Question 1

What does uroflowmetry measure during a urodynamic study?

Accepted Answer

Uroflowmetry is a non-invasive test that measures urine volume, flow rate, and the time taken to void. It generates a uroflowgram—a flow graph showing flow rate in milliliters per second on the vertical axis and time in seconds on the horizontal axis. This test helps diagnose bladder outlet obstruction, bladder dysfunction, incomplete bladder emptying, incontinence, and urinary blockages.

Question 2

How should patients prepare for a uroflowmetry test?

Accepted Answer

Before the test, patients should drink moderate water—approximately 1 to 2 liters—and refrain from urinating until their bladder is comfortably full. A sufficiently filled bladder ensures accurate measurements of urinary flow and bladder function. However, the bladder should not be overfilled, as this can lead to inaccurate measurements or discomfort during the test.

Question 3

What is the maximum flow rate and why is it clinically significant?

Accepted Answer

Maximum flow rate (Qmax) is the highest speed of urine flow during urination, marked by the peak of the uroflowgram curve. Normal Qmax values typically range from 15 to 25 milliliters per second in healthy adults. A significantly reduced Qmax may suggest bladder outlet obstruction, as seen in conditions like benign prostatic hyperplasia or urethral strictures.

Question 4

What do different flow curve patterns indicate about urinary tract health?

Accepted Answer

A bell-shaped flow curve indicates normal urinary flow, while flattened or irregular patterns may suggest obstruction or weak bladder contractions. Women may display flatter flow patterns due to anatomical differences. The shape of the flow curve provides important diagnostic clues about underlying urinary tract conditions and bladder function.

Question 5

What is post-void residual volume and how is it assessed?

Accepted Answer

Post-void residual (PVR) volume is the amount of urine remaining in the bladder after urination. Normal PVR is typically less than 50 milliliters, though volumes up to 100 milliliters can be acceptable in older adults. The healthcare provider may assess PVR using imaging studies ii ultrasonography or catheterization if the bladder does not seem to empty completely.

Question 6

What does a low voided volume indicate in uroflowmetry results?

Accepted Answer

Voided volume (Vvoid) is the total amount of urine expelled during the test, with normal volumes ranging from 150 to 600 milliliters. A small voided volume may indicate urinary retention or bladder dysfunction. Voided volume is critical to interpreting flow rates accurately, as adequate volume ensures reliable test results.

Question 7

How does delayed time to maximum flow relate to urinary dysfunction?

Accepted Answer

Time to maximum flow (TQmax) is the duration from the start of urination to reaching peak flow. A delayed TQmax could indicate difficulty initiating urination, often associated with neurogenic bladder or significant urinary retention. Extended voiding times may also suggest weak bladder contractions or partial obstruction of the urinary tract.
