OBJECTIVES: Intra-arterial digital subtraction angiography (IA-DSA), an invasive procedure, is the current reference examination after percutaneous transluminal angioplasty and stenting for the detection of in-stent restenosis (ISR). In this phantom study, we evaluated flat-panel angiographic computed tomography after intravenous contrast agent application (IV-ACT) and multidetector computed tomographic angiography (MDCTA) as potential noninvasive follow-up alternatives after intracranial percutaneous transluminal angioplasty and stenting. MATERIALS AND METHODS: We simulated an intracranial vessel using a silicon tube placed inside a human skull. Three different stent systems were deployed inside the silicon tubes, each with diameters of 3 or 4 mm. Three grades of ISR (25%, 50%, and 75%) were simulated. The IA-DSA and IV-ACT examinations were performed on a flat-panel detector angiography system. The MDCTA images were acquired with a 128-slice computed tomographic scanner. The mean stenosis diameters, measured with each technique, were compared using the Bland-Altman plot. The difference between the known stenosis diameter and the measured stenosis diameter was calculated for each examination. RESULTS: Stenosis measurements on the IA-DSA images showed no statistically significant differences compared with the known stenosis diameters (P = 0.19). In the 3-mm stent category, when compared with the known stenosis diameter, mean (SD) differences of 0.01 (0.15) mm, 0.03 (0.24) mm and 0.16 (0.5) mm were calculated for the IA-DSA, IV-ACT, and MDCTA stenosis measurements, respectively. As for the 4-mm stents, IA-DSA and IV-ACT were again very accurate, with mean (SD) differences of -0.03 (0.11) mm and 0.07 (0.19) mm, respectively, compared with the known stenosis diameters, whereas MDCTA overestimated ISR, with a mean (SD) difference of 0.49 (0.53) mm. The Bland-Altman plots show a mean (SD) difference of 0.08 (0.2) mm between IA-DSA and IV-ACT (95% confidence interval, 0.05-0.11) and a mean (SD) difference of 0.34 (0.56) mm between IA-DSA and MDCTA measurements (95% confidence interval, 0.25-0.42). CONCLUSIONS: In our phantom study, IA-DSA was the only examination to predict accurately degrees of stenosis compared with the known stenosis diameters. The results of the IV-ACT measurements were comparable with those of IA-DSA. Multidetector computed tomographic angiography was less accurate in the quantification of stenosis, usually overestimating ISR.

Evaluation of Noninvasive Follow-up Methods for the Detection of Intracranial In-Stent Restenosis: A Phantom Study.

