There is a paucity of regional cerebral blood flow studies on Japanese encephalitis (JE) with none of these studies describing brain perfusion abnormalities in all three stages of the disease. In this communication we report the changes noted in brain perfusion as detected by single photon emission tomography (SPET), in the acute, subacute and chronic stages of JE. Between December 2000 and March 2006, 31 patients, 19 men and 12 women, mean age 49 y, range 6-64 y of various stages of JE underwent brain perfusion SPET. Tauhese patients were at the following stages of the disease: acute stage, five patients, subacute stage 17 and chronic stage nine . The acute stage was considered as up to seven days from the onset of symptoms, the subacute, from seven to 56 days and any duration beyond 56 days was considered as the chronic stage. In the acute stage all five patients demonstrated focal areas of hyperperfusion involving mainly the thalamus. Additionally, bilateral thalami involvement was noted in three, frontal lobe involvement in four and parietotemporal hyperperfusion in three of these patients. In the subacute stage group, hypoperfusion of the thalamus was noted in all patients while frontoparietal hypoperfusion in seven patients. In the chronic stage group, hypoperfusion of thalamus was noted in four, one patient demonstrated additional occipital lobe hypoperfuion whereas normal perfusion was noted in the remaining five patients. In conclusion, the brain perfusion abnormalities observed depended on the stage of the disease. In the acute stage there was focal hyperperfusion to sites of the brain where JE virus is considered to replicate. In the subacute stage focal hypoperfusion was found to be possibly due to virus induced damage of cellular protein synthesis and in the chronic stage perfusion returns to normal due to regeneration of cellular organelles. Our results also confirm the high frequency of thalamic involvement in JE.

Sequential brain perfusion abnormalities in various stages of Japanese encephalitis.

The development of a septal aneurysm in the natural history of membranous ventricular septal defects usually makes the defect hemodynamically less significant. This report describes a case of severe right ventricular outflow obstruction produced by a membranous septal aneurysm in a patient who had an anterior malaligned ventricular septal defect with aortomitral discontinuity (double-outlet right ventricle). This patient did not have pulmonary stenosis other than the dynamic obstruction produced by the septal aneurysm. In this patient, the septal aneurysm produced both favorable and unfavorable hemodynamic effects. A reduction in the size of the ventricular septal defect produced a favorable effect, whereas a right ventricular outflow obstruction led to the unfavorable situation of right ventricular hypertension and hypertrophy. The large septal aneurysm in the presence of an already compromised right ventricular outflow tract related to an anteriorly malaligned septum resulted in severe obstruction.

Acetic acid-Urea-Triton (AUT) PAGE is commonly used method to separate histone variants and their post-translationally modified forms. Coomassie staining is the preferred method for protein visualization; however, its sensitivity is less than that of silver staining. Though silver staining of histones in AUT-PAGE has been reported, the method is time-consuming, dependent on prior staining by Amido black and has not been reported suitable for mass spectrometry. Here, we propose 'SDS-Silver' method for rapid, sensitive and mass spectrometry-compatible staining of histones resolved on AUT-PAGE.

Fixation failure of glenoid components is the main cause of unsuccessful total shoulder arthroplasties. The characteristics of these failures are still not well understood, hence, attempts at improving the implant fixation are somewhat blind and the failure rate remains high. This lack of understanding is largely due to the fundamental problem that direct observations of failure are impossible as the fixation is inherently embedded within the bone. Twenty custom made implants, reflecting various common fixation designs, and a specimen set-up was prepared to enable direct observation of failure when the specimens were exposed to cyclic superior loads during laboratory experiments. Finite element analyses of the laboratory tests were also carried out to explain the observed failure scenarios. All implants, irrespective of the particular fixation design, failed at the implant-cement interface and failure initiated at the inferior part of the component fixation. Finite element analyses indicated that this failure scenario was caused by a weak and brittle implant-cement interface and tensile stresses in the inferior region possibly worsened by a stress raiser effect at the inferior rim. The results of this study indicate that glenoid failure can be delayed or prevented by improving the implant/cement interface strength. Also any design features that reduce the geometrical stress raiser and the inferior tensile stresses in general should delay implant loosening.

There is paucity of reliable contemporary data on prevalence of coronary artery disease (CAD) and risk factors in Indians. Only a few studies on prevalence of CAD have been conducted in Kerala, a Southern Indian state. The main objective of the Cardiological Society of India Kerala Chapter Coronary Artery Disease and Its Risk Factors Prevalence Study (CSI Kerala CRP Study) was to determine the prevalence of CAD and risk factors of CAD in men and women aged 20-79 years in urban and rural settings of three geographical areas of Kerala.

Sinus of valsalva aneurysm is considered to be one of the rarest complications of inflammatory aortitis. Herewith, we are reporting a young male patient who presented to us with severe aortic regurgitation. On evaluation, he was found to have unruptured sinus of valsalva aneurysm. CT angiography and magnetic resonance imaging have shown value in the diagnosis of sinus of valsalva aneurysm.
