Patient Care Management

Patient Care Management is the organized coordination of healthcare services, information, and resources to address a patient’s needs across the continuum of care. It works by assessing health status, setting goals, developing individualized care plans, coordinating communication among clinicians, and monitoring responses so that care can be adjusted over time. In clinical practice, this approach supports continuity between providers and care settings, helps patients participate in decisions, and promotes timely follow-up. Effective patient care management can improve care coordination, reduce duplication, and guide the delivery of safer, more efficient, and patient-centered services.
