Radiotherapy Treatment Planning

Radiotherapy treatment planning is the process of designing how ionizing radiation will be delivered to a tumor while limiting exposure to nearby healthy tissues. Using computed tomography and other medical imaging, clinicians define target volumes and organs at risk, then select beam directions, energies, and intensities; computerized dose calculations and optimization shape the planned distribution to meet prescribed dose constraints. Radiation oncologists, medical physicists, and dosimetrists review the plan and perform quality assurance before treatment begins. Accurate planning supports techniques such as three-dimensional conformal radiotherapy, intensity-modulated radiotherapy, and image-guided treatment, improving tumor control while reducing treatment-related toxicity.

The current priority in breast cancer radiotherapy is to reduce cardiac doses without compromising target tissue coverage. The voluntary breath-hold technique described here is a simple, inexpensive solution to this problem and capable of being instituted widely without the need for specialized equipment.

Brain tumor patients routinely undergo cranial radiotherapy, and while beneficial, this treatment often results in debilitating cognitive dysfunction. This serious unresolved problem has at present, no clinical recourse, and has driven our efforts to devise stem cell based therapies for the recovery of radiation-induced cognitive decrements.
