Nursing Assessment

Nursing assessment is a systematic process for collecting, validating, and interpreting information about a patient’s health, needs, and responses to illness, forming the foundation of individualized clinical care. Nurses combine subjective data, such as symptoms and health history, with objective findings from observation, physical examination, vital signs, and diagnostic results, then organize and prioritize the findings. This ongoing assessment supports clinical judgment, identifies actual or potential health problems, and guides nursing diagnoses, care planning, interventions, and evaluation. Reassessment after changes in condition or treatment helps detect deterioration, measure outcomes, communicate essential information across the care team, and promote safe, responsive patient care.
