The endemic spread of tuberculosis after World War II and the deficiency of appropriate antituberculous drugs had led to a renaissance of the surgical tuberculosis therapy until the early 1950s. Late complications of plombage performed decades before are rare and are mainly related to infection and/or migration of the inserted foreign material and are scarcely recognized today. We report on a 73-year-old male patient, who was admitted to the emergency room of our hospital with acute massive haemoptysis for four days. On physical examination the patient presented with decreased breath sounds over the left lung and an old left-sided thoracotomy scar. Radiological findings and bronchoscopy revealed an empyema and a fistula as late complications 53 years after collapse therapy with insertion of a plombage for the treatment of pulmonary tuberculosis. The endobronchial nylon threads in the left bronchial tree and the fistula ending in the left lower bronchus confirmed our diagnosis. The patient was successfully treated by resection of the affected lower lobe. The present casuistic demonstrates a rare cause of spontaneous haemoptysis: late complications after extrapleural pneumolysis and plombage for cavitary tuberculosis over 50 years after the initial operation.

La propagation end&eacute;mique de la tuberculose apr&egrave;s la seconde guerre mondiale et l&#39;insuffisance de m&eacute;dicaments antituberculeux appropri&eacute;s avaient conduit &agrave; une renaissance de la th&eacute;rapie chirurgicale de la tuberculose, jusqu&#39;au d&eacute;but des ann&eacute;es 1950. Les complications tardives des d&eacute;cennies de plombage effectu&eacute; avant sont rares et sont principalement li&eacute;es &agrave; l&#39;infection et/ou de la migration du mat&eacute;riel &eacute;tranger ins&eacute;r&eacute; et sont rarement reconnues aujourd&#39;hui. Nous rapportons un patient m&acirc;le &acirc;g&eacute; de 73 ans, qui a &eacute;t&eacute; admis aux urgences de l&#39;h&ocirc;pital avec une h&eacute;moptysie massive aigu&euml; pendant quatre jours. &Agrave; l&#39;examen physique du patient s&#39;est pr&eacute;sent&eacute; avec diminu&eacute; sons souffle le poumon gauche et une vieille cicatrice de thoracotomie c&ocirc;t&eacute; gauche. Bronchoscopie et constatations radiologiques ont r&eacute;v&eacute;l&eacute; un empy&egrave;me et une fistule comme complications tardives 53 ans apr&egrave;s l&#39;effondrement de th&eacute;rapie avec l&#39;insertion d&#39;un plombage pour le traitement de la tuberculose pulmonaire. Les fils de nylon endobronchique dans l&#39;arbre bronchique gauche et la fistule se terminant dans la bronche inf&eacute;rieure gauche a confirm&eacute; notre diagnostic. Le patient a &eacute;t&eacute; trait&eacute; avec succ&egrave;s par r&eacute;section du lobe inf&eacute;rieur touch&eacute;e. Le pr&eacute;sent relevant de la casuistique montre une cause rare d&#39;h&eacute;moptysie spontan&eacute;e : complications tardives apr&egrave;s extrapleural pneumolysis et plombage pour la tuberculose cavitaire plus de 50 ans apr&egrave;s la mise en service.

Die endemischen Ausbreitung der Tuberkulose nach dem zweiten Weltkrieg und der Mangel an geeigneten antituberculous Drogen f&uuml;hrten zu einer Renaissance der chirurgischen Tuberkulose-Therapie bis Anfang der 1950er Jahre. Sp&auml;tfolgen der Plombage durchgef&uuml;hrt Jahrzehnte vor sind selten und beziehen sich haupts&auml;chlich auf Infektionen und/oder Migration des eingef&uuml;gten fremden Materials und werden heute kaum erkannt. Wir berichten &uuml;ber einen 73 Jahre alten m&auml;nnlichen Patienten f&uuml;r vier Tage in die Notaufnahme unseres Krankenhauses mit akuten massive H&auml;moptyse aufgenommen wurde. Auf k&ouml;rperliche Untersuchung sank der Patient pr&auml;sentiert mit Atem Ger&auml;usche &uuml;ber der linken Lunge und eine alte Narbe Links-seitig Thorakotomie. Radiologische Befunde und Bronchoskopie ergab ein Empyem und eine Fistel als Sp&auml;tkomplikationen 53 Jahre nach Zusammenbruch-Therapie mit der Einf&uuml;gung von einer Plombage zur Behandlung der Lungentuberkulose. Endobronchialen Nylon Threads im linken Bronchialbaumes und die Fistel endet in der linken unteren Bronchien best&auml;tigt unsere Diagnose. Der Patient wurde erfolgreich von Resektion der betroffenen unteren Lobus behandelt. Kasuistische Gegenwart veranschaulicht eine seltene Ursache der spontanen H&auml;moptyse: Sp&auml;tfolgen nach Extrapleural Pneumolysis und Plombage f&uuml;r Cavitary Tuberkulose &uuml;ber 50 Jahre nach der Inbetriebnahme.

