We present the analysis of a prospective multicentre study to investigate genetic effects on the prognosis of newly treated epilepsy. Patients with a new clinical diagnosis of epilepsy requiring medication were recruited and followed up prospectively. The clinical outcome was defined as freedom from seizures for a minimum of 12 months in accordance with the consensus statement from the International League Against Epilepsy (ILAE). Genetic effects on remission of seizures after starting treatment were analysed with and without adjustment for significant clinical prognostic factors, and the results from each cohort were combined using a fixed-effects meta-analysis. After quality control (QC), we analysed 889 newly treated epilepsy patients using 472 450 genotyped and 6.9 × 10(6) imputed single-nucleotide polymorphisms. Suggestive evidence for association (defined as Pmeta < 5.0 × 10(-7)) with remission of seizures after starting treatment was observed at three loci: 6p12.2 (rs492146, Pmeta = 2.1 × 10(-7), OR[G] = 0.57), 9p23 (rs72700966, Pmeta = 3.1 × 10(-7), OR[C] = 2.70) and 15q13.2 (rs143536437, Pmeta = 3.2 × 10(-7), OR[C] = 1.92). Genes of biological interest at these loci include PTPRD and ARHGAP11B (encoding functions implicated in neuronal development) and GSTA4 (a phase II biotransformation enzyme). Pathway analysis using two independent methods implicated a number of pathways in the prognosis of epilepsy, including KEGG categories 'calcium signaling pathway' and 'phosphatidylinositol signaling pathway'. Through a series of power curves, we conclude that it is unlikely any single common variant explains >4.4% of the variation in the outcome of newly treated epilepsy. 

A genome-wide association study and biological pathway analysis of epilepsy prognosis in a prospective cohort of newly treated epilepsy.

Rapid transmembrane flow of sodium ions produces the depolarising phase of action potentials (APs) in most excitable tissue through voltage-gated sodium channels (NaV). Macroscopic currents display rapid activation followed by fast inactivation (IF) within milliseconds. Slow inactivation (IS) has been subsequently observed in several preparations including neuronal tissues. IS serves important physiological functions but the kinetic properties are incompletely characterised, especially the operative time-scales. Here we characterise for an "intermediate inactivation (II)" process in rat hippocampal CA1 neurons with time constants of the order of 101-102 ms. The half-inactivation potential (V0.5) of steady-state inactivation curves were hyperpolarised by increasing conditioning pulse duration from 50 to 500 ms. II state transitions were observed after opening as well as subthreshold potentials. Entry into II after opening was relatively insensitive to membrane potential and recovery of II became more rapid at hyperpolarised potentials. Near resting membrane potential it is estimated that 40% of channels occupy this intermediate state. II recovery was considerably slower from closed inactivated states than from inactivated states reached through the open state. Removal of IF with cytoplasmic papaine revealed time constants of INa decay corresponding to II and IS with long depolarisations. Dynamic clamp revealed attenuation of trains of APs over the 10(2) ms timescale suggesting a functional role of II in repetitive firing accommodation. These experimental findings could be reproduced with a Markov state model. It is likely that II affects several important aspects of hippocampal neuron response, and may provide a drug target for sodium channel modulation.

Properties of an Intermediate-duration Inactivation Process of the Voltage-gated Sodium Conductance in Rat Hippocampal CA1 Neurons.

Fracture risk is a serious comorbidity in epilepsy and may relate to the use of antiepileptic drugs (AEDs). Many AEDs inhibit ion channel function, and the expression of these channels in osteoblasts raises the question of whether altered bone signaling increases bone fragility. We aimed to confirm the expression of voltage-gated sodium (NaV ) channels in mouse osteoblasts, and to investigate the action of carbamazepine and phenytoin on NaV channels.

The antiepileptic medications carbamazepine and phenytoin inhibit native sodium currents in murine osteoblasts.

The antiepileptic drug phenytoin (PHT) is thought to reduce the excitability of neural tissue by stabilizing sodium channels (Na) in inactivated states. It has been suggested the fast-inactivated state (I) is the main target, although slow inactivation (I) has also been implicated. Other studies on local anesthetics with similar effects on sodium channels have implicated the Na voltage sensor interactions. In this study, we reexamined the effect of PHT in both equilibrium and dynamic transitions between fast and slower forms of inactivation in rat hippocampal CA1 pyramidal neurons. The effects of PHT were observed on fast and slow inactivation processes, as well as on another identified "intermediate" inactivation process. The effect of enzymatic removal of I was also studied, as well as effects on the residual persistent sodium current (I). A computational model based on a gating charge interaction was derived that reproduced a range of PHT effects on Na equilibrium and state transitions. No effect of PHT on I was observed; rather, PHT appeared to facilitate the occupancy of other closed states, either through enhancement of slow inactivation or through formation of analogous drug-bound states. The overall significance of these observations is that our data are inconsistent with the commonly held view that the archetypal Na channel inhibitor PHT stabilizes fast inactivation states, and we demonstrate that conventional slow activation "I" and the more recently identified intermediate-duration inactivation process "I" are the primary functional targets of PHT. In addition, we show that the traditional explanatory frameworks based on the "modulated receptor hypothesis" can be substituted by simple, physiologically plausible interactions with voltage sensors. Additionally, I was not preferentially inhibited compared with peak I at short latencies (50 ms) by PHT.

