With each advance in genomic technology, new statistical methods have regularly emerged to test genetic hypotheses in complex inheritance, as evidenced throughout this book. Notwithstanding the approach used, the greatest challenge in the genetics of complex traits remains the identification of the gene(s) and the molecular variant(s) accounting for a genetic inference based on statistical testing. We take the example of quantitative trait locus (QTL) mapping for blood pressure (BP) and related phenotypes in rodents to review the current landscape. Traditional approaches to refined mapping are typically hampered by the small effect and the small proportion of the variance attached to individual QTLs. The alternative of functional screens in intact animals, whether by chemical mutagenesis or gene targeting, remains a daunting undertaking. Such limitations account for the slow progress to date of inferences from QTL to gene(s). We select a QTL for differential sodium sensitivity between two mouse inbred lines to propose an approach that can be used in relatively large genomic regions (1) by optimizing the selection of candidate genes and (2) by subjecting such genes to high-throughput functional screens. While this is still work in progress, we think it abundantly illustrates what is ahead of us in delineating genetic variation that underlie complex disease.

From genetics to mechanism of disease liability.

Avec chaque avanc&eacute;e dans la technologie g&eacute;nomique, nouvelles m&eacute;thodes statistiques sont apparus r&eacute;guli&egrave;rement pour v&eacute;rifier les hypoth&egrave;ses g&eacute;n&eacute;tiques dans l&#39;h&eacute;r&eacute;dit&eacute; complexe, comme en t&eacute;moigne tout au long de ce livre. Malgr&eacute; l&#39;approche utilis&eacute;e, le plus grand d&eacute;fi dans la g&eacute;n&eacute;tique des traits complexes reste l&#39;identification des ou des g&egrave;nes et la variante mol&eacute;culaire repr&eacute;sente une inf&eacute;rence g&eacute;n&eacute;tique bas&eacute;es sur des tests statistiques. Nous prenons l&#39;exemple de la cartographie quantitative trait locus (QTL) pour la pression art&eacute;rielle (PA) et les ph&eacute;notypes associ&eacute;s chez les rongeurs de revoir le paysage actuel. Les approches traditionnelles de cartographie raffin&eacute;e sont heurtent g&eacute;n&eacute;ralement l&#39;effet petit et la faible proportion de la variance attach&eacute;e &agrave; QTL individuels. L&#39;alternative d&#39;&eacute;crans fonctionnels chez les animaux intacts, que ce soit par mutagen&egrave;se chimique ou ciblage g&eacute;n&eacute;tique, reste une entreprise ardue. Ces limitations repr&eacute;sentent la lenteur des progr&egrave;s r&eacute;alis&eacute;s &agrave; ce jour des inf&eacute;rences de QTL de g&egrave;ne (s). Nous s&eacute;lectionnons un QTL pour la sensibilit&eacute; diff&eacute;rentielle de sodium entre deux lign&eacute;es de souris consanguine de proposer une approche qui peut &ecirc;tre utilis&eacute;e dans des r&eacute;gions g&eacute;nomiques relativement importantes (1) en optimisant la s&eacute;lection des g&egrave;nes candidats et (2) en soumettant ces g&egrave;nes &agrave; haut d&eacute;bit &eacute;crans fonctionnels. C&#39;est encore en travaux, nous pensons qu&#39;elle illustre abondamment ce qui est devant nous dans la d&eacute;limitation des variations g&eacute;n&eacute;tiques qui sous-tendent la maladie complexe.

De la g&eacute;n&eacute;tique au m&eacute;canisme de la responsabilit&eacute; de la maladie.

Mit jedem Fortschritt in der genomischen Technologie entstanden neue statistische Methoden regelm&auml;&szlig;ig um genetische Hypothesen in komplexen Vererbung zu testen, wie dieses Buch belegt. Die Alternative der funktionalen Bildschirme bei intakten Tieren bleibt chemische Mutagenese oder Gene targeting, eine gewaltige Unterfangen. Solche Beschr&auml;nkungen entfallen nur die langsamen Fortschritte der R&uuml;ckschl&uuml;sse von QTL, geh&auml;uft. Wir w&auml;hlen eine QTL f&uuml;r differenzielle Natrium Empfindlichkeit zwischen zwei Maus durch Inzucht-Linien einen Ansatz vorzuschlagen, der in relativ gro&szlig;en genomische Regionen (1) durch die Optimierung der Auswahl von Kandidatengenen und (2) indem solcher Gene auf Hochdurchsatz-funktionale Bildschirme verwendet werden k&ouml;nnen. W&auml;hrend dies noch In Arbeit ist, denken wir, dass es reichlich illustriert, was liegt vor uns in Abgrenzung genetischen Variation, die komplexe Krankheit zugrunde liegen.

Von Genetik Mechanismus der Krankheit Haftung.

