PURPOSE: To evaluate the performance of a gas-cooled, high-powered microwave system. MATERIALS AND METHODS: Investigators performed 54 ablations in ex vivo bovine livers using three devices-a single 17-gauge cooled radiofrequency(RF) electrode; a cluster RF electrode; and a single 17-gauge, gas-cooled microwave (MW) antenna-at three time points (n = 6 at 4 minutes, 12 minutes, and 16 minutes). An approved in vivo study was performed using 13 domestic pigs. Hepatic ablations were performed using single applicators and the above-mentioned MW and RF generator systems at treatment times of 2 minutes (n = 7 MW, n = 6 RF), 5 minutes (n = 23 MW, n = 8 RF), 7 minutes (n = 11 MW, n = 6 RF), and 10 minutes (n = 7 MW, n = 9 RF). Mean transverse diameter and length of the ablation zones were compared using analysis of variance (ANOVA) with post-hoc t tests and Wilcoxon rank-sum tests. RESULTS: Single ex vivo MW ablations were larger than single RF ablations at all time points (MW mean diameter range 3.5-4.8 cm 4-16 minutes; RF mean diameter range 2.6-3.1 cm 4-16 minutes) (P < .05). There was no difference in mean diameter between cluster RF and MW ablations (RF 3.3-4.4 cm 4-16 minutes; P = .4-.9). In vivo lesion diameters for MW (and RF) were as follows: 2.6 cm Â± 0.72 (RF 1.5 cm Â± 0.14), 3.6 cm Â± 0.89 (RF 2.0 cm Â± 0.4), 3.4 cm Â± 0.87 (RF 1.8 cm Â± 0.23), and 3.8 cm Â± 0.74 (RF 2.1 cm Â± 0.3) at 2 minutes, 5 minutes, 7 minutes, and 10 minutes (P < .05 all time points). CONCLUSIONS: Gas-cooled, high-powered MW ablation allows the generation of large ablation zones in short times.

High-Powered Microwave Ablation with a Small-Gauge, Gas-Cooled Antenna: Initial Ex Vivo and In Vivo Results.

OBJECTIF: Pour &#xE9;valuer la performance d&#x26;#39;un refroidis au gaz, syst&#xE8;me micro-ondes de forte puissance. OBJETIVO: Evaluar el rendimiento de un refrigerado por gas, sistema de microondas de alta potencia. MATERIAL Y M&#xC9;TODOS: Los investigadores realizaron 54 ablaciones en los h&#xED;gados de los ex vivo de la especie bovina con tres dispositivos de un solo de calibre 17 se enfri&#xF3; de radiofrecuencia (RF) de los electrodos, un electrodo de radiofrecuencia grupo, y un solo de calibre 17, refrigerado por gas microondas (MW) de antena en tres momentos (n = 6 en 4 minutos, 12 minutos, y 16 minutos). De potencia de RF se aplic&#xF3; mediante impedancia basada en pulsos con un m&#xE1;ximo de 200 W de salida del generador. MW de potencia de 135 W a 2,45 GHz fue entregado de forma continua. Un estudio in vivo aprobado se realiz&#xF3; utilizando 13 cerdos dom&#xE9;sticos. Ablaciones hep&#xE1;ticas se realizaron utilizando aplicadores individuales y los sistemas antes mencionados MW y RF del generador en los tiempos de tratamiento de 2 minutos (n = 7 MW, n = 6 RF), 5 minutos (n = 23 MW, n = 8 RF), 7 minutos (n = 11 MW, n = 6 RF), y 10 minutos (n = 7 MW, n = 9 RF). La media de di&#xE1;metro transversal y la longitud de las zonas de ablaci&#xF3;n se compararon mediante an&#xE1;lisis de varianza (ANOVA) con post-hoc de pruebas t y pruebas de Wilcoxon Rank Sum. RESULTADOS: Single ex vivo ablaciones MW eran m&#xE1;s grandes que un solo ablaciones de RF en todos los puntos (MW significa rango de di&#xE1;metro 3.5-4.8 cm 4-16 minutos; RF significa rango de di&#xE1;metro 2.6-3.1 cm de 4-16 minutos) (p &#x26;lt;.05) . CONCLUSIONES: refrigerado por gas, de alta potencia de ablaci&#xF3;n MW permite la generaci&#xF3;n de grandes zonas de ablaci&#xF3;n en tiempos cortos.