OBJECTIFS : L&#39;angiographie de soustraction num&eacute;rique intra-art&eacute;rielle (IA-DSA), une proc&eacute;dure invasive, est l&#39;examen de r&eacute;f&eacute;rence actuel apr&egrave;s angioplastie transluminale percutan&eacute;e et la pose d&#39;un stent pour la d&eacute;tection de rest&eacute;nose intrastent (ISR). Dans cette &eacute;tude fant&ocirc;me, nous avons &eacute;valu&eacute; la tomodensitom&eacute;trie angiographique &agrave; plat une fois demande d&#39;agent de contraste intraveineux (IV-ACT) et multid&eacute;tecteur calcul&eacute;es angiographie tomographique (MDCTA) comme solutions possibles de suivi non invasives apr&egrave;s la pose d&#39;un stent et angioplastie transluminale percutan&eacute;e intracr&acirc;nienne. MAT&Eacute;RIEL et m&eacute;thodes : Nous avons simul&eacute; un navire intracr&acirc;niens &agrave; l&#39;aide d&#39;un tube de silicone plac&eacute; &agrave; l&#39;int&eacute;rieur d&#39;un cr&acirc;ne humain. Trois syst&egrave;mes de stent diff&eacute;rents ont &eacute;t&eacute; d&eacute;ploy&eacute;s &agrave; l&#39;int&eacute;rieur des tubes de silicone, chacun avec un diam&egrave;tre de 3 ou 4 mm. Trois qualit&eacute;s de ISR (25 %, 50 % et 75 %) ont &eacute;t&eacute; simul&eacute;es. Les IA-DSA et les examens de l&#39;acte IV ont &eacute;t&eacute; effectu&eacute;es sur un syst&egrave;me d&#39;angiographie de d&eacute;tecteur de plat. Les images MDCTA ont &eacute;t&eacute; acquis avec un scanner tomographique calcul&eacute; de 128-tranche. Les diam&egrave;tres de st&eacute;nose moyenne, mesur&eacute;es &agrave; l&#39;aide de chaque technique, ont &eacute;t&eacute; compar&eacute;s en utilisant le Bland-Altman. La diff&eacute;rence entre le diam&egrave;tre de la st&eacute;nose connus et le diam&egrave;tre de la st&eacute;nose mesur&eacute; a &eacute;t&eacute; calcul&eacute;e pour chaque examen. R&Eacute;SULTATS : Les mesures st&eacute;nose sur les images de IA-DSA ne montr&eacute;s aucune diff&eacute;rence statistiquement significative compar&eacute;e avec les diam&egrave;tres de st&eacute;nose connu (P = 0,19). Dans la cat&eacute;gorie 3-mm stent, en comparaison avec le diam&egrave;tre de la st&eacute;nose connus, signifie des diff&eacute;rences (SD) de 0,01 (0,15) mm, 0,03 (0,24) mm et 0,16 (0,5) mm ont &eacute;t&eacute; calcul&eacute;s pour le IA-DSA, acte IV et des mesures de MDCTA de st&eacute;nose, respectivement. Ziele: Intraarterielle Digitale Subtraktionsangiographie (IA-DSA), ein invasives Verfahren ist die aktuelle Referenz-Untersuchung nach der Perkutane transluminale Angioplastie und Stenting zum Nachweis von in-Stent-Restenose (ISR). In dieser phantom Studie werteten wir Flachbildschirm angiographischen Computertomographie nach intraven&ouml;ser Kontrast-Agent-Anwendung (IV-ACT) und Multidetector computertomographische Angiographie (MDCTA) als potenzielle nicht-invasive Alternativen Follow-up nach intrakranielle Perkutane transluminale Angioplastie und Stenting berechnet. Material und Methoden: Wir simuliert eine intrakraniale Schiff mit einem Silizium-Rohr in einen menschlichen Sch&auml;del platziert. Drei unterschiedliche Stent-Systeme wurden in der Silizium-Rohre, jeweils mit 3 oder 4 mm Durchmesser eingesetzt. Drei Grade der ISR (25 %, 50 % und 75 %) wurden simuliert. Die IA-DSA und IV-ACT Pr&uuml;fungen wurden auf einem Flachdetektor-Angiographie-System durchgef&uuml;hrt. Die MDCTA-Bilder wurden mit einem 128-Segment berechneten tomographischen Scanner erworben. Die mittlere Stenose-Durchmesser, gemessen mit jeder Technik wurden verglichen mit den Bland Altman Plot. Der Unterschied zwischen der bekannten Stenose-Durchmesser und der gemessenen Stenose-Durchmesser wurde f&uuml;r jede Pr&uuml;fung berechnet. Ergebnisse: Stenose Messungen auf die IA-DSA-Bilder zeigte keine statistisch signifikante Unterschiede im Vergleich mit den bekannten Stenose-Durchmessern (P = 0.19). In der Kategorie 3-mm-Stent bedeuten im Vergleich zu den bekannten Stenose-Durchmesser (SD) Unterschiede von 0,01 (0.15) mm, 0,03 (0,24) mm und 0,16 (0,5) mm berechnet f&uuml;r die IA-DSA, IV-ACT und MDCTA Stenose Messungen, beziehungsweise. Wie f&uuml;r die 4-mm-Stents, IA-DSA und IV-ACT waren wieder sehr genau, mit Mittelwert (SD) unterschieden von 0,03 (0.11) mm und 0,07 (0.19) mm, bzw., verglichen mit den bekannten Stenose-Durchmessern, w&auml;hrend MDCTA ISR, mit einem Mittelwert (SD)-Unterschied von 0,49 (0,53) &uuml;bersch&auml;tzt mm. Die Bland-Altman-Plots zeigen einen Mittelwert (SD)-Unterschied von 0,08 (0,2) mm zwischen IA-DSA und IV-ACT (95 % Konfidenzintervall, 0.05-0.11) und ein Mittelwert (SD) Unterschied von 0,34 (0,56) mm IA-DSA und MDCTA Messungen (95 % Konfidenzintervall, 0.25-0,42). Schlussfolgerungen: In unserer Studie phantom war IA-DSA die einzige Pr&uuml;fung Grad der Stenose im Vergleich mit den bekannten Stenose-Durchmessern genau vorherzusagen. Die IV-ACT-Messergebnisse waren vergleichbar mit denen der IA-DSA. Multidetector berechnet war computertomographische Angiographie weniger genau in der Quantifizierung der Stenose, in der Regel &uuml;bersch&auml;tzt ISR.

Bewertung von nicht-invasive Follow-up-Methoden zum Nachweis von intrakraniellen In-Stent-Restenose: ein Phantom-Studie.

