Abstract Spinal muscular atrophy (SMA), an autosomal recessive neuromuscular disorder, is the leading genetic cause of infant mortality. SMA is caused by the homozygous loss of Survival Motor Neuron-1 (SMN1). In humans, a nearly identical copy gene is present, SMN2. SMN2 is retained in all SMA patients and encodes the same protein as SMN1. However, SMN1 and SMN2 differ by a silent C-to-T transition at the 5' end of exon 7, causing alternative splicing of SMN2 transcripts and low levels of full-length SMN. SMA is monogenic and therefore well suited for gene-replacement strategies. Recently, self-complementary adeno-associated virus (scAAV) vectors have been used to deliver the SMN cDNA to an animal model of disease, the SMNÎ”7 mouse. In this study, we examine a severe model of SMA, Smn(-/-);SMN2(+/+), to determine whether gene replacement is viable in a model in which disease development begins in utero. Using two delivery paradigms, intracerebroventricular injections and intravenous injections, we delivered scAAV9-SMN and demonstrated a two to four fold increase in survival, in addition to improving many of the phenotypic parameters of the model. This represents the longest extension in survival for this severe model for any therapeutic intervention and suggests that postsymptomatic treatment of SMA may lead to significant improvement of disease severity.

R&#xE9;sum&#xE9; amyotrophie spinale (SMA), une transmission autosomique r&#xE9;cessive neuromusculaire, est la principale cause g&#xE9;n&#xE9;tique de la mortalit&#xE9; infantile. SMA est caus&#xE9;e par la perte homozygote de la survie du motoneurone-1 (SMN1). Chez l&#x26;#39;homme, un g&#xE8;ne &#xE0; copie presque identique est pr&#xE9;sent, SMN2. SMN2 est conserv&#xE9;e chez tous les patients SMA et encode la m&#xEA;me prot&#xE9;ine que SMN1. Toutefois, SMN1 et SMN2 diff&#xE8;rent par un silence de C-&#xE0;-transition T &#xE0; l&#x26;#39;extr&#xE9;mit&#xE9; 5 &#x26;#39;de l&#x26;#39;exon 7, ce qui provoque l&#x26;#39;&#xE9;pissage alternatif de SMN2 relev&#xE9;s de notes et de faibles niveaux de pleine longueur SMN. SMA wird durch das homozygote Verlust von Survival Motor Neuron-1 (SMN1) verursacht. Beim Menschen ist eine fast identische Kopie-gen vorhanden, SMN2. SMN2 bleibt bei allen Patienten mit SMA und das gleiche Protein als SMN1 codiert. Jedoch unterscheiden sich SMN1 und SMN2 durch den stillen C-to-Tape-&Uuml;bergang am 5&#39;-Ende des Exon 7, alternatives Splei&szlig;en des SMN2-Transkripte und niedrigem in SMN verursacht. SMA ist monogene und daher gut geeignet f&uuml;r gen-Ersatz-Strategien. Vor kurzem wurden selbst-erg&auml;nzende Adeno-assoziierte Virus (ScAAV) Vektoren verwendet das SMN-cDNA an einem Tiermodell der Krankheit, die SMN&Delta;7-Maus zu liefern. In dieser Studie untersuchen wir eine schwere Modell von SMA, Smn(-/-);SMN2(+/+), um festzustellen, ob Gen-Ersatz in einem Modell machbar ist in welcher Krankheit Entwicklung in der Geb&auml;rmutter beginnt. Spinal Muscular Atrophy (SMA), an autosomal recessive neuromuscular disorder, is the leading genetic cause of infant mortality. SMA is caused by the homozygous loss of Survival Motor Neuron-1 (SMN1). SMA, however, is not due to complete absence of SMN, rather a low level of functional full-length SMN is produced by a nearly identical copy gene called SMN2. Despite SMN's ubiquitous expression, motor neurons are preferentially affected by low SMN levels. Recently gene replacement strategies have shown tremendous promise in animal models of SMA. In this study, we used self-complementary Adeno Associated Virus (scAAV) expressing full-length SMN cDNA to compare two different routes of viral delivery in a severe SMA mouse model. This was accomplished by injecting scAAV9-SMN vector intravenously (IV) or intracerebroventricularly (ICV) into SMA mice. Both routes of delivery resulted in a significant increase in lifespan and weight compared to untreated mice with a subpopulation of mice surviving more than 200days. However, the ICV injected mice gained significantly more weight than their IV treated counterparts. Likewise, survival analysis showed that ICV treated mice displayed fewer early deaths than IV treated animals. Collectively, this report demonstrates that route of delivery is a crucial component of gene therapy treatment for SMA.

