Question 1

What is the coronary sinus and what does it do?

Accepted Answer

The coronary sinus is the heart's principal vein, located in the coronary sulcus on the posterior aspect of the heart. It receives nearly all venous blood from the myocardium through three primary veins: the great cardiac vein, middle cardiac vein, and small cardiac vein. The coronary sinus then empties this deoxygenated blood into the right atrium.

Question 2

Where does the superior vena cava originate and what does it drain?

Accepted Answer

The superior vena cava originates posterior to the right first costal cartilage, formed by the union of the right and left brachiocephalic veins. Approximately 7.5 cm long and 2 cm in diameter, it drains blood from the head, neck, upper limbs, and thorax, then empties into the superior part of the right atrium.

Question 3

How does the inferior vena cava differ from the superior vena cava?

Accepted Answer

The inferior vena cava is the largest vein in the human body, originating anterior to the fifth lumbar vertebra from the union of the common iliac veins. Unlike the superior vena cava, it drains the abdomen, pelvis, and lower limbs. It ascends behind the peritoneum and pierces the diaphragm before entering the inferior part of the right atrium.

Question 4

What are the three major tributaries that feed the coronary sinus?

Accepted Answer

The coronary sinus receives blood from three major channels: the great cardiac vein located anteriorly, and the middle and small cardiac veins located posteriorly. Additionally, some anterior cardiac veins drain directly into the right atrium. Together, these vessels ensure that nearly all venous blood from the heart muscle returns to the right atrium.

Question 5

How can pregnancy affect the inferior vena cava?

Accepted Answer

During the later stages of pregnancy, the enlarging uterus often compresses the inferior vena cava, leading to temporary varicose veins and edema of the ankles and feet. This compression can cause blood pressure imbalances and circulatory complications as venous return from the lower body becomes restricted.

Question 6

At what anatomical level does the inferior vena cava enter the right atrium?

Accepted Answer

The inferior vena cava pierces the caval opening of the diaphragm at the level of the eighth thoracic vertebra before entering the inferior part of the right atrium. This anatomical landmark is clinically important for understanding the vascular anatomy of the thorax and abdomen.

Question 7

What is the functional role of systemic veins in circulation?

Accepted Answer

Systemic veins drain deoxygenated blood from body tissues and return it to the right atrium of the heart. The three main systemic veins—coronary sinus, superior vena cava, and inferior vena cava—collectively ensure that all deoxygenated blood from the heart muscle and body tissues is efficiently returned for reoxygenation in the lungs.