OBIETTIVI: L&#39;angiografia intra-arteriosa sottrazione digitale (DSA-IA), una procedura invasiva, &egrave; l&#39;esame di riferimento corrente dopo angioplastica transluminale percutanea e stenting per la rilevazione della ristenosi in-stent (ISR). In questo studio fantasma, abbiamo valutato la tomografia computerizzata angiografica flat-panel dopo applicazione agente di contrasto per via endovenosa (IV-ACT) e multidetettore computato angiografia tomografica (MDCTA) come potenziali alternative di follow-up non invasive dopo impianto di stent e angioplastica transluminale percutanea intracranica. MATERIALI e metodi: Abbiamo simulato un vaso intracranico utilizzando un tubo di silicone inserito all&#39;interno di un teschio umano. Tre sistemi diversi stent sono stati distribuiti all&#39;interno dei tubi di silicio, ciascuno con un diametro di 3 o 4 mm. Tre gradi di ISR (25%, 50% e 75%) sono stati simulati. L&#39;IA-DSA ed esami IV-ACT sono stati eseguiti su un sistema di angiografia flat-panel detector. Le immagini MDCTA sono state acquisite con uno scanner tomografica calcolato 128-fetta. I diametri medi stenosi, misurati con ogni tecnica, sono stati confrontati con la trama di Bland-Altman. La differenza tra il diametro della stenosi noto e il diametro misurato la stenosi &egrave; stata calcolata per ogni esame. RISULTATI: Misurazioni stenosi sulle immagini IA-DSA ha mostrate senza differenze statisticamente significative rispetto con i diametri di stenosi noto (P = 0.19). Nella categoria 3-mm stent, se confrontate con il diametro della stenosi noto, dire le differenze (SD) di 0,01 (0,15) mm, 0,03 (0,24) mm e 0,16 (0,5) mm sono state calcolate per l&#39;IA-DSA, atto IV e le misure di stenosi MDCTA, rispettivamente. Per quanto riguarda gli stent di 4 mm, IA-DSA e IV atto erano ancora molto accurata, con differenze di media (SD) di 0,03 (0,11) mm e 0,07 (0,19) mm, rispettivamente, confrontato con i diametri di stenosi noto, mentre MDCTA sovrastimato ISR, con una differenza media (SD) di 0,49 (0,53) mm. Le trame di Bland-Altman mostrano una differenza media (SD) di 0,08 (0,2) mm tra IA-DSA e IV-ACT (intervallo di confidenza 95%, 0,05-0,11) e una differenza media (SD) di 0,34 (0,56) mm tra IA-DSA e MDCTA misurazioni (95% intervallo di confidenza, 0,25-0,42). CONCLUSIONI: Nel nostro studio fantasma, IA-DSA &egrave; stato il solo esame di predire con precisione gradi di stenosi confrontato con i diametri di stenosi conosciuto. I risultati delle misurazioni ACT IV erano comparabili con quelli di IA-DSA. Multidetettore computato angiografia tomografica era meno preciso nella quantificazione della stenosi, solitamente sopravvalutando ISR.

MATERIALES y m&eacute;todos: Hemos simulado un vaso intracraneal usando un tubo de silicona dentro de un cr&aacute;neo humano. Tres sistemas diferentes de stent fueron desplegados dentro de los tubos de silicona, cada una con un di&aacute;metro de 3 o 4 mm. Se simularon tres grados de ISR (25%, 50% y 75%). El IA-DSA y ex&aacute;menes IV-ley fueron realizados en un sistema de angiograf&iacute;a detector de panel plano. Las im&aacute;genes MDCTA fueron adquiridas con un esc&aacute;ner tomogr&aacute;fico computado 128 divisiones. Los di&aacute;metros de estenosis media, medidos con cada t&eacute;cnica, se compararon mediante la trama de Bland-Altman. La diferencia entre el di&aacute;metro de la estenosis conocida y el di&aacute;metro medido de la estenosis se calcul&oacute; para cada examen. Resultados: Medidas estenosis en las im&aacute;genes de IA-DSA mostr&oacute; diferencias estad&iacute;sticamente significativas en comparaci&oacute;n con los di&aacute;metros de estenosis conocida (P = 0,19). En la categor&iacute;a de stent de 3 mm, en comparaci&oacute;n con el di&aacute;metro de la estenosis conocida, significa diferencias (SD) de 0.01 (0.15) mm, 0.03 (0,24) mm y 0.16 (0.5) mm se calcularon para el IA-DSA, IV-ACT y mediciones de estenosis MDCTA, respectivamente. En cuanto a los stents de 4 mil&iacute;metros, IA-DSA y IV-ley otra vez fueron muy precisos, con diferencias de media (SD) de -0,03 (0,11) mm y 0.07 (0,19) mil&iacute;metros, respectivamente, comparados con los di&aacute;metros de estenosis conocida, mientras que MDCTA sobreestimado ISR, con una diferencia de media (SD) de 0.49 (0,53) mm. Las parcelas de Bland-Altman muestran una diferencia de media (SD) de 0.08 (0,2) m entre IA-DSA, IV-ley (intervalo de confianza del 95%, 0.05-0,11) y una diferencia de media (SD) de 0.34 (0.56) mm entre las mediciones de IA-DSA y MDCTA (intervalo de confianza del 95%, 0.25-0.42). CONCLUSIONES: En nuestro estudio fantasma, IA-DSA fue el &uacute;nico examen para predecir con precisi&oacute;n grados de estenosis en comparaci&oacute;n con los di&aacute;metros de estenosis conocida. Los resultados de las mediciones de IV-ACT eran comparables con los de IA-DSA. Computarizada con multidetectores angiograf&iacute;a tomogr&aacute;fica fue menos precisa en la cuantificaci&oacute;n de la estenosis, suelen sobreestimar ISR

Patient selection is crucial in the endovascular treatment of acute ischemic stroke patients.
