Several laser and non-laser refractive surgical procedures have been used to modify the shape of the cornea and correct myopia, hyperopia, astigmatism, and presbyopia. Introduction of the excimer laser to reshape the cornea has resulted in remarkable developments in the correction of these refractive errors. Combined with other advanced ophthalmological instruments, laser refractive eye surgery has resulted in a substantial rise in the safety, efficacy, and predictability of surgical outcomes. Despite these advances, certain limitations and complications persist. In this review, we describe the history, preoperative assessment, surgical techniques, outcomes, and complications of laser refractive surgery.

The growth of new blood vessels (angiogenesis) within tumors is essential for tumor growth, maintenance, and metastasis. Angiogenesis research has identified a host of pro- and anti-angiogenic factors that regulate an "angiogenic switch," which when turned on, allows tumors to assume a more aggressive form. Angiogensis inhibitors that target this switch are in clinical trials for a wide array of tumor types. Although angiogenesis inhibitors are already widely used to treat ocular disease, only limited case reports are currently available for the use of angiogenesis inhibitors to treat ocular tumors. Evidence for angiogenesis in the growth and spread of uveal melanoma, retinoblastoma, and von Hippel Lindau (VHL) disease exists. The very limited trials of angiogenesis inhibitors in the treatment of uveal melanoma and VHL are promising, although more extensive controlled trials will be needed to confirm their efficacy.

Anti-angiogenic therapy: Prospects for treatment of ocular tumors.

NaBC1 (the SLC4A11 gene) belongs to the SLC4 family of sodium-coupled bicarbonate (carbonate) transporter proteins and functions as an electrogenic sodium borate cotransporter. Mutations in SLC4A11 cause either corneal abnormalities (corneal hereditary dystrophy type 2) or a combined auditory and visual impairment (Harboyan syndrome). The role of NaBC1 in sensory systems is poorly understood, given the difficulty of studying patients with NaBC1 mutations. We report our findings in Slc4a11(-/-) mice generated to investigate the role of NaBC1 in sensorineural systems. In wild-type mice, specific NaBC1 immunoreactivity was detected in fibrocytes of the spiral ligament, from the basal to the apical portion of the cochlea. NaBC1 immunoreactivity was present in the vestibular labyrinth, in stromal cells underneath the non-immunoreactive sensory epithelia of the macula utricle, sacule, and crista ampullaris, and the membranous vestibular labyrinth was collapsed. Both auditory brain response and vestibular evoked potential waveforms were significantly abnormal in Slc4a11(-/-) mice. In the cornea, NaBC1 was highly expressed in the endothelial cell layer with less staining in epithelial cells. However, unlike humans, the corneal phenotype was mild with a normal slit lamp evaluation. Corneal endothelial cells were morphologically normal; however, both the absolute height of the corneal basal epithelial cells and the relative basal epithelial cell/total corneal thickness were significantly increased in Slc4a11(-/-) mice. Our results demonstrate for the first time the importance of NaBC1 in the audio-vestibular system and provide support for the hypothesis that SLC4A11 should be considered a potential candidate gene in patients with isolated sensorineural vestibular hearing abnormalities.

