The aim of this study was to determine whether bipolar patients who had been stabilized on combined antipsychotic and mood-stabilizer medications and were currently in remission benefited from continuation of the antipsychotic medication. Remitted bipolar patients were randomly assigned to either remain on adjunctive antipsychotic medication or to taper to placebo. Antipsychotic/placebo medication assignment was double-blind. Subjects were outpatients at a university-affiliated community mental health center. Fifteen subjects consented and proceeded with eligibility assessments. Five subjects were never randomized. One of these was excluded when the Structured Clinical Interview for DSM-IV interview revealed schizoaffective disorder. The remaining four subjects were not randomized for other reasons. Three randomized subjects never received study medications, or were withdrawn by the investigator within 1 week after beginning study medications. The seven remaining subjects received study medication for more than 1 week. Five subjects were randomized to taper to placebo and two to antipsychotic continuation. Of the five randomized to taper to placebo, three successfully tapered and completed the year of follow-up in continuous remission. One subject became manic 4 months after taper was completed, and one subject became psychotic, in the absence of a mood episode, during taper. Of the two subjects randomized to double-blind antipsychotic continuation, both completed the year of follow-up in continuous remission. When adjunctive antipsychotic medications are discontinued, bipolar patients' clinical symptoms can remain unchanged. Others are, however, at risk for manic relapse.

Le but de cette &eacute;tude &eacute;tait de d&eacute;terminer si les patients bipolaires, qui avaient &eacute;t&eacute; stabilis&eacute;s combin&eacute;e de m&eacute;dicaments antipsychotiques et stabilisateur de l&#39;humeur et sont trouvent actuellement en r&eacute;mission ont b&eacute;n&eacute;fici&eacute; de la continuation de l&#39;antipsychotique. Trois sujets randomis&eacute;s jamais re&ccedil;u des m&eacute;dicaments de l&#39;&eacute;tude ou ont &eacute;t&eacute; retir&eacute;es par l&#39;enqu&ecirc;teur dans 1 semaine apr&egrave;s le d&eacute;but de m&eacute;dicaments de l&#39;&eacute;tude. Les sept sujets restants a re&ccedil;u le m&eacute;dicament &agrave; l&#39;&eacute;tude pendant plus de 1 semaine. Cinq sujets ont &eacute;t&eacute; randomis&eacute;s &agrave; c&ocirc;ne &agrave; un placebo et deux &agrave; continuation antipsychotique. Des cinq randomis&eacute;s &agrave; c&ocirc;ne au placebo, trois coniques avec succ&egrave;s et a termin&eacute; l&#39;ann&eacute;e de suivi en r&eacute;mission continue. Un sujet est devenu maniaco 4 mois apr&egrave;s la fin de c&ocirc;ne et un seul sujet est devenu psychotique, en l&#39;absence d&#39;un &eacute;pisode d&#39;humeur, pendant le d&eacute;filement. Des deux sujets randomis&eacute;s &agrave; double insu continuation antipsychotique, tous deux termin&eacute; l&#39;ann&eacute;e de suivi en r&eacute;mission continue. Lorsque les m&eacute;dicaments antipsychotiques d&#39;appoint sont abandonn&eacute;es, les sympt&ocirc;mes cliniques de patients bipolaires peuvent restent inchang&eacute;s. D&#39;autres sont &agrave; risque de rechute maniaque.

Lo scopo di questo studio era di determinare se i pazienti bipolari che erano stati stabilizzati su combinato di farmaci antipsicotici e stabilizzatore dell&#39;umore ed erano attualmente in remissione beneficiato di continuazione del farmaco antipsicotico. Rimessi pazienti bipolari sono stati assegnati in modo casuale o restare su farmaci antipsicotici aggiuntivo o a cono al placebo. Assegnazione di farmaci antipsicotici/placebo era doppio-cieco. Soggetti erano pazienti ambulatoriali presso un centro di salute mentale di comunit&agrave; Universit&agrave;-affiliated. Quindici soggetti acconsent&igrave; e procedettero con le valutazioni di ammissibilit&agrave;. Cinque soggetti sono stati randomizzati mai. Uno di questi &egrave; stato escluso quando intervista clinica strutturata per intervista DSM-IV ha rivelato disordine schizoaffective. Quando sono fuori produzione aggiuntive farmaci antipsicotici, i sintomi clinici dei pazienti bipolari possono rimangono invariati. Gli altri sono, tuttavia, a rischio di recidiva maniaca.

