Intraoperative Video Analysis

Intraoperative video analysis is the systematic examination of video recorded during surgery to characterize procedural actions, workflow, and clinically relevant events. It combines video capture with time-based annotation, image processing, and, increasingly, computer vision or machine-learning models that identify instruments, anatomical views, surgical phases, and changes in the operative field. In engineering, these analyses support objective assessment of technique, workflow optimization, surgical education, and development of intelligent operating-room systems. By converting complex visual data into measurable features and performance indicators, the method can help evaluate safety, efficiency, human-machine interaction, and the design of future surgical technologies.

Here, we present a case where intraoperative video consultation between a rural hospital and an academic hepatobiliary surgeon facilitated immediate diagnosis of bile duct transection during laparoscopic cholecystectomy, direct to operating room transfer between centers, and injury repair with robotic-assisted Roux-en-Y hepaticojejunostomy.

Intraoperative Gastroscopy for Tumor Localization in Laparoscopic Surgery for Gastric Adenocarcinoma

In early gastric cancer, the aim of surgery is to precisely remove the distal stomach including the primary tumor. To do this, accurate localization of the tumor is crucial, especially in totally laparoscopic surgery. This protocol describes a procedure for intraoperative gastroscopy in totally laparoscopic subtotal gastrectomy.

Fluorescence imaging is a promising innovative modality for image-guided surgery in surgical oncology. In this video we describe the technical procedure for detection of the sentinel lymph node using fluorescence imaging as showcased in gynecologic oncologicy. A multispectral fluorescence camera system, together with the fluorescent agent indocyanine green, is applied.