NaBC1 (gene SLC4A11) appartiene alla famiglia delle SLC4 delle proteine di trasporto accoppiato sodio bicarbonato (carbonato) e funzioni come un che di elettrogeniche borato di sodio. Mutazioni in SLC4A11 causano o anomalie della cornea (distrofia corneale ereditaria tipo 2) o una minorazione visiva e uditiva combinata (sindrome di Harboyan). Il ruolo di NaBC1 in sistemi sensoriali &egrave; scarsamente compreso, data la difficolt&agrave; di studiare i pazienti con mutazioni di NaBC1. Riportiamo i nostri risultati nei topi Slc4a11(-/-) generati per indagare il ruolo di NaBC1 in sistemi neurosensoriale. Nei topi wild-type, specifici NaBC1 immunoreattivit&agrave; &egrave; stata rilevata nei fibroblasti del legamento spirale, dal basale alla porzione apicale della coclea. NaBC1 immunoreattivit&agrave; era presente nel labirinto vestibolare, nelle cellule stromali sotto l&#39;epitelio sensoriale non immunoreattive l&#39;utricolo macula, sacule e crista ampullaris, e il labirinto membranoso vestibolare era crollato. Entrambi uditiva del cervello risposta e vestibolari evocate potenziali forme d&#39;onda sono stati significativamente anormali nei topi Slc4a11(-/-). Nella cornea, NaBC1 era altamente espresso nello strato delle cellule endoteliali con meno di colorazione delle cellule epiteliali. Tuttavia, a differenza degli esseri umani, il fenotipo cornea &egrave; stato mite, con una valutazione di lampada fessura normale. O(2) permeability was higher in MeOH-immersed films with Dk values of 10(-10) (mL O(2) x cm) x (cm(-1) x s(-1) x mmHg(-1)), while those of water-annealed films reached only one fifth of this value. All films showed a decrease in Young's modulus and increased plastic deformation by two orders of magnitude when submerged in saline solution. FT-IR showed that beta-sheet content in water-annealed films increased with increasing water vapor pressure, while MeOH-immersed films showed no change.

BACKGROUND: The aim of this work is to characterize a transparent tissue layer partially covering the anterior surface of the type I Boston permanent keratoprosthesis front plate in four patients. METHODS: The tissue over the front plate was easily scrolled back as a single transparent layer using a sponge. In two cases, histopathologic analysis was undertaken and immunofluorescent staining with a cytokeratin 3-specific antibody was performed. The relationship of the tissue to the keratoprosthesis device was further characterized using spectral domain high-definition optical coherence tomography (HD-OCT). RESULTS: Histopathologic analysis revealed the tissue to be non-keratinized squamous epithelium. No goblet cells were seen, suggesting the cells were of corneal, and not conjunctival, epithelial origin. Immunofluorescent staining of all cells was positive for cytokeratin 3, a protein strongly associated with corneal epithelium. The tissue was easily discerned by HD-OCT and was of substantial thickness near the external junction between the keratoprosthesis device and the carrier corneal tissue. In three cases, visual acuity was unaffected by the presence or absence of this tissue. In one case, a prominent tissue margin temporarily obscured the visual axis and reduced visual acuity; this resolved with mechanical central debridement and has not recurred. CONCLUSIONS: The transparent tissue layer covering the anterior surface of the type I Boston keratoprosthesis front plate was found to represent non-keratinized squamous epithelium, most likely of corneal epithelial origin. This potentially represents a further step in bio-integration of the keratoprosthesis device. In particular, epithelial coverage of the critical junction between the device and the carrier corneal tissue might serve an important barrier function and further reduce the incidence of infection and extrusion of the type I Boston permanent keratoprosthesis.

