Radiolabeled Pittsburgh compound B (PIB) is a benzothiazole imaging agent that usually binds with high affinity, specificity, and stoichiometry to cerebral beta-amyloid (Abeta) in patients with Alzheimer's disease. Among a cohort of ten AD subjects examined postmortem, we describe a case of idiopathic, end-stage Alzheimer's disease with heavy Abeta deposition yet substantially diminished high-affinity binding of (3)H-PIB to cortical homogenates and unfixed cryosections. Cortical tissue samples were analyzed by immunohistochemistry, electron microscopy, ELISA, immunoblotting, MALDI-TOF mass spectrometry, in vitro (3)H-PIB binding and (3)H-PIB autoradiography. The PIB-refractory subject met the histopathological criteria for AD. However, cortical tissue from this case contained more vascular beta-amyloidosis, higher levels of insoluble Abeta40 and Abeta42, and a higher ratio of Abeta40:Abeta42 than did tissue from the nine comparison AD cases. Furthermore, cerebral Abeta from the PIB-refractory subject displayed an unusual distribution of low- and high-molecular weight Abeta oligomers, as well as a distinct pattern of N- and C-terminally truncated Abeta peptides in both the soluble and insoluble cortical extracts. Genetically, the patient was apolipoprotein-E3/4 heterozygous, and exhibited no known AD-associated mutations in the genes for the beta-amyloid precursor protein, presenilin1 or presenilin2. Our findings suggest that PIB may differentially recognize polymorphic forms of multimeric Abeta in humans with Alzheimer's disease. In addition, while the prevalence of PIB-refractory cases in the general AD population remains to be determined, the paucity of high-affinity binding sites in this AD case cautions that minimal PIB retention in positron-emission tomography scans of demented patients may not always rule out the presence of Alzheimer-type Abeta pathology.

Deficient high-affinity binding of Pittsburgh compound B in a case of Alzheimer's disease.

Toutefois, le tissu cortical de cette affaire contenait plus de b&#xEA;ta-amylo&#xEF;de vasculaire, des niveaux plus &#xE9;lev&#xE9;s de Abeta40 insoluble et Abeta42, et un ratio plus &#xE9;lev&#xE9; de Abeta40: Abeta42 que fait le tissu des neuf cas de MA de comparaison. Radioaktive Pittsburgh compound B (PIB) ist ein Benzothiazol imaging-Agent, der in der Regel mit hoher Affinit&auml;t, Spezifit&auml;t und St&ouml;chiometrie zu zerebralen Beta-Amyloid (Abeta) bei Patienten mit Alzheimer-Krankheit bindet. Unter einer Kohorte von 22 Themen abschlie&szlig;ende Analyse gepr&uuml;ft, wir beschreiben einen Fall von idiopathischer, Endstadium der Alzheimer-Krankheit mit schweren Abeta-Abscheidung noch erheblich vermindert hoher Affinit&auml;t Binden von (3) H-PIB, kortikale Homogenates und unfixierten Cryosections. Radiomarcado Compuesto Pittsburgh B (PIB) es un agente de imagen benzotiazol que por lo general se une con gran afinidad y especificidad, y la estequiometr&#xED;a de amiloide cerebral (Abeta) en pacientes con enfermedad de Alzheimer. Entre un grupo de diez sujetos AD examinados post-mortem, se describe el caso de la enfermedad idiop&#xE1;tica, en fase terminal de Alzheimer con gran deposici&#xF3;n de Abeta pero sustancialmente disminuida de alta afinidad de uni&#xF3;n de H (3)-PIB de homogeneizados corticales y cryosections no fijadas. Muestras de tejidos corticales fueron analizados mediante t&#xE9;cnicas de inmunohistoqu&#xED;mica, microscop&#xED;a electr&#xF3;nica, ELISA, inmunotransferencia, MALDI-TOF espectrometr&#xED;a de masas, in vitro (3) H-PIB vinculante y (3) H-PIB autorradiograf&#xED;a. El tema PIB-refractaria cumplieron con los criterios histopatol&#xF3;gicos para la EA. Sin embargo, el tejido cortical de este caso conten&#xED;a m&#xE1;s beta-vascular, la amiloidosis, un mayor nivel de insolubles Abeta40 y Abeta42, y una mayor proporci&#xF3;n de Abeta40: Abeta42 que hizo de tejido de los nueve casos de EA de comparaci&#xF3;n. Adem&#xE1;s, Abeta cerebral del sujeto BIP-refractario muestra una distribuci&#xF3;n inusual de bajo y alto peso molecular olig&#xF3;meros Abeta, as&#xED; como un patr&#xF3;n distinto de N-y C-terminal truncado p&#xE9;ptidos Abeta tanto en los extractos corticales solubles e insolubles. Gen&#xE9;ticamente, la paciente fue la apolipoprote&#xED;na-E3 / 4 heterocigotos, y no mostraron conocidos asociados AD-mutaciones en los genes de la prote&#xED;na precursora de beta-amiloide, o presenilin1 presenilin2. Nuestros hallazgos sugieren que la diferencial de PIB pueden reconocer formas polim&#xF3;rficas de Abeta multim&#xE9;rica en los seres humanos con enfermedad de Alzheimer. Adem&#xE1;s, mientras que la prevalencia de PIB-refractarios casos en la poblaci&#xF3;n general de AD que queda por determinar, la escasez de sitios de alta afinidad de uni&#xF3;n en este caso AD advierte que la retenci&#xF3;n m&#xED;nima en el PIB de la tomograf&#xED;a por emisi&#xF3;n de positrones de los pacientes con demencia puede no ser siempre descartar la presencia de la enfermedad de Alzheimer de tipo Abeta patolog&#xED;a.