Atrofia Muscular Espinal (AME), un trastorno autos&#xF3;mico recesivo neuromuscular, es la principal causa gen&#xE9;tica de la mortalidad infantil. Spinal muscular atrophy (SMA) is a leading genetic cause of infantile death. Loss of a gene called Survival Motor Neuron 1 (SMN1) and, as a result, reduced levels of the Survival Motor Neuron (SMN) protein leads to SMA development. SMA is characterized by the loss of functional motor neurons in the spinal cord. However, accumulating evidence suggests the contribution of other organs to the composite SMA phenotype and disease progression. A growing number of congenital heart defects have been identified in severe SMA patients. Consistent with the clinical cases, we have recently identified developmental and functional heart defects in two SMA mouse models, occurring at embryonic stage in a severe SMA model and shortly after birth in a less severe model (SMNâˆ†7). Our goal was to examine the late stage cardiac abnormalities in untreated SMNâˆ†7 mice and to determine whether gene replacement therapy restores cardiac structure/function in rescued SMNâˆ†7 model. To reveal the extent of the cardiac structural/functional repair in the rescued mice, we analyzed the heart of untreated and treated SMNâˆ†7 model using self-complementary Adeno-associated virus (serotype 9) expressing the full-length SMN cDNA. We examined the characteristics of the heart failure such as remodeling, fibrosis, oxidative stress, and vascular integrity in both groups. Our results clearly indicate that fibrosis, oxidative stress activation, vascular remodeling, and a significant decrease in the number of capillaries exist in the SMA heart. The cardiac structural defects were improved drastically in the rescued animals, however, the level of impairment was still significant compared to the age-matched wildtype littermates. Furthermore, functional analysis by in vivo cardiac magnetic resonance imaging (MRI) revealed that the heart of the treated SMA mice still exhibits functional defects. In conclusion, cardiac abnormalities are only partially rescued in post-birth treated SMA animals and these abnormalities may contribute to the premature death of vector-treated SMA animals with seemingly rescued motor function but an average life span of less than 70days as reported in several studies.

SMA est caract&#xE9;ris&#xE9; par la perte de neurones moteurs fonctionnels de la moelle &#xE9;pini&#xE8;re. Cependant, l&#x26;#39;accumulation de preuves sugg&#xE8;re la contribution d&#x26;#39;autres organes &#xE0; l&#x26;#39;ph&#xE9;notype SMA composite et progression de la maladie. Notre objectif &#xE9;tait d&#x26;#39;examiner les anomalies cardiaques &#xE0; un stade avanc&#xE9; non trait&#xE9;s dans SMN&#x394;7 souris et de d&#xE9;terminer si la th&#xE9;rapie de remplacement g&#xE9;nique restaure la structure cardiaque / fonction dans sauv&#xE9; SMN&#x394;7 mod&#xE8;le. Pour r&#xE9;v&#xE9;ler l&#x26;#39;&#xE9;tendue de la cardiaques structurelles / fonctionnelles r&#xE9;paration chez les souris sauv&#xE9;es, nous avons analys&#xE9; le c&#x153;ur de non trait&#xE9;e et trait&#xE9;e SMN&#x394;7 mod&#xE8;le &#xE0; l&#x26;#39;aide d&#x26;#39;auto-compl&#xE9;mentaires virus ad&#xE9;no-associ&#xE9; (s&#xE9;rotype 9) l&#x26;#39;expression de la SMN ADNc de pleine longueur. Nous avons examin&#xE9; les caract&#xE9;ristiques de l&#x26;#39;insuffisance cardiaque tels que le remodelage, la fibrose, le stress oxydatif, et de l&#x26;#39;int&#xE9;grit&#xE9; vasculaire dans les deux groupes. Nos r&#xE9;sultats indiquent clairement que la fibrose, l&#x26;#39;activation du stress oxydatif, le remodelage vasculaire, et une diminution significative du nombre de capillaires exister dans le c&#x153;ur SMA. Spinale Muskelatrophie (SMA) ist eine genetische Todesursache infantile. Verlust eines Gens Survival Motor Neuron 1 (SMN1) genannt und folglich verringert Niveaus des Survival Motor Neuron (SMN) Protein zu SMA-Entwicklung f&uuml;hrt. SMA zeichnet sich