Alta Potencia de ablaci&#xF3;n por microondas con un calibre peque&#xF1;o, refrigerado por gas Antena: ex vivo inicial y los resultados in vivo

Introducci&#xF3;n Presentamos nuestra experiencia en el uso de una soluci&#xF3;n de contraste yodado de hydrodissect estructuras adyacentes antes de la crioablaci&#xF3;n percut&#xE1;nea renal. Hidrodisecci&#xF3;n T&#xE9;cnica se llev&#xF3; a cabo antes de la crioablaci&#xF3;n con la colocaci&#xF3;n de un calibre 20, 15 cm de aguja gu&#xED;a en el retroperitoneo en la TC o la gu&#xED;a del ultrasonido seguida de una infusi&#xF3;n de dextrosa al 5% en agua y un 2% de contraste yodado entre el ri&#xF1;&#xF3;n y el &#xF3;rgano adyacente. Diez pacientes fueron sometidos a hidrodisecci&#xF3;n con una soluci&#xF3;n de contraste yodado en nuestra instituci&#xF3;n. El tama&#xF1;o medio del tumor fue de 3,1 &#xB1; 1,2 cm. Los &#xF3;rganos desplazadas incluye dos puntos (n = 7), intestino delgado (n = 1), p&#xE1;ncreas (n = 1) y en un caso tanto el colon y el ur&#xE9;ter fueron desplazados. El desplazamiento promedio de todos los &#xF3;rganos del ri&#xF1;&#xF3;n fue de 2,8 cm (rango de 2.2-3.5cm). No hubo complicaciones y lesiones no a las estructuras adyacentes. Las conclusiones de la inyecci&#xF3;n de contraste yodado permite la movilizaci&#xF3;n segura y diferenciaci&#xF3;n de las estructuras adyacentes del tumor renal y par&#xE9;nquima que lleva a la crioablaci&#xF3;n potencialmente m&#xE1;s segura.

Introduction We describe our experience using an iodinated contrast solution to hydrodissect adjacent structures prior to percutaneous renal cryoablation. Technique Hydrodissection was performed prior to cryoablation with placement of a 20-gauge 15 cm introducer needle into the retroperitoneum under CT or ultrasound guidance followed by infusion of 5% dextrose in water and 2% iodinated contrast between the kidney and the adjacent organ. Ten patients underwent hydrodissection with an iodinated contrast solution at our institution. The mean tumor size was 3.1Â±1.2cm. The organs displaced included colon (n=7), small bowel (n=1), pancreas (n=1) and in one case both the colon and ureter were displaced. The average displacement of all organs from the kidney was 2.8 cm (range 2.2-3.5cm). There were no complications and no injuries to any adjacent structures. Conclusions The injection of iodinated contrast allows for safe mobilization and differentiation of adjacent structures from the renal tumor and parenchyma leading to potentially safer cryoablation.

Hydrodissection Utilizing an Iodinated Contrast Medium During Percutaneous Renal Cryoablation.

Introduction Nous d&#xE9;crivons notre exp&#xE9;rience en utilisant une solution de contraste iod&#xE9; &#xE0; hydrodissect structures adjacentes avant percutan&#xE9;e cryoablation r&#xE9;nale. Hydrodissection Technique a &#xE9;t&#xE9; r&#xE9;alis&#xE9;e avant la cryoablation avec le placement d&#x26;#39;un de calibre 20 aiguille 15 cm introducteur dans le r&#xE9;trop&#xE9;ritoine sous scanner ou &#xE9;chographie suivie d&#x26;#39;une perfusion de dextrose &#xE0; 5% dans de l&#x26;#39;eau et 2% de contraste iod&#xE9; entre le rein et l&#x26;#39;organe adjacent. Dix patients ont subi une hydrodissection avec une solution de contraste iod&#xE9; dans notre institution. La taille tumorale moyenne &#xE9;tait de 3,1 &#xB1; 1,2 cm. Les organes d&#xE9;plac&#xE9;es inclus du c&#xF4;lon (n = 7), l&#x26;#39;intestin gr&#xEA;le (n = 1), pancr&#xE9;as (n = 1) et dans un cas &#xE0; la fois le colon et de l&#x26;#39;uret&#xE8;re ont &#xE9;t&#xE9; d&#xE9;plac&#xE9;es. Le d&#xE9;placement moyen de tous les organes du rein &#xE9;tait de 2,8 cm (plage de 2,2 3.5cm). Il n&#x26;#39;y avait pas de complications et pas de blessures subies par les structures adjacentes. Conclusions L&#x26;#39;injection de produit de contraste iod&#xE9; permet la mobilisation s&#xE9;curitaire et la diff&#xE9;renciation des structures adjacentes de la tumeur et le parenchyme r&#xE9;nal conduisant &#xE0; la cryoablation potentiellement plus s&#xFB;r.

