Biotinidase Deficiency

Biotinidase deficiency is an inherited metabolic disorder in which low activity of the biotinidase enzyme disrupts the body’s ability to recycle biotin, an essential cofactor for several carboxylase enzymes. Normally, biotinidase cleaves biocytin and biotinylated peptides released during protein breakdown, making biotin available again; deficiency can therefore impair fatty-acid synthesis, amino-acid metabolism, and gluconeogenesis. The resulting metabolic stress may cause seizures, developmental delay, hypotonia, hearing or vision problems, and skin abnormalities, although early detection can prevent many complications. Newborn screening, confirmatory enzyme or genetic testing, and lifelong oral biotin supplementation support diagnosis and management, making this condition an important example of how biochemical pathways inform preventive care.

Reduced/absent expression of Pms2 and/or ERCC1 in entire crypts is a frequent event within 10 cm on each side of colonic adenocarcinomas, likely the basis of a field defect with high mutability and progression to cancer. Deficiency in Ku86 or CcOI is much less frequent in these field defects.
