Discharge Planning

Discharge planning is a coordinated clinical process that prepares patients to leave a hospital or other care setting safely and continue appropriate care at home or in another facility. It assesses medical stability, functional needs, medications, follow-up appointments, warning signs, caregiver support, and access to community services, then communicates this information among patients, families, and healthcare professionals. Effective discharge planning can reduce medication errors, prevent avoidable readmissions, and support recovery by ensuring continuity of care. In clinical practice, individualized plans are especially important for people with chronic conditions, complex treatment regimens, limited resources, or changing care needs.