Effect of phenytoin on sodium conductances in rat hippocampal CA1 pyramidal neurons.

In this study, we investigated the relative efficacy of different classes of commonly used anti-epileptic drugs (AEDs) with different mechanisms of action, individually and in combination, to suppress epileptiform discharges in an in vitro model. Extracellular field potential were recorded in 450 μm thick transverse hippocampal slices prepared from juvenile Wistar rats, in which "epileptiform discharges" (ED's) were produced with a high-K+ (8.5 mM) bicarbonate-buffered saline solution. Single and dual recordings in stratum pyramidale of CA1 and CA3 regions were performed with 3-5 MΩ glass microelectrodes. All drugs-lamotrigine (LTG), phenytoin (PHT) and valproate (VPA)-were applied to the slice by superfusion at a rate of 2 ml/min at 32°C. Effects upon frequency of ED's were assessed for LTG, PHT and VPA applied at different concentrations, in isolation and in combination. We demonstrated that high-K+ induced ED frequency was reversibly reduced by LTG, PHT and VPA, at concentrations corresponding to human therapeutic blood plasma concentrations. With a protocol using several applications of drugs to the same slice, PHT and VPA in combination displayed additivity of effect with 50μM PHT and 350μM VPA reducing SLD frequency by 44% and 24% individually (n = 19), and together reducing SLD frequency by 66% (n = 19). 20μM LTG reduced SLD frequency by 32% and 350μM VPA by 16% (n = 18). However, in combination there was a supra-linear suppression of ED's of 64% (n = 18). In another independent set of experiments, similar results of drug combination responses were also found. In conclusion, a combination of conventional AEDs with different mechanisms of action, PHT and VPA, displayed linear additivity of effect on epileptiform activity. More intriguingly, a combination of LTG and VPA considered particularly efficacious clinically showed a supra-additive suppression of ED's. This approach may be useful as an in vitro platform for assessing drug combination efficacy.

Anti-Epileptic Drug Combination Efficacy in an In Vitro Seizure Model - Phenytoin and Valproate, Lamotrigine and Valproate.

To determine the utility of high-frequency activity (HFA) and epileptiform spikes as biomarkers for epilepsy, we examined the variability in their rates and locations using long-term ambulatory intracranial EEG (iEEG) recordings.

Spatiotemporal Patterns of High-Frequency Activity (80-170 Hz) in Long-Term Intracranial EEG.

The hippocampus and associated cholinergic inputs have important roles in spatial memory in rodents. Muscarinic acetylcholine receptors (mAChRs) are involved in the communication of cholinergic signals and regulate spatial memory. They have been found to impact the memory encoding process, but the effect on memory retrieval is controversial. Previous studies report that scopolamine (a non-selective antagonist of mAChR) induces cognitive deficits on animals, resulting in impaired memory encoding, but the effect on memory retrieval is less certain. We tested the effects of blocking mAChRs on hippocampal network activity and neural ensembles that had previously encoded spatial information. The activity of hundreds of neurons in mouse hippocampal CA1 was recorded using calcium imaging with a miniaturised fluorescent microscope and properties of place cells and neuronal ensemble behaviour in a linear track environment were observed. We found that the decoding accuracy and the stability of spatial representation revealed by hippocampal neural ensemble were significantly reduced after the administration of scopolamine. Several other parameters, including neural firing rate, total number of active neurons, place cell number and spatial information content were affected. Similar results were also observed in a simulated hippocampal network model. This study enhances the understanding of the function of mAChRs on spatial memory impairment.

Scopolamine Impairs Spatial Information Recorded With "Miniscope" Calcium Imaging in Hippocampal Place Cells.

Gamma and theta oscillations have been functionally associated with cognitive processes, such as learning and memory. Synaptic conductances play an important role in the generation of intrinsic network rhythmicity, but few studies have examined the effects of voltage-gated ion channels (VGICs) on these rhythms. In this report, we have used a pyramidal-interneuron-gamma (PING) network consisting of excitatory pyramidal cells and two types of inhibitory interneurons. We have constructed a conductance-based neural network incorporating a persistent sodium current (  ), a delayed rectifier potassium current (  ), a inactivating potassium current (  ) and a hyperpolarization-activated current (  ). We have investigated the effects of several conductances on network theta and gamma frequency oscillations. Variation of all conductances of interest changed network rhythmicity. Theta power was altered by all conductances tested. Gamma rhythmogenesis was dependent on   and   . The   currents in excitatory pyramidal cells as well as both types of inhibitory interneurons were essential for theta rhythmogenesis and altered gamma rhythm properties. Increasing   suppressed both gamma and theta rhythms. Addition of noise did not alter these patterns. Our findings suggest that VGICs strongly affect brain network rhythms. Further investigations  will be of great interest, including potential effects on neural function and cognition.

