There was no standard treatment for hepatocellular carcinoma with portal vein tumour thrombosis (PVTT). This prospective, randomised, two-arm clinical trial aims to investigate the feasibility, safety and effectiveness of transarterial chemoembolisation (TACE) combined with the endovascular implantation of an iodine-125 seed strand for the treatment of hepatocellular carcinoma with portal vein tumour thrombosis versus conventional TACE.

Transarterial chemoembolisation (TACE) combined with endovascular implantation of an iodine-125 seed strand for the treatment of hepatocellular carcinoma with portal vein tumour thrombosis versus TACE alone: a two-arm, randomised clinical trial.

Portal vein thrombosis (PVT) is commonly classified into acute (symptom duration <60 days and absence of portal carvernoma and portal hypertension) and chronic types. However, the rationality of this classification has received little attention. In this study, 60 patients (40 men and 20 women) with PVT were examined using contrast-enhanced computed tomography (CT). The percentage of vein occlusion, including portal vein (PV) and superior mesenteric vein (SMV), was measured on CT image. Of 60 patients, 17 (28.3%) met the criterion of acute PVT. Symptoms occurred more frequently in patients with superior mesenteric vein thrombosis (SMVT) compared to those without SMVT (p<0.001). However, there was no significant difference in PV occlusion between patients with and without symptoms. The frequency of cavernous transformation was significantly higher in patients with complete PVT than those with partial PVT (p<0.001). Complications of portal hypertension were significantly associated with cirrhosis (p<0.001) rather than with the severity of PVT and presence of cavernoma. These results suggest that the severity of PVT is only associated with the formation of portal cavernoma but unrelated to the onset of symptoms and the development of portal hypertension. We classified PVT into complete and partial types, and each was subclassified into with and without portal cavernoma. In conclusion, neither symptom duration nor cavernous transformation can clearly distinguish between acute and chronic PVT. The new classification system can determine the pathological alterations of PVT, patency of portal vein and outcome of treatment in a longitudinal study. 

Rational classification of portal vein thrombosis and its clinical significance.

This study aims to establish an animal model of implanted main portal vein tumor thrombus (MPVTT) and to evaluate safety and efficacy of brachy therapy with Iodine-125 ((125)I) seeds strand to treat MPVTT of rabbit. VX2 tumor thrombus was implanted in main portal vein (MPV) of 32 New Zealand white rabbits. These rabbits were randomly divided into treatment group (Group T, T1-T16) and control group (Group C, C1-C16). (125)I seeds and blank seeds strand were implanted in MPV of rabbits in Group T and C, respectively. Changes of general condition, body weight and blood laboratory examination were monitored at every time point after procedure. 2 weeks later, 8 rabbits of each group were sacrificed for pathologic examination. The rest of rabbits were dissected postmortem, and therapeutic effects were evaluated on basis of multi-detector computed tomography and histopathology. Ki-67 labeling index (Ki-67 LI) and apoptosis index (AI) were compared between two groups. Overall survival period was recorded. At every time point after brachytherapy, more serious weight loss were detected in Group C. Results of liver function tests and blood cells counts showed no significant difference between two groups. Mean volume of tumor tissue within MPV were 565.40 ± 220.90 mm(3) in Group T and 2269.90 ± 437.00 mm(3) in Group C (P < 0.001). (Ki-67 LI) and AI were (4.14 ± 1.84)% and (6.51 ± 1.92)% in Group T, compared with (33.82 ± 6.07)% and (0.91 ± 0.26)% in Group C, respectively (P < 0.001). Media survival time of rabbits were 39.50 ± 2.37 days in Group T and 27.38 ± 1.22 days in Group C, respectively (P = 0.001). In conclusion, injecting and suspensory fixing VX2 tumor strip into MPV is a reliable method to establish MPVTT animal model. Brachytherapy with (125)I seeds strand was safe and effective to treat VX2 tumor strand inoculated in the MPV of rabbit. 

Brachytherapy with Iodine-125 seeds strand for treatment of main portal vein tumor thrombi: an experimental study in a rabbit model.

The purpose of this study was to analyze the long-term outcomes, including safety, efficacy, and prognostic features, of intraluminal brachytherapy with Iodine-125 (I) seed strand and stent placement for treatment of patients with malignant obstructive jaundice (MOJ).

Management of patients with bone metastasis is complex and should include different specialties. Goals of therapy should be identified for each individual patient prior to the start of treatment. Preoperative embolization has generally been considered a safe and effective means of reducing intraoperative blood loss with recent studies and advances in technique reported. Update on indications, contraindications, technique, and efficacy, as well as prognostic factors and complications of preoperative embolization of bone metastases will be reviewed. New trends such as transradial arterial access and usage of liquid embolic agents will be discussed. Large tumor size, increased preprocedural tumor vascularity, longer embolization-to-surgery interval, and radical surgical procedures are associated with greater intraoperative blood loss and prolonged operative time. An accurate, noninvasive method to evaluate tumor vascularity prior to angiography is needed to identify patients who are most likely to benefit from preoperative embolization. Particular attention will be paid to skeletal metastases and spinal metastases as each has its own set of complexity.
