Purpose:â€‚ Impaired consciousness in epileptic seizures has a major negative impact on patient quality of life. Prior work on epileptic unconsciousness has mainly used retrospective and nonstandardized methods. Our goal was to validate and to obtain initial data using a standardized prospective testing battery. Methods:â€‚ The responsiveness in epilepsy scale (RES) was used on 52 patients during continuous video-electroencephalography (EEG) monitoring. RES begins with higher-level questions and commands, and switches adaptively to more basic sensorimotor responses depending on patient performance. RES continues after seizures and includes postictal memory testing. Scoring was conducted based on video review. Key Findings:â€‚ Testing on standardized seizure simulations yielded good intrarater and interrater reliability. We captured 59 seizures from 18 patients (35% of participants) during 1,420â€ƒh of RES monitoring. RES impairment was greatest during and after tonic-clonic seizures, less in partial seizures, and minimal in auras and subclinical seizures. In partial seizures, ictal RES impairment was significantly greater if EEG changes were present. Maximum RES impairment (lowest ictal score) was also significantly correlated with long postictal recovery time, and poor postictal memory. Significance:â€‚ We found that prospective testing of responsiveness during seizures is feasible and reliable. RES impairment was related to EEG changes during seizures, as well as to postictal memory deficits and recovery time. With a larger patient sample it is hoped that this approach can identify brain networks underlying specific components of impaired consciousness in seizures. This may allow the development of improved treatments targeted at preventing dysfunction in these networks.

Signification : Nous avons trouv&eacute; que l&#39;&eacute;ventuel test de r&eacute;activit&eacute; durant les crises est r&eacute;alisable et fiable. D&eacute;ficience RES concernait les modifications EEG durant les crises, ainsi que des d&eacute;ficits de m&eacute;moire postictal et temps de r&eacute;cup&eacute;ration. Avec un &eacute;chantillon plus important de patient qu&#39;il est &agrave; esp&eacute;rer que cette approche peut identifier cerveau r&eacute;seaux sous-jacents des composants sp&eacute;cifiques de troubles de la conscience dans les saisies. Bedeutung: Wir fanden, dass potenzielle Pr&uuml;fung der Reaktionsf&auml;higkeit w&auml;hrend der Anf&auml;lle m&ouml;glich und zuverl&auml;ssig ist. RES wiederum wurde zu EEG-Ver&auml;nderungen w&auml;hrend der Anf&auml;lle sowie postictal Ged&auml;chtnisl&uuml;cken und Wiederherstellungszeit zusammenh&auml;ngt. Mit einer gr&ouml;&szlig;eren Patientenprobe, was zu hoffen ist Netzwerke, dass dieser Ansatz Gehirn identifizieren kann zugrunde liegenden spezifische Komponenten beeintr&auml;chtigt Bewusstseins Sicherstellungen. Dadurch kann die Entwicklung verbesserte Therapien, die gezielt auf die Vermeidung von Funktionsst&ouml;rungen in diese Netze.

Con un grande campione del paziente che si spera che questo approccio pu&ograve; identificare cervello reti sottostanti componenti specifici della coscienza alterata in convulsioni. Prop&oacute;sito: Alteraci&oacute;n de la conciencia en crisis epil&eacute;pticas tiene un impacto negativo importante en paciente calidad de vida. Trabajos previos en inconsciencia epil&eacute;ptico principalmente ha utilizado m&eacute;todos retrospectivos y no normalizados. Nuestro objetivo era para validar y obtener datos iniciales usando una bater&iacute;a de prueba estandarizada prospectiva. M&eacute;todos: La capacidad de respuesta en escala de epilepsia (RES) fue utilizado en 52 pacientes durante el monitoreo continua video-electroencefalograf&iacute;a (EEG). RES comienza con preguntas y comandos de m&aacute;s alto nivel y cambia de forma adaptativa a respuestas m&aacute;s b&aacute;sicas sensoriomotoras seg&uacute;n el desempe&ntilde;o del paciente. RES contin&uacute;a despu&eacute;s de las convulsiones e incluye pruebas de memoria postictal. Puntuaci&oacute;n se realiz&oacute; en base a la revisi&oacute;n del video. Resultados: Pruebas en estandardizan simulaciones de asimiento intrarater buena y confiabilidad interrater. Capturamos 59 convulsiones de 18 pacientes (35% de los participantes) en 1.420 h de vigilancia de la RES. Debilitaci&oacute;n de la RES fue mayor durante y despu&eacute;s de las convulsiones t&oacute;nico-cl&oacute;nicas, menos en convulsiones parciales y m&iacute;nima en auras y convulsiones subcl&iacute;nicas. En las convulsiones parciales, deterioro de RES ictal fue significativamente mayor si cambios EEG estaban presentes. M&aacute;ximo deterioro de RES (menor puntuaci&oacute;n ictal) fue tambi&eacute;n significativamente correlacionado con el tiempo de recuperaci&oacute;n largo postictal y la mala memoria postictal. Significado: Encontramos que posibles pruebas de capacidad de respuesta durante las convulsiones son factible y confiable. Debilitaci&oacute;n de la RES se relacion&oacute; con cambios EEG durante las convulsiones, as&iacute; como los d&eacute;ficits de memoria postictal y tiempo de recuperaci&oacute;n. Con una muestra mayor de paciente que se espera que este enfoque puede identificar cerebro redes subyacentes componentes espec&iacute;ficos de alteraci&oacute;n de la conciencia en las convulsiones. The incidence of cutaneous squamous cell carcinomas (SCCs) in immunosuppressed solid organ transplant recipients (SOTRs) is 65- to 250-fold greater than in the general population. In addition, SCC in SOTRs is more aggressive than in the general population. SOTRs must undergo skin cancer screenings at intervals based on their risk stratification. The incidence of SCC in SOTRs varies with the type, intensity, and duration of the immunosuppressive regimen. Notably, patients on sirolimus have lower incidence of SCC compared to patients on calcineurin inhibitors. Revision of immunosuppressive regimen to include sirolimus may be a viable preventative measure against SCC in SOTRs who are high at risk for developing SCCs. Retinoids are also emerging as a means of chemoprophylaxis against development of new SCCs in high-risk patients. Treatments of SCC include electrodesiccation and curettage, surgical resection, cryosurgery, radiation, and systemic chemotherapy such as 5-fluorouracil and cetuximab.
