Human and animal studies suggest that acupuncture produces many beneficial effects through the central nervous system. However, the neural substrates of acupuncture actions are not completely clear to date. fMRI studies at Hegu (LI4) and Zusanli (ST36) indicated that the limbic system may play an important role for acupuncture effects. To test if this finding applies to other major classical acupoints, fMRI was performed on 10 healthy adults during manual acupuncture at Taichong (LV3), Xingjian (LV2), Neiting (ST44), and a sham point on the dorsum of the left foot. Although certain differences could be observed between real and sham points, the hemodynamic response (BOLD signal changes) and psychophysical response (sensory experience) to acupuncture were generally similar for all four points. Acupuncture produced extensive deactivation of the limbic-paralimbic-neocortical system. Clusters of deactivated regions were seen in the medial prefrontal cortex (frontal pole, pregenual cingulate), the temporal lobe (amygdala, hippocampus, and parahippocampus) and the posterior medial cortex (precuneus, posterior cingulate). The sensorimotor cortices (somatosensory cortices, supplementary motor cortex), thalamus and occasional paralimbic structures such as the insula and anterior middle cingulate cortex showed activation. Our results provide additional evidence in support of previous reports that acupuncture modulates the limbic-paralimbic-neocortical network. We hypothesize that acupuncture may mediate its antipain, antianxiety, and other therapeutic effects via this intrinsic neural circuit that plays a central role in the affective and cognitive dimensions of pain as well as in the regulation and integration of emotion, memory processing, autonomic, endocrine, immunological, and sensorimotor functions.

The salient characteristics of the central effects of acupuncture needling: limbic-paralimbic-neocortical network modulation.

Des &eacute;tudes humaines et animales sugg&egrave;rent que l&#39;acupuncture produit de nombreux effets b&eacute;n&eacute;fiques &agrave; travers le syst&egrave;me nerveux central. Cependant, les substrats neurones des actions de l&#39;acupuncture ne sont pas tout &agrave; fait clairs &agrave; ce jour. &eacute;tudes IRMf Hegu (LI4) et Zusanli (ST36) a indiqu&eacute; que le syst&egrave;me limbique peut jouer un r&ocirc;le important pour les effets de l&#39;acupuncture. It is well documented that the psychiatric disorders are common in Asthma patients in China while the studies on the relationship between asthma control and psychological disorder are comparatively rare. We therefore performed a cross-sectional study on asthmatic outpatients in one Chinese tertiary center to investigate the correlation among asthma, anxiety and depression. Demographic data, anxiety and depression scores, the level of asthma control were collected in 261 patients. All patients were evaluated with Asthma Control Test (ACT), Self-Rating Anxiety Scale (SAS) and Self-rating depression scale (SDS) questionnaires. A multivariate analysis was performed to investigate the relationship between asthma control and psychological disorder. The result showed that 31 asthma patients had anxiety symptom while thirty-five asthma patients had depression. One hundred and ninety-two asthma patients were well controlled and 69 patients were not. The study found a negative correlation between ACT and SAS(r = -0.231, p<0.001) as well as ACT and SDS(r = -0.23, p<0.001) and depression (OR: 12.295, 95%CI: [5.374-28.128], p<0.001) were both independently associated with poor asthma control. We concluded that Asthma control is greatly affected by psychological disorder in Chinese patients.

The advent of the big data era as well as the formation and practice accumulation of the TCM clinical research paradigm in the real world have provided new perspectives, techniques, and methods for inheriting experience from famous TCM specialists. Through continuous exploration and practice, the research group proposes the innovation research mode based on the real-world TCM clinical research paradigm, which involves the inheritance and innovation of the existing modes. Acute Q fever, caused by , is a zoonotic infection presenting with non-specific symptoms such as high fever, severe headache and myalgia, making it challenging to diagnose. Traditional diagnostic methods often fall short due to their time-consuming nature and limited sensitivity. A 26-year-old male presented with severe headache, persistent high fever and nausea following a hiking trip. Initial tests, including serology and PCR, were inconclusive. Targeted next-generation sequencing (tNGS) identified  within 24 h, leading to a prompt diagnosis of acute Q fever. This rapid identification facilitated the initiation of appropriate antibiotic therapy, resulting in significant clinical improvement. This case underscores the diagnostic utility of tNGS in rapidly identifying rare pathogens and highlights its potential to influence clinical decision-making and improve patient outcomes. This case highlights the effectiveness of tNGS in diagnosing acute Q fever, particularly in regions where the disease is uncommon. The use of tNGS facilitated rapid identification and treatment, underscoring its potential as a valuable diagnostic tool in clinical practice.
