Question 1

What are the main types of direct-acting cholinergic agonists?

Accepted Answer

Direct-acting cholinergic agonists include two main categories: choline esters such as acetylcholine, methacholine, and bethanechol, and alkaloids such as pilocarpine and cevimeline. Choline esters are synthetic compounds that directly stimulate cholinergic receptors, while alkaloids are naturally derived substances with similar receptor-binding properties. Both types activate muscarinic and nicotinic receptors to produce therapeutic effects.

Question 2

How is acetylcholine used in ophthalmologic surgery?

Accepted Answer

Acetylcholine is applied topically during eye surgery to constrict the pupils through a process called miosis. However, its clinical utility is limited by its non-selectivity and temporary effects, as it activates multiple receptor types and is rapidly metabolized. Despite these limitations, it remains useful for specific surgical procedures requiring pupil constriction.

Question 3

What is the diagnostic purpose of methacholine inhalation?

Accepted Answer

Methacholine is administered via inhalation to diagnose bronchial airway hyperreactivity in patients suspected of having asthma. The degree of airway narrowing in response to methacholine stimulation helps clinicians identify asthma and assess its severity. This test is particularly useful in nonasthmatic patients to rule out airway hyperresponsiveness.

Question 4

How does pilocarpine treat glaucoma and dry mouth?

Accepted Answer

Pilocarpine, a muscarinic and ganglionic stimulator, lowers intraocular pressure by increasing aqueous humor outflow, effectively treating open-angle glaucoma. As a sialogogue, it increases salivation and lacrimal secretion, alleviating xerostomia and dry eyes caused by radiotherapy or Sjögren syndrome. Its dual therapeutic actions make it valuable for both ophthalmologic and systemic conditions.

Question 5

Why is cevimeline preferred over pilocarpine for treating dry mouth?

Accepted Answer

Cevimeline is a selective M3 agonist that stimulates salivary and lacrimal glands to increase salivation and tear production. It is preferred over pilocarpine as a sialogogue because it produces fewer side effects due to its receptor selectivity. This targeted action reduces unwanted muscarinic effects while maintaining therapeutic benefits for xerostomia.

Question 6

What gastrointestinal and urinary conditions does bethanechol treat?

Accepted Answer

Bethanechol improves gastrointestinal and urinary tract contractions by stimulating muscarinic receptors. It is used to treat postoperative or postpartum urinary retention, neurogenic bladder issues, and gastroesophageal reflux symptoms. Bethanechol also relieves constipation by enhancing intestinal motility and promoting normal bowel function.

Question 7

What factors determine the clinical use of direct-acting cholinergic agonists?

Accepted Answer

The clinical use of direct-acting cholinergic agonists is determined by weighing their receptor selectivity and potential side effects against therapeutic benefits. Selective agonists like cevimeline produce fewer adverse effects, while non-selective agents like acetylcholine have limited applications despite rapid onset. Clinicians choose specific agents based on the target tissue, desired duration, and acceptable risk profile for each patient.
