Microsurgical Technique

Microsurgical technique is a set of operative methods that uses optical magnification and miniature instruments to manipulate tissues, vessels, nerves, or other structures too small for unaided vision. Surgeons work through an operating microscope or high-powered loupe, stabilizing the field while performing precise dissection, tissue handling, and suturing with very fine needles and thread. In biology, these methods support vascular and nerve repair, transplantation, tissue engineering, and experimental animal models of regeneration and disease. By limiting mechanical trauma and enabling accurate reconnection of delicate structures, microsurgery improves the study and treatment of complex biological systems.

Neointimal hyperplasia is the primary cause of stenosis in arterialized veins. We propose a new protocol whereby the right external jugular vein is grafted using the cuff technique in the common carotid artery of Sprague Dawley rats. The survival rate was 100 % at the time point of sacrifice.

An optimized technique for the microsurgical creation of arterial bifurcation aneurysms mimicking bifurcation human cerebral aneurysms is described. A venous pouch is sutured into an artificially created true bifurcation of both common carotid arteries. Facilitated microsurgical techniques and aggressive postoperative anticoagulation and analgesia lead to minimized morbidity rates and high aneurysm patency rates.