Hintergrund: Das Ziel dieser Arbeit ist, eine transparente Gewebe-Schicht, die teilweise &uuml;ber die Oberfl&auml;che des Typs ich Boston permanente Keratoprothese n. Front bei 4 Patienten Platte zu charakterisieren. Methoden: Das Gewebe &uuml;ber die Frontplatte wurde wieder als einzelne transparente Ebene mit einem Schwamm leicht gescrollt. In beiden F&auml;llen histopathologische Analyse wurde durchgef&uuml;hrt und Immunfluoreszenz-F&auml;rbung mit einem Keratin 3-spezifische Antik&ouml;rper durchgef&uuml;hrt wurde. ANTECEDENTES: El objetivo de este trabajo es caracterizar una capa de tejido transparente que cubre parcialmente la superficie anterior del tipo de placa que Boston permanente frente a queratopr&#xF3;tesis en cuatro pacientes. M&#xC9;TODOS: El tejido en la placa frontal se desplaza f&#xE1;cilmente de nuevo en una sola capa transparente, utilizando una esponja. En dos casos, el an&#xE1;lisis histopatol&#xF3;gico se llev&#xF3; a cabo y la tinci&#xF3;n de inmunofluorescencia con un 3-citoqueratina de anticuerpos espec&#xED;ficos se realiz&#xF3;. La relaci&#xF3;n del tejido al dispositivo queratopr&#xF3;tesis se caracteriz&#xF3; adem&#xE1;s usando dominio espectral de alta definici&#xF3;n tomograf&#xED;a de coherencia &#xF3;ptica (HD-octubre). RESULTADOS: El an&#xE1;lisis histopatol&#xF3;gico revel&#xF3; el tejido que no queratinizado epitelio escamoso. No se observaron c&#xE9;lulas caliciformes, lo que sugiere que las c&#xE9;lulas eran de la c&#xF3;rnea, la conjuntiva y no de origen epitelial. Inmunofluorescencia tinci&#xF3;n de las c&#xE9;lulas fue positiva para citoqueratina 3, una prote&#xED;na fuertemente asociado con el epitelio de la c&#xF3;rnea. El tejido fue discernido f&#xE1;cilmente por HD-OCT, y era un espesor considerable cerca de la uni&#xF3;n entre el dispositivo externo queratopr&#xF3;tesis y el tejido portador de la c&#xF3;rnea. En tres casos, la agudeza visual no se vio afectada por la presencia o ausencia de este tejido. En un caso, un margen de tejido prominente temporalmente oculta el eje visual y la disminuci&#xF3;n de la agudeza visual, lo resolvi&#xF3; con desbridamiento mec&#xE1;nico central y no ha vuelto a aparecer. CONCLUSIONES: La capa de tejido transparente que cubre la superficie anterior de la placa de tipo I frente a Boston queratopr&#xF3;tesis se encontr&#xF3; que representan no queratinizado epitelio escamoso, lo m&#xE1;s probable de origen epitelial de la c&#xF3;rnea. Potencialmente, esto representa un paso m&#xE1;s en la bio-integraci&#xF3;n del dispositivo queratopr&#xF3;tesis. En particular, la cobertura, epiteliales de la uni&#xF3;n cr&#xED;tica entre el dispositivo y el tejido corneal portador puede servir una funci&#xF3;n de barrera importante y reducir a&#xFA;n m&#xE1;s la incidencia de la infecci&#xF3;n y la extrusi&#xF3;n del tipo I de Boston permanente queratopr&#xF3;tesis.

PURPOSE: To evaluate the efficacy of a prophylactic regimen of daily topical 0.5% moxifloxacin and 5% povidone-iodine (PI) in patients with Boston type I keratoprosthesis (KPro) and to assess the applicability of a novel molecular diagnostic technique to analyze the ocular surface microbiota in these patients. METHODS: Ten patients had their inferior conjunctival fornix sampled for standard culture methods before the addition of topical 5% PI to the prophylactic regimen and were considered the control group (group 1). The inferior conjunctival fornix and the KPro-donor cornea interface of 10 patients treated with the mentioned prophylactic regimen were sampled and analyzed by standard culture methods and using a polymerase chain reaction/electrospray ionization mass spectrometry assay (group 2). RESULTS: Samples from the inferior conjunctival fornix were positive for coagulase-negative staphylococcus in 3 patients and for Aerobasidium pullulans in 1 patient in group 1. The inferior conjunctival fornix and the KPro-donor cornea interface scrapings were positive for coagulase-negative staphylococcus in 2 patients and 1 patient, respectively, in group 2. No bacteria and fungi growth were detected in any patient from group 2 with the molecular diagnostic approach. None of the patients with culture-positive results developed keratitis or endophthalmitis during the study. CONCLUSIONS: Topical 0.5% moxifloxacin associated with topical 5% PI is an effective prophylactic regimen in patients with Boston type I KPro. The molecular diagnostic approach using serial polymerase chain reaction and mass spectrometry was comparable with standard microbiologic techniques as a surveillance tool in these patients.