El objetivo de este estudio era determinar si los pacientes bipolares que hab&iacute;an sido estabilizados en combinaci&oacute;n medicamentos antipsic&oacute;ticos y estabilizador del humor y estaban actualmente en remisi&oacute;n que se beneficiaron de la continuaci&oacute;n de la medicaci&oacute;n antipsic&oacute;tica. Remite pacientes bipolares fueron asignados aleatoriamente o bien permanecer en medicaci&oacute;n antipsic&oacute;tica adyuvante o para afilar con placebo. Asignaci&oacute;n de medicamentos antipsic&oacute;ticos/placebo fue doble ciego. Los sujetos fueron pacientes ambulatorios en un centro de salud mental de la comunidad de Universidad-afiliado. Quince temas consintieron y procedieron con evaluaciones de elegibilidad. Cinco temas nunca fueron asignados al azar. Uno de ellos fue excluido cuando la entrevista cl&iacute;nica estructurada para entrevista de DSM-IV revel&oacute; trastorno esquizoafectivo. Los cuatro temas restantes no fueron asignados al azar por otras razones. Tres sujetos al azar nunca recibieron medicamentos de estudio, o se retiraron por el investigador dentro de 1 semana despu&eacute;s de comenzar el estudio de medicamentos. Los siete sujetos restantes recibieron medicamento del estudio durante m&aacute;s de una semana. Cinco sujetos fueron asignados al azar para afilar con placebo y dos a continuaci&oacute;n antipsic&oacute;tico. De los cinco al azar para afilar con placebo, con &eacute;xito tres c&oacute;nicos y termin&oacute; el a&ntilde;o en remisi&oacute;n continua de seguimiento. Several recent studies of individually administered cognitive behavior therapy (CBT) for early psychosis have reported only modest treatment benefits. The purpose of the current study was to review the literature to determine how outcomes of group CBT differ from outcomes of individually administered CBT among early cases. Our findings suggest that group CBT for early psychosis may be a more effective modality for this group of patients. We speculate that patients' uncertainty about illness in general may impair the effectiveness of individually administered CBT for early cases and that group CBT may be more effective for these young patients by better addressing those factors with the aid of peer-to-peer interactions, identification, and modeling.

The American Psychiatric Association Task Force on DSM-5 has recently proposed consideration of Attenuated Psychotic Symptoms Syndrome as a new diagnosis, based on nearly 15 years of prospective research in centers across the globe. The condition is also known as "psychosis risk syndrome," "at-risk mental state," "ultra-high risk," and "putative prodrome." We review evidence favoring its inclusion as a new diagnosis in DSM-5 and report new preliminary findings on DSM-IV diagnoses in current clinical use for these patients and on results of diagnostic interviews in unselected volunteers. The main evidence supporting inclusion is: (1) the patients are currently ill, (2) the patients are at high risk for getting worse, (3) no DSM-IV diagnosis accurately captures their current illness or future risk, (4) the diagnosis has been made with reliability and validity in the research setting, and (5) placement in DSM-5 would help promote the needed treatment and prevention research to enable articulation of a standard of care to benefit these patients and their families. Potential harms can be minimized by patient, family, and provider education. It will be important to demonstrate through well-designed field trials whether the diagnostic criteria can be used with reliability in everyday clinical practice.

La Asociaci&oacute;n psiqui&aacute;trica American Task Force en DSM-5 tiene consideraci&oacute;n recientemente propuesta de s&iacute;ndrome de s&iacute;ntomas psic&oacute;ticos atenuados como un nuevo diagn&oacute;stico, basado en casi 15 a&ntilde;os de investigaci&oacute;n prospectiva en centros en todo el mundo. La condici&oacute;n es tambi&eacute;n conocido como &quot;s&iacute;ndrome de riesgo de psicosis,&quot; &quot;estado mental en riesgo&quot; &quot;ultra alto riesgo&quot; y &quot;supuesta pr&oacute;dromo.&quot; Repasamos evidencia favoreciendo su inclusi&oacute;n como un nuevo diagn&oacute;stico en el DSM-5 y divulgamos nuevos hallazgos preliminares en diagn&oacute;sticos del DSM-IV de uso cl&iacute;nico actual para estos pacientes y en los resultados de las entrevistas diagn&oacute;sticos en voluntarios no seleccionados. La principal evidencia que apoya inclusi&oacute;n es: (1) los pacientes son actualmente enfermos, (2) los pacientes corren un alto riesgo de empeoramiento, (3) diagn&oacute;stico DSM-IV no captura con exactitud su enfermedad actual o futuro riesgo, se ha realizado con fiabilidad y validez en el marco de la investigaci&oacute;n (4) el diagn&oacute;stico y colocaci&oacute;n (5) en el DSM-5 ayudar&iacute;a a promover la investigaci&oacute;n, tratamiento y prevenci&oacute;n necesaria para permitir la articulaci&oacute;n de un est&aacute;ndar de atenci&oacute;n en beneficio de estos pacientes y sus familias. Educaci&oacute;n del paciente, la familia y proveedor pueden minimizarse los da&ntilde;os potenciales. Ser&aacute; importante demostrar a trav&eacute;s de ensayos bien dise&ntilde;ados del campo si los criterios de diagn&oacute;stico pueden utilizarse con fiabilidad en la pr&aacute;ctica cl&iacute;nica diaria.
