Xenotransplantation offers a transformative solution to the global organ shortage crisis. However, the survival of xenografts remains limited despite various proposed strategies. In this study, we present an endothelial cell protection strategy that extends graft survival through the  construction of biomimetic glycan-enriched nanofibers. These biomimetic glycan-enriched molecules specifically target integrin αβ and form a polysaccharide-structured nanofiber network on the vascular endothelial surface. This network protects endothelial cells without compromising their normal physiological functions. The constructed biomimetic glycan-enriched layer significantly increased the xenograft survival by 1.64-fold compared to the untreated groups. This work introduces a novel strategy to enhance the survival of heart xenografts.

In Situ Biomimetic Glycocalyx Layer Protects Endothelial Damage in Xenotransplantation.

Organ failure poses a substantial global health challenge, and xenotransplantation emerges as one of the most promising avenues to mitigate the critical shortage of donor organs. In recent years, numerous research institutions have undertaken clinical and preclinical xenotransplantation in humans, instilling hope for notable progress. Nevertheless, formidable obstacles persist before success can be fully achieved. Chinese researchers have been at the forefront of xenotransplantation studies, actively contributing to several pivotal areas: the identification of critical genes essential for xenotransplantation and the creation of genetically modified pigs; preclinical studies on pig-to-nonhuman primate organ and tissue xenotransplantation, as well as the utilization of genetically engineered pig-derived biomaterials; contributions to both preclinical and clinical xenotransplantation research; and the formulation and refinement of xenotransplantation policies and ethical guidelines in China. In conclusion, this review seeks to not only acknowledge the contributions of Chinese researchers but also to encourage further collaboration between Chinese scholars and their international counterparts in advancing the field of xenotransplantation.

Xenotransplantation in China: Past, Present, and Future.

Organ transplantation remains the foremost effective intervention for end-stage organ failure. Nevertheless, the scarcity of donors has resulted in prolonged waiting times for countless patients globally. The advent of xenografts presents a promising solution to the organ shortage crisis. Although the utilization of xenografts has a long history, it is only in recent years that breakthroughs in genetically modified pigs have rendered successful xenotransplantation a feasible option. In the past 4 years, numerous subclinical and clinical trials have involved xenotransplantation from genetically modified pigs to humans. However, the outcomes have been disappointing, necessitating a reassessment of basic and preclinical research to address the emerging challenges. Furthermore, immunosuppressive therapies remain essential in xenotransplantation. The range of immunosuppressive agents, encompassing traditional immunosuppressants and monoclonal antibodies such as anti-CD154/CD40 monoclonal antibodies, exhibits considerable diversity. However, the most effective drug combination for achieving optimal efficacy remains elusive. This review will offer a succinct overview of the results from recent clinical and subclinical xenotransplantation trials. Moreover, it will highlight recent advancements in immunosuppressive strategies and discuss potential future research directions in this field.

Advances in Subclinical and Clinical Trials and Immunosuppressive Therapies in Xenotransplantation.

Despite being a serious health condition that significantly increases cardiovascular and metabolic disease risks, sleep apnea syndrome (SAS) remains largely underdiagnosed. While polysomnography (PSG) remains the gold standard for diagnosis, its clinical application is limited by high costs, complex setup requirements, and sleep quality interference. Although wearable devices using photoplethysmography (PPG) have shown promise in SAS detection, their continuous operation demands substantial power consumption, hindering long-term monitoring capabilities. Here, a dual-modal wearable system is presented integrating a piezoelectric nanogenerator (PENG) and PPG sensor with a biomimetic fingertip structure for SAS detection. A two-stage detection strategy is adopted where the self-powered PENG performs continuous preliminary screening, activating the PPG sensor only when suspicious events are detected. Combined with a Vision Transformer-based deep learning model, the high-accuracy configuration achieves 99.59% accuracy, while the low-power two-stage approach maintained 94.95% accuracy. This dual-modal wearable pulse detection system provides a practical solution for long-term SAS monitoring, overcoming the limitations of traditional PSG while maintaining high detection accuracy. The system's versatility in both home and clinical settings offers the potential for improving early detection rates and treatment outcomes for SAS patients.

A Dual-Modal Wearable Pulse Detection System Integrated with Deep Learning for High-Accuracy and Low-Power Sleep Apnea Monitoring.

Using a cross-sectional survey design from 11 public tertiary hospitals (a specialist hospital, four Chinese medicine hospitals, and six general hospitals) in the urban areas of Heilongjiang, we determined the nature of workplace violence that medical staff have encountered in Chinese hospitals and identified factors associated with those experiences of violence. A total of 1129 health workers participated. The specialist hospital had the highest prevalence of physical violence (35.4%), while the general hospitals had the highest prevalence of non-physical violence (76%). Inexperienced medical staff (p < 0.001) were more likely to suffer non-physical violence than physical violence in Chinese medicine hospitals compared to experienced staff. Medical units (p = 0.001) had a high risk of non-physical violence, while surgical units (p = 0.005) had a high risk of physical violence. In general hospitals, staff with higher levels of anxiety about workplace violence were more vulnerable to both physical violence (1.67, 95% CI 1.36-2.10) and non-physical violence (1.309, 95% CI 1.136-1.508) compared to those with lower levels of anxiety, while rotating shift workers had a higher odds of physical violence (2.2, 95% CI 1.21-4.17) and non-physical violence (1.65, 95% CI 1.13-2.41) compared to fixed day shift workers. Thus, prevention should focus not only on high-risk sections of hospitals, but also on the nature of the hospital itself.

Cardiac allograft vasculopathy (CAV) is a major barrier to long-term survival after heart transplantation, characterized by progressive vascular remodeling and luminal narrowing. Fibrosis is one of the key pathological features of CAV progression, but its underlying mechanisms remain unclear. This study aims to investigate the mechanisms of CAV-associated vascular fibrosis and explore potential therapeutic targets.

Blocking AREG-EGFR signaling attenuates pan-arterial fibrosis in chronic cardiac allograft rejection.

Polyamines, particularly spermidine, are well-documented for their cardiovascular protective, antitumor, and longevity-promoting properties. However, their role in heart transplantation, and specifically the contribution of T-cell polyamine metabolism to transplant acceptance, remains undefined. In this study, we integrated preclinical and clinical studies to address this gap. We found that polyamine metabolism was significantly upregulated in T cells during acute rejection (AR) in both murine allografts and human AR samples. Critically, conditional inhibition of polyamine metabolism in T cells completely prevents AR and promotes long-term graft survival. Mechanistically, we found that polyamine metabolism is crucial for the differentiation of cytotoxic CD8 T cells. Further, this regulatory effect was mediated by polyamine-dependent hypusination of eukaryotic initiation factor 5A (eIF5A), with inhibition of T cell eIF5A hypusination recapitulating the anti-AR effects of polyamine blockade. Our study identifies T cell polyamine metabolism as a critical regulator of cytotoxic CD8 T cell differentiation and AR in heart transplantation. Targeting this pathway holds promise as a novel therapeutic strategy for treating AR.
