Hand Allotransplantation

Hand allotransplantation is a reconstructive procedure that replaces a missing hand with a donor hand, restoring the potential for movement, sensation, and appearance after severe limb loss. As a form of vascularized composite allotransplantation, it connects the donor and recipient’s bones, tendons, nerves, blood vessels, and skin, while lifelong immunosuppressive therapy helps prevent immune-mediated rejection. Careful donor matching, microsurgical transplantation, rehabilitation, and monitoring are essential to functional recovery and graft survival. The procedure offers an alternative to prosthetic reconstruction for selected patients and provides an important clinical model for studying immune tolerance, nerve regeneration, and complex tissue repair.

Currently any kind of vascularized composite allotransplantation depends on long-term-immunosuppression, difficult to support for non-life-critical indications. We present a new porcine tibial VCA model that can be used to study bone VCA and demonstrate the use of surgical angiogenesis to maintain bone viability without the need of long-term immune-modulation.

We present a reproducible mouse model of heterotopic hindlimb-to-neck vascularized composite allotransplantation. This supermicrosurgical protocol, involving 0.3-0.5 mm vessel anastomoses, integrates nylon filament guidance and venous cuffing. It provides a reliable platform for translational studies on tolerance, ischemia-reperfusion injury, and immunological mechanisms in vascularized composite allotransplantation.