Effects of Several Classes of Voltage-Gated Ion Channel Conductances on Gamma and Theta Oscillations in a Hippocampal Microcircuit Model.

Local field potentials (LFPs) recorded intracranially display a range of location-specific oscillatory spectra which have been related to cognitive processes. Although the mechanisms producing LFPs are not completely understood, it is likely that voltage-gated ion channels which produce action potentials and patterned discharges play a significant role. It is also known that antipsychotic drugs (APDs) affect LFP spectra and a direct inhibitory effect on voltage-gated potassium channels has been reported. Additionally, voltage-gated potassium channels have been implicated in the pathophysiology of schizophrenia, a disorder for which APDs are primary therapies. In this study we sought to: i) better characterise the effects of two APDs on LFPs spectra and connectivity measures and ii) examine the effects of potassium channel modulators on LFPs and potential overlap of effects with APDs. Intracranial electrodes were implanted in hippocampus (HIP) and pre-frontal cortex (PFC) of C57BL/6J mice; power spectra, coherence and phase-amplitude cross-frequency coupling were measured. Drugs tested were APDs haloperidol and clozapine as well as voltage-gated potassium channel modulators (KVMs) 4-aminopyridine (4-AP), tetraethylammonium, retigabine and E-4031. Both APDs and KVMs significantly reduced gamma power except 4-AP, which conversely increased gamma power. Clozapine and retigabine additionally reduced gamma coherence between HIP and PFC, while 4-AP demonstrated the opposite effect. Phase-amplitude coupling between theta and gamma oscillations in HIP was significantly reduced by the administration of haloperidol and retigabine. These results provide previously undescribed effects of APDs on LFP properties and demonstrate novel modulation of LFP characteristics by KVMs that intriguingly overlap with the APD effects.

Effects of antipsychotic drugs and potassium channel modulators on spectral properties of local field potentials in mouse hippocampus and pre-frontal cortex.

Oxidative stress is one of the pathophysiological mechanisms implicated in drug-resistant epilepsy. Recurrent seizures and prolonged treatment with anti-seizure medicines (ASMs) can produce reactive oxygen species (ROS) resulting in neuronal cell damage, cell toxicity, and cell death. This damage may contribute to the loss of efficacy of anti-seizure medicines. Add-on therapy with antioxidants, neuroimmunophilins, and polyphenols may thus be beneficial in drug-resistant epilepsy. In vitro and in vivo studies have shown a significant improvement in drug efficacy and seizure suppression using co-treatment of anti-seizure medication with naturally available antioxidants including alpha-lipoic acid (α-lipoic acid) from walnut; however, the underlying mechanisms of action remain to be fully understood.

Alpha-lipoic acid analogues in the regulation of redox balance in epilepsy: A molecular docking and simulation study.

Neurons receive synaptic input primarily onto their dendrites. While we know much about the electrical properties of dendrites in rodents, we have only just started to describe their properties in the human brain. Here, we investigate the capacity of human dendrites to generate NMDA-receptor-dependent spikes (NMDA spikes). Using dendritic glutamate iontophoresis, as well as local dendritic synaptic stimulation, we find that human layer 2/3 pyramidal neurons can generate dendritic NMDA spikes. The capacity to evoke NMDA spikes in human neurons, however, was significantly reduced compared with that in rodents. Simulations in morphologically realistic and simplified models indicated that human neurons have a higher synaptic threshold for NMDA spike generation primarily due to the wider diameter of their dendrites. In summary, we find reduced NMDA spike generation in human compared with rodent layer 2/3 pyramidal neurons and provide evidence that this is due to the wider diameter of human dendrites.

High synaptic threshold for dendritic NMDA spike generation in human layer 2/3 pyramidal neurons.

Two major theories have been proposed to explain hippocampal function: cognitive map and the relational theories. They differ in their views on whether hippocampal neurons can process non-spatial information independently. However, the explanatory power of these theories remains unresolved. Additionally, more complex aspects of hippocampal neural population responses to non-spatial stimuli have not been investigated. Here, we used miniaturized fluorescence microscopy to investigate mouse CA1 responses to spatial, visual, auditory modalities, and combinations. We found that while neuronal populations primarily processed spatial information, they also showed strong sensitivity to non-spatial modalities independent of spatial inputs, exhibiting distinct neuronal dynamics and coding patterns. These results provide strong support for the relational theories.