Con cada avance en tecnolog&iacute;a gen&oacute;mica, nuevos m&eacute;todos estad&iacute;sticos regularmente han surgido para probar hip&oacute;tesis gen&eacute;ticas en la herencia compleja, como lo demuestran a lo largo de este libro. No obstante el enfoque utilizado, el mayor desaf&iacute;o en la gen&eacute;tica de rasgos complejos sigue siendo la identificaci&oacute;n de los genes y las variantes moleculares representan una inferencia gen&eacute;tica basan en pruebas estad&iacute;sticas. Tomamos el ejemplo de asignaci&oacute;n de lugar geom&eacute;trico (QTL) de car&aacute;cter cuantitativo para la presi&oacute;n arterial (PA) y fenotipos relacionados en roedores para revisar el panorama actual. Los enfoques tradicionales para asignaci&oacute;n de refinado t&iacute;picamente se ven obstaculizados por el efecto peque&ntilde;o y la peque&ntilde;a proporci&oacute;n de la varianza atada QTLs individuales. La alternativa de pantallas funcionales en animales intactos, ya sea mediante mutag&eacute;nesis qu&iacute;mica o gene targeting, sigue siendo una tarea desalentadora. Tales limitaciones representan el lento progreso hasta la fecha de inferencias de QTL para genes. Seleccionamos un QTL para sensibilidad de sodio diferencial entre dos l&iacute;neas endog&aacute;mica de rat&oacute;n para proponer un enfoque que puede ser utilizado en regiones gen&oacute;micas relativamente grandes (1) mediante la optimizaci&oacute;n de la selecci&oacute;n de genes candidatos y (2) sometiendo dichos genes a pantallas funcionales de alto rendimiento. Mientras que esto todav&iacute;a est&aacute; en proceso, pensamos abundantemente ilustra lo que est&aacute; delante de nosotros en delinear una variaci&oacute;n gen&eacute;tica que subyacen la enfermedad compleja.

People who begin daily smoking at an early age are at greater risk of long-term nicotine addiction. We tested the hypothesis that associations between nicotinic acetylcholine receptor (nAChR) genetic variants and nicotine dependence assessed in adulthood will be stronger among smokers who began daily nicotine exposure during adolescence. We compared nicotine addiction-measured by the Fagerstrom Test of Nicotine Dependence-in three cohorts of long-term smokers recruited in Utah, Wisconsin, and by the NHLBI Lung Health Study, using a candidate-gene approach with the neuronal nAChR subunit genes. This SNP panel included common coding variants and haplotypes detected in eight alpha and three beta nAChR subunit genes found in European American populations. In the 2,827 long-term smokers examined, common susceptibility and protective haplotypes at the CHRNA5-A3-B4 locus were associated with nicotine dependence severity (p = 2.0x10(-5); odds ratio = 1.82; 95% confidence interval 1.39-2.39) in subjects who began daily smoking at or before the age of 16, an exposure period that results in a more severe form of adult nicotine dependence. A substantial shift in susceptibility versus protective diplotype frequency (AA versus BC = 17%, AA versus CC = 27%) was observed in the group that began smoking by age 16. This genetic effect was not observed in subjects who began daily nicotine use after the age of 16. These results establish a strong mechanistic link among early nicotine exposure, common CHRNA5-A3-B4 haplotypes, and adult nicotine addiction in three independent populations of European origins. The identification of an age-dependent susceptibility haplotype reinforces the importance of preventing early exposure to tobacco through public health policies.

Duchenne muscular dystrophy (DMD) displays a clinical range that is not fully explained by the primary DMD mutations. Ltbp4, encoding latent transforming growth factor-β binding protein 4, was previously discovered in a genome-wide scan as a modifier of murine muscular dystrophy. We sought to determine whether LTBP4 genotype influenced DMD severity in a large patient cohort.

LTBP4 genotype predicts age of ambulatory loss in Duchenne muscular dystrophy.

Most mutations that truncate the reading frame of the DMD gene cause loss of dystrophin expression and lead to Duchenne muscular dystrophy. However, amelioration of disease severity has been shown to result from alternative translation initiation beginning in DMD exon 6 that leads to expression of a highly functional N-truncated dystrophin. Here we demonstrate that this isoform results from usage of an internal ribosome entry site (IRES) within exon 5 that is glucocorticoid inducible. We confirmed IRES activity by both peptide sequencing and ribosome profiling in muscle from individuals with minimal symptoms despite the presence of truncating mutations. We generated a truncated reading frame upstream of the IRES by exon skipping, which led to synthesis of a functional N-truncated isoform in both human subject-derived cell lines and in a new DMD mouse model, where expression of the truncated isoform protected muscle from contraction-induced injury and corrected muscle force to the same level as that observed in control mice. These results support a potential therapeutic approach for patients with mutations within the 5' exons of DMD.

