Steps for measuring blood pressure in the leg include the following.
Start by performing hand hygiene.
To measure BP in the thigh, assist the patient to a prone or supine position with a slightly flexed knee to ensure easy access to the artery.
Gently feel the pulse of the popliteal artery, position the cuff above the popliteal fossa, and place the stethoscope over the popliteal fossa.
To measure BP in the calf, help the patient to a prone or supine position with a slightly flexed knee and palpate the dorsalis pedis or posterior tibial artery pulse.
Apply the cuff over the lower half of the calf one inch above the malleoli and place the stethoscope at the malleoli.
Close and tighten the valve to avoid air leaks.
Slowly inflate the cuff until the first korotkoff sound indicates systolic blood pressure. Gradually deflate and observe where the sound disappears, indicating diastolic blood pressure.
Deflate and remove the cuff. Assist the patient to a comfortable position.
Replace equipment after cleaning. Wash hands and record the findings.

Measuring leg blood pressure takes careful setup, correct cuff placement, and close listening for clear readings. This procedure helps healthcare providers collect reliable vital signs and support patient care.

The first step is hand hygiene. Use bactericidal soap or an alcohol-based rub to help reduce the spread of infectious agents. Then position the patient so the popliteal artery, the artery behind the knee, can be reached without obstruction. A prone position works best. If that is not possible, a supine position with the knee slightly flexed is acceptable. The legs should not be crossed, and any tight clothing around the thigh should be removed.

For a thigh reading, first palpate the popliteal pulse. Then place the blood pressure cuff on the lower third of the thigh. The cuff markers should line up over the artery, and the cuff should sit one inch above the knee crease. For a calf reading, palpate the dorsalis pedis or posterior tibial pulse. Place the cuff over the lower calf, one inch above the ankle bones, also called the malleoli.

Place the stethoscope diaphragm just below the lower edge of the cuff. This helps avoid muffling the Korotkoff sounds, which are the sounds used to measure blood pressure. Close the sphygmomanometer valve, inflate the cuff quickly to a pressure above the expected systolic reading, and then deflate it slowly. The first Korotkoff sound marks systolic pressure, and the point when the sound disappears marks diastolic pressure.

After the measurement, fully deflate the cuff and help the patient into a comfortable position. Clean and store the equipment, then perform hand hygiene again. Record the blood pressure values in the patient record or the vital signs chart so the reading is documented clearly.