Einf&uuml;hrung beschreiben wir unsere Erfahrungen mit einem jodierten Kontrast-L&ouml;sung zu Hydrodissect angrenzenden Geb&auml;uden vor perkutane renale Kryoablation. Technik-Hydrodissection war vor Kryoablation durch die Platzierung von einer 20-St&auml;rke 15 cm Introducer Nadel in das Retroperitoneum unter CT oder Ultraschall Anleitung gefolgt von Infusion von 5 % Dextrose in Wasser und 2 % jodierten Kontrast zwischen der Niere und der angrenzenden Orgel durchgef&uuml;hrt. Zehn Patienten wurden Hydrodissection mit einer jodierten Kontrast L&ouml;sung an unserer Institution. Die mittlere Tumor betrug 3.1&plusmn;1.2 cm. Die Organe verdr&auml;ngt inklusive Doppelpunkt (n = 7), D&uuml;nndarm (n = 1), Pankreas (n = 1) und in einem Fall der Doppelpunkt und der Harnleiter verdr&auml;ngt wurden. Die durchschnittliche Verschiebung aller Organe aus der Niere war 2,8 cm (Bereich 2.2-3.5 cm). Es gab keine Komplikationen und keine Verletzungen, keine benachbarten Strukturen. Schlussfolgerungen erm&ouml;glicht sichere Mobilisierung und Differenzierung von angrenzenden Geb&auml;uden vom Nierentumor, seltener und Parenchym zu potentiell sicherer Kryoablation f&uuml;hrt die Injektion von jodierten Kontrast.

Introduzione descriviamo la nostra esperienza utilizzando una soluzione di contrasto iodati per strutture adiacenti hydrodissect prima della crioablazione renale percutanea. Hydrodissection tecnica &egrave; stata eseguita prima della crioablazione con posizionamento di un ago introduttore 15 cm 20-gauge nel retroperitoneo sotto guida TC o ecografia, seguita da infusione di destrosio 5% in acqua e 2% di contrasto iodati tra il rene e l&#39;organo adiacente. Dieci pazienti hanno subito hydrodissection con una soluzione di contrasto iodati alla nostra istituzione. La dimensione media del tumore era 3.1&plusmn;1.2 cm. Gli organi spostati incluso del colon (n = 7), dell&#39;intestino tenue (n = 1), pancreas (n = 1) e in un caso sono stati spostati il colon e l&#39;uretere. Lo spostamento medio di tutti gli organi dal rene era 2,8 cm (range 2.2-3.5 cm). C&#39;erano complicazioni e nessun ferito per eventuali strutture adiacenti. Conclusioni l&#39;iniezione di contrasto iodato permette di mobilitazione sicuro e la differenziazione delle strutture adiacenti il tumore renale e parenchima portando a crioablazione potenzialmente pi&ugrave; sicuro.

The advent of gadolinium-based "hepatobiliary" contrast agents offers new opportunities for diagnostic magnetic resonance imaging (MRI) and has triggered great interest for innovative imaging approaches to the liver and bile ducts. In this review article we discuss the imaging properties of the two gadolinium-based hepatobiliary contrast agents currently available in the U.S., gadobenate dimeglumine and gadoxetic acid, as well as important pharmacokinetic differences that affect their diagnostic performance. We review potential applications, protocol optimization strategies, as well as diagnostic pitfalls. A variety of illustrative case examples will be used to demonstrate the role of these agents in detection and characterization of liver lesions as well as for imaging the biliary system. Changes in MR protocols geared toward optimizing workflow and imaging quality are also discussed. La llegada de los agentes basados &#x200B;&#x200B;en gadolinio &#x26;quot;hepatobiliares&#x26;quot; contraste ofrece nuevas oportunidades para el diagn&#xF3;stico por imagen de resonancia magn&#xE9;tica (MRI) y ha generado gran inter&#xE9;s por los enfoques innovadores para la formaci&#xF3;n de im&#xE1;genes de los conductos del h&#xED;gado y la bilis. En este art&#xED;culo se discuten las propiedades de las im&#xE1;genes de los dos basados &#x200B;&#x200B;en gadolinio agentes de contraste hepatobiliar actualmente disponibles en el &#xE1;cido EE.UU., dimeglumina gadobenato y gadoxetato, as&#xED; como importantes diferencias farmacocin&#xE9;ticas que afectan a su rendimiento diagn&#xF3;stico. Se revisan las aplicaciones potenciales, estrategias de optimizaci&#xF3;n del protocolo, as&#xED; como las trampas de diagn&#xF3;stico. Una variedad de ejemplos ilustrativos de casos se utilizar&#xE1; para demostrar el papel de estos agentes en la detecci&#xF3;n y caracterizaci&#xF3;n de las lesiones hep&#xE1;ticas, as&#xED; como para formar im&#xE1;genes del sistema biliar.