La diffusione endemica della tubercolosi dopo la II guerra mondiale e la carenza di farmaci antitubercolare appropriate aveva condotto ad una rinascita della terapia chirurgica della tubercolosi fino ai primi anni cinquanta. Tarda complicazioni di sigillo eseguita decenni prima sono rare e sono principalmente correlate all&#39;infezione e/o migrazione del materiale straniero inserito e a malapena sono riconosciute oggi. Segnaliamo su un paziente maschio di 73 anni, che &egrave; stato ricoverato al pronto soccorso del nostro ospedale con acuta emottisi massive per quattro giorni. Su esame fisico del paziente ha presentato con diminuito suoni respiro sopra il polmone di sinistra e una cicatrice vecchia toracotomia sinistra-parteggiato. Broncoscopia e radiologici ha rivelato un empiema e una fistola come complicanze tarda 53 anni dopo il crollo di terapia con l&#39;inserimento di un sigillo per il trattamento della tubercolosi polmonare. I fili di nylon endobronchiale sinistro albero bronchiale e la fistola che termina con il Bronco sinistro inferiore ha confermato la diagnosi. Il paziente &egrave; stato trattato con successo da resezione del lobo inferiore interessato. Il presente casistico dimostra una rara causa di emottisi spontanea: tarda complicazioni dopo pneumolysis extrapleurica e sigillo per tubercolosi cavitarie oltre 50 anni dopo l&#39;operazione iniziale.

La propagaci&oacute;n end&eacute;mica de tuberculosis despu&eacute;s de la segunda guerra mundial y la carencia de medicamentos antituberculosos adecuados hab&iacute;an conducido a un renacimiento de la terapia quir&uacute;rgica tuberculosis hasta principios de los cincuentas. Complicaciones tard&iacute;as de d&eacute;cadas convierten realizado antes son raras y est&aacute;n principalmente relacionados con infecci&oacute;n y/o migraci&oacute;n de la insertado material extra&ntilde;o y apenas se reconocen hoy. Divulgamos sobre un paciente masculino de 73 a&ntilde;os de edad, que ingres&oacute; a la sala de urgencias de nuestro hospital con hemoptisis masiva aguda durante cuatro d&iacute;as. En la examinaci&oacute;n f&iacute;sica el paciente presentado con disminuci&oacute;n de ruidos respiratorios sobre el pulm&oacute;n izquierdo y una vieja cicatriz de la toracotom&iacute;a izquierda. Broncoscopia y resultados radiol&oacute;gicos revelaron un empiema y una f&iacute;stula como complicaciones tard&iacute;as 53 a&ntilde;os despu&eacute;s de la terapia de colapso con la inserci&oacute;n de un plombage para el tratamiento de la tuberculosis pulmonar. Los hilos de nylon endobronquial en el &aacute;rbol bronquial izquierdo y la f&iacute;stula que terminan en el bronquio izquierdo inferior confirman nuestro diagn&oacute;stico. El paciente fue tratado con &eacute;xito por la resecci&oacute;n del l&oacute;bulo inferior afectado. El presente casu&iacute;stico demuestra una causa rara de la hemoptisis espont&aacute;nea: complicaciones tard&iacute;as despu&eacute;s extrapleural pneumolysis y convierten para la tuberculosis cavitaria m&aacute;s de 50 a&ntilde;os despu&eacute;s de la operaci&oacute;n inicial.