durch den Verlust der funktionellen Motoneuronen im R&uuml;ckenmark. Allerdings deuten die Anh&auml;ufung von Beweise den Beitrag der anderen Organe der zusammengesetzten SMA-Ph&auml;notyp und Krankheit Weiterentwicklung. Eine wachsende Zahl von angeborenen Herzfehlern ist in schweren SMA-Patienten wurden. In &Uuml;bereinstimmung mit den klinischen F&auml;llen, haben wir vor kurzem Entwicklungs- und funktionale Herzfehler in zwei SMA-Maus-Modellen, identifiziert in embryonalen Stadium in einem schweren SMA-Modell und kurz nach der Geburt in einem weniger schweren Modell (SMN&#x2206;7) auftreten. Um das Ausma&szlig; der kardialen Struktur-/funktionale Reparatur in die geretteten M&auml;use zu offenbaren, analysierten wir Herzen von unbehandelten und behandelten SMN&#x2206;7-Modell mit selbst-erg&auml;nzende Adeno-assoziierte Virus (Serotyp 9) zum Ausdruck zu bringen die in SMN cDNA. Wir untersuchten die Merkmale der Herzinsuffizienz wie Umbau, Fibrose, oxidativer Stress und vaskul&auml;re Integrit&auml;t in beiden Gruppen. Unsere Ergebnisse zeigen deutlich, dass Fibrose und oxidativen Stress, vaskul&auml;re Umbau und eine signifikante Abnahme der Zahl der Kapillaren im Herzen SMA vorhanden sind. Die strukturellen Herzfehlern wurden in die geretteten Tiere drastisch verbessert, jedoch war die H&ouml;he der Wertminderung immer noch signifikant im Vergleich zu den altersbezogenen Wildtyp-Wurfgeschwister. Funktionsanalyse durch in-vivo kardiale Magnetresonanz-Bildgebung (MRI) ergab au&szlig;erdem, dass das Herz von der behandelten SMA-M&auml;usen noch Funktionsst&ouml;rungen aufweist. La atrofia muscular espinal (AME) es la principal causa gen&#xE9;tica de la muerte infantil. La p&#xE9;rdida de un gen llamado neurona motora de supervivencia 1 (SMN1) y, como resultado, los niveles reducidos de la supervivencia de la neurona motora (SMN) de prote&#xED;nas conduce al desarrollo de SMA. SMA se caracteriza por la p&#xE9;rdida de neuronas motoras funcionales en la m&#xE9;dula espinal. Sin embargo, la evidencia acumulada sugiere que la contribuci&#xF3;n de otros &#xF3;rganos en el fenotipo SMA compuesto y progresi&#xF3;n de la enfermedad. Un n&#xFA;mero creciente de defectos cong&#xE9;nitos del coraz&#xF3;n se han identificado en pacientes con AME graves. De acuerdo con los casos cl&#xED;nicos, se han identificado recientemente los defectos del coraz&#xF3;n y de desarrollo funcional en dos modelos de rat&#xF3;n de SMA, que se producen en la etapa embrionaria en un modelo de SMA severa y poco despu&#xE9;s de nacer en un modelo menos grave (SMN&#x394;7). Nuestro objetivo fue examinar las anomal&#xED;as cardiacas en fase avanzada no tratados SMN&#x394;7 ratones y para determinar si la terapia g&#xE9;nica de reemplazo restaura la estructura cardiaca / funci&#xF3;n en rescatados SMN&#x394;7 modelo. Para revelar el alcance de la card&#xED;aca estructural / funcional de reparaci&#xF3;n en los ratones rescatados, se analiz&#xF3; el coraz&#xF3;n de la no tratada y tratada SMN&#x394;7 modelo de uso de auto-complementarios virus adeno-asociado (serotipo 9) expresa el SMN ADNc de longitud completa. Hemos examinado las caracter&#xED;sticas de la insuficiencia card&#xED;aca como la remodelaci&#xF3;n, la fibrosis, el estr&#xE9;s oxidativo, y la integridad vascular en ambos grupos. Nuestros resultados indican claramente que la fibrosis, la activaci&#xF3;n estr&#xE9;s oxidante, remodelado vascular, y una disminuci&#xF3;n significativa en el n&#xFA;mero de capilares existen en el coraz&#xF3;n de SMA. Los defectos card&#xED;acos estructurales se han mejorado dr&#xE1;sticamente en los animales rescatados, sin embargo, el nivel de deterioro era todav&#xED;a significativa en comparaci&#xF3;n con los compa&#xF1;eros de camada de tipo salvaje de la misma edad. Adem&#xE1;s, el an&#xE1;lisis funcional in vivo cardiaca im&#xE1;genes por resonancia magn&#xE9;tica (MRI) revel&#xF3; que el coraz&#xF3;n de los ratones tratados con AME a&#xFA;n presenta defectos funcionales.