Zweck: Zur Bewertung der Wirksamkeit der eine prophylaktische Gabe von t&auml;glich aktuell 0,5 % Moxifloxacin und 5 % Povidon-Iod (PI) bei Patienten mit Boston Typ I Keratoprothese n. (KPro) und die Anwendbarkeit der eine neue Molekulare Diagnose Technik zur Analyse der okul&auml;ren Oberfl&auml;che Microbiota bei diesen Patienten zu beurteilen. Methoden: 10 Patienten hatten ihre minderwertigen konjunktivale Fornix Stichprobe f&uuml;r die Standardkultur Methoden vor der Zugabe von aktuell 5 % PI um die prophylaktische Gabe und wurden als die Kontrollgruppe (Gruppe 1). Die minderwertigen konjunktivale Fornix und der KPro-Spender-Hornhaut-Schnittstelle von 10 Patienten, die mit den genannten prophylaktischen Therapie behandelt wurden gesampelt und von Standardkultur Methoden analysiert und mit einer Polymerase Kettenreaktion/Elektrospray-Ionisation Massenspektrometrie Assay (Gruppe 2). Ergebnisse: Proben aus den minderwertigen konjunktivale Fornix waren positiv f&uuml;r coagulasepositive-Negative Staphylokokken bei 3 Patienten und Aerobasidium Pullulans bei 1 Patienten in Gruppe 1. Die minderwertigen konjunktivale Fornix und der KPro-Spender-Hornhaut-Schnittstelle Knochenputz waren positiv f&uuml;r coagulasepositive-Negative Staphylokokken bei 2 Patienten und 1 Patient, in Gruppe 2. In jeder Patient aus Gruppe 2 mit der molekularen Diagnostik Ansatz wurden keine Bakterien und Pilze-Wachstum erkannt. Keiner der Patienten mit Kultur-Positive Ergebnisse entwickelten Keratitis oder Endophthalmitis w&auml;hrend der Studie. Fazit: Aktuell 0,5 % Moxifloxacin zugeordnete aktuell 5 % PI eine wirksame prophylaktische Therapie in ist Patienten mit Boston Typ I Indexdaten. Der Molekulare Diagnose Ansatz mit seriellen Polymerasekettenreaktion und Massenspektrometrie war als ein Tool zur Beobachtung bei diesen Patienten vergleichbar mit mikrobiologischer Standardtechniken.

Metodi: Dieci pazienti hanno avuti loro fornice congiuntivale inferiore campionate per cultura standard metodi prima dell&#39;aggiunta di attualit&agrave; 5% PI per la profilassi di regime e sono stati considerati il gruppo di controllo (gruppo 1). Il fornice congiuntivale inferiore e l&#39;interfaccia cornea KPro-donatore dei 10 pazienti trattati con il regime profilattico accennato erano campionati e analizzati da metodi di coltura standard e utilizzando un dosaggio di spettrometria totale di ionizzazione della polimerasi reazione a catena/electrospray (gruppo 2). RISULTATI: Campioni dal fornice congiuntivale inferiore erano positivi per staphylococcus coagulasi-negativi in 3 pazienti e Aerobasidium pullulans in 1 paziente nel gruppo 1. Il fornice congiuntivale inferiore e la raschiatura di interfaccia cornea KPro-donatore era positive per staphylococcus coagulasi-negativi in 2 pazienti ed 1 paziente, rispettivamente, nel gruppo 2. Nessuna crescita di batteri e funghi sono stati rilevati in qualsiasi paziente dal gruppo 2 con l&#39;approccio diagnostico molecolare. Nessuno dei pazienti con risultati positivi per cultura sviluppato cheratite o endoftalmiti durante lo studio. CONCLUSIONI: Attualit&agrave; moxifloxacina 0.5% associata topico 5% PI &egrave; un efficacia regime profilattico in pazienti con Boston di tipo KPro. L&#39;approccio diagnostico molecolare mediante reazione a catena della polimerasi seriale e spettrometria di massa era paragonabile con tecniche standard microbiologici come strumento di sorveglianza in questi pazienti.

Microbiota valutazione di pazienti con un tipo di Boston sono Keratoprosthesis trattato con topici moxifloxacina 0.5% e 5% Povidone-iodio.
