isso. Finalmente, alguns testes chamados testes de patela podem ser realizados para avaliar a disfunção, dor ou luxação femoropatelar. A primeira manobra neste grupo é chamada de teste de apreensão. Empurre a rótula do paciente na direção lateral e veja se o paciente fica apreensivo com a possibilidade de a patela se deslocar, o que sugere uma luxação prévia da patela. Em seguida, execute o teste de moagem da patela empurrando para baixo a rótula do paciente e triture-a para frente e para trás. A dor sentida pelo paciente durante essa manobra pode sugerir disfunção patelofemoral. Finalmente, verifique o quadríceps ou o ângulo Q. Com o paciente em decúbito dorsal, estimar o ângulo formado por uma linha que liga a espinha ilíaca ântero-superior e o centro da patela, com uma linha do centro da patela descendo pela tíbia anterior até o tubérculo tibial. Knee injury assessment focuses on the largest joint in the body, where the femur meets the tibia. Because the knee sits in the middle of the lower leg, it faces traumatic and degenerative forces. Its structure also makes the exam more detailed. The joint is inherently unstable and relies on ligaments and menisci for support.

The menisci act as shock absorbers and help increase the contact area of the joint. The patella sits in front of the knee and acts as a fulcrum for forceful extension. That movement is important for actions like running and kicking. The patella is also the largest sesamoid bone in the body, so the knee is often a source of pain after trauma or overuse.

A proper knee exam starts with enough clothing removed to expose the entire thigh, knee, and lower leg. The clinician first inspects and palpates, which means looking at and feeling key anatomic landmarks. Next, the examiner checks range of motion, or ROM. The exam then moves to tests for ligament or meniscus injury and special tests for patellofemoral dysfunction and dislocation of the patella.

The opposite knee is often used as the standard for comparison. This works best when the other knee has not been injured before. Using the healthy side helps the examiner judge changes in the injured knee more accurately.