Exon-skipping therapies aim to convert Duchenne muscular dystrophy (DMD) into less severe Becker muscular dystrophy (BMD) by altering pre-mRNA splicing to restore an open reading frame, allowing translation of an internally deleted and partially functional dystrophin protein. The most common single exon deletion-exon 45 (Δ45)-may theoretically be treated by skipping of either flanking exon (44 or 46). We sought to predict the impact of these by assessing the clinical severity in dystrophinopathy patients.

Clinical phenotypes as predictors of the outcome of skipping around DMD exon 45.

The expressivity of Mendelian diseases can be influenced by factors independent from the pathogenic mutation: in Duchenne muscular dystrophy (DMD), for instance, age at loss of ambulation (LoA) varies between individuals whose DMD mutations all abolish dystrophin expression. This suggests the existence of trans-acting variants in modifier genes. Common single nucleotide polymorphisms (SNPs) in candidate genes (SPP1, encoding osteopontin, and LTBP4, encoding latent transforming growth factor β [TGFβ]-binding protein 4) have been established as DMD modifiers. We performed a genome-wide association study of age at LoA in a sub-cohort of European or European American ancestry (n = 109) from the Cooperative International Research Group Duchenne Natural History Study (CINRG-DNHS). We focused on protein-altering variants (Exome Chip) and included glucocorticoid treatment as a covariate. As expected, due to the small population size, no SNPs displayed an exome-wide significant p value (< 1.8 × 10). Subsequently, we prioritized 438 SNPs in the vicinities of 384 genes implicated in DMD-related pathways, i.e., the nuclear-factor-κB and TGFβ pathways. The minor allele at rs1883832, in the 5'-untranslated region of CD40, was associated with earlier LoA (p = 3.5 × 10). This allele diminishes the expression of CD40, a co-stimulatory molecule for T cell polarization. We validated this association in multiple independent DMD cohorts (United Dystrophinopathy Project, Bio-NMD, and Padova, total n = 660), establishing this locus as a DMD modifier. This finding points to cell-mediated immunity as a relevant pathogenetic mechanism and potential therapeutic target in DMD.

Association Study of Exon Variants in the NF-κB and TGFβ Pathways Identifies CD40 as a Modifier of Duchenne Muscular Dystrophy.

The reading frame rule suggests that Duchenne muscular dystrophy (DMD) results from DMD mutations causing an out-of-frame transcript, whereas the milder Becker muscular dystrophy results from mutations causing an in-frame transcript. However, predicted nonsense mutations may instead result in altered splicing and an in-frame transcript. Here we report a 10-year-old boy with a predicted nonsense mutation in exon 42 who had a 6-minute walk time of 157% of that of age matched DMD controls, characterized as intermediate muscular dystrophy. RNA sequencing analysis from a muscle biopsy revealed only 6.0-9.8% of DMD transcripts were in-frame, excluding exon 42, and immunoblot demonstrated only 3.2% dystrophin protein expression. Another potential genetic modifier noted was homozygosity for the protective IAAM LTBP4 haplotype. This case suggests that very low levels of DMD exon skipping and dystrophin protein expression may result in amelioration of skeletal muscle weakness, a finding relevant to current dystrophin-restoring therapies.

Vici syndrome is a multisystem disorder characterized by agenesis of the corpus callosum, oculocutaneous hypopigmentation, cataracts, cardiomyopathy, combined immunodeficiency, failure to thrive, profound developmental delay, and acquired microcephaly. Most individuals are severely affected and have a markedly reduced life span. Here we describe an 8-year-old boy with a history of developmental delay, agenesis of the corpus callosum, failure to thrive, myopathy, and well-controlled epilepsy. He was initially diagnosed with a mitochondrial disorder, based in part upon nonspecific muscle biopsy findings, but mitochondrial DNA mutation analysis revealed no mutations. Whole exome sequencing revealed compound heterozygosity for two EPG5 variants, inherited in trans. One was a known pathogenic mutation in exon 13 (c.2461C > T, p.Arg821X). The second was reported as a variant of unknown significance found within intron 16, six nucleotides before the exon 17 splice acceptor site (c.3099-6C > G). Reverse transcription-polymerase chain reaction of the EPG5 mRNA showed skipping of exon 17-which maintains an open reading frame-in 77% of the transcript, along with 23% expression of wild-type mRNA suggesting that intronic mutations may affect splicing of the EPG5 gene and result in symptoms. However, the expression of 23% wild-type mRNA may result in a significantly attenuated Vici syndrome phenotype.

Canine models of Duchenne muscular dystrophy (DMD) are a valuable tool to evaluate potential therapies because they faithfully reproduce the human disease. Several cases of dystrophinopathies have been described in canines, but the Golden Retriever muscular dystrophy (GRMD) model remains the most used in preclinical studies. Here, we report a new spontaneous dystrophinopathy in a Labrador Retriever strain, named Labrador Retriever muscular dystrophy (LRMD).
