Preoperative comprehensive malignancy risk estimation for thyroid nodules: Development and verification of a network-based prediction model.

Improving access to HIV/AIDS healthcare services is of great concern to government and policymakers striving to strengthen overall public health. How to reasonably allocate HIV/AIDS healthcare resources and maximize the equality of access to healthcare services across subdistrict areas has become an urgent problem to be solved. However, there is limited research on this topic in China. It is necessary to evaluate spatial accessibility to improve the accessibility and equity of HIV/AIDS healthcare services. In this study, the improved multi-modal two-step floating catchment area (2SFCA) and inverted 2SFCA (i2SFCA) methods are used to measure the spatial accessibility of HIV/AIDS healthcare services and the crowdedness of the healthcare sites in Shandong Province, China. Then, the theoretical supply and the optimal spatial distribution of resources are calculated and visualized by minimizing the accessibility gaps between demand locations. This study showed that the spatial accessibility of HIV/AIDS service resources in Shandong Province was concentrated and unevenly distributed, and the accessibility scores in the marginal areas of prefecture-level cities were significantly lower than those in other areas. Regions with a large number of doctors had significantly higher levels of spatial accessibility. The ART accessibility scores in the southwest of Shandong Province were higher than those in other regions. As the travel friction coefficient increased, the accessibility scores formed an approximately circular cluster distribution centered on the healthcare sites in geographical distribution. More ART drugs needed to be supplied in marginal areas and more doctors were needed to work on HIV/AIDS in urban areas to address the spatial distribution imbalance of HIV/AIDS healthcare services. This study profoundly analyzed the spatial accessibility of HIV/AIDS healthcare services and provided essential references for decision-makers. In addition, it gives a significant exploration for achieving the goal of equal access to HIV/AIDS healthcare services in the future.

Evaluating the spatial accessibility and spatial layout optimization of HIV/AIDS healthcare services in Shandong Province, China.

To prevent the overaggressive treatment of axillary lymph nodes (ALNs) in breast cancer, it is necessary to develop a convenient analysis method that accurately and comprehensively reflects whether ALNs are metastatic or nonmetastatic. We retrospectively analyzed data from patients who underwent surgery for breast cancer at the Weifang Hospital of Traditional Chinese Medicine between January 2019 and June 2023. Binary logistic regression analysis was used to predict the metastasis status of ALNs. The developmental data set included 531 patients (January 2019-June 2023). The validation set included separate data points (n = 178, January 2019-June 2023). Multivariate analysis revealed that positive findings on breast physical examination, ultrasound grades of ALNs, lymphovascular invasion, and Her-2 status had significant predictive value for metastatic ALNs. Based on these findings, a 5-grade risk scoring system and 3-level management recommendations were developed. The risk of metastasis ranged from 11.25 to 93.46%, which was positively correlated with an increase in risk grade. The areas under the curve of the development and validation sets were 0.895 and 0.865, respectively. Ultimately, a convenient, accurate and comprehensive web-based predictive model was constructed using various breast cancer clinical, imaging and pathological criteria to stratify ALNs according to the metastasis probability.

Preoperative comprehensive risk estimation for axillary lymph node metastasis in breast cancer: development and verification of a network-based prediction model.

The present study aimed to construct a human epidermal growth factor receptor 2 (HER2)-related gene risk model to predict breast cancer prognosis. Gene expression and clinical follow-up data were extracted from The Cancer Genome Atlas database, while the GSE7390 dataset was obtained from the Gene Expression Omnibus database. Prognostic and clinical feature analyses were performed. In addition, differentially expressed genes (DEGs) between HER2-negative and -positive groups were screened, followed by enrichment analysis. Subsequently, a prognostic model was established, and prognosis was predicted using a nomogram. In addition, the association of risk score with immunity was analyzed, and single-cell analysis was performed. Next, key genes were identified by reverse transcription-quantitive PCR (RT-qPCR) analysis. The results revealed that HER2 was significantly associated with estrogen receptor status, progesterone receptor status, N stage, American Joint Committee on Cancer stage, mutation count and tumor mutation burden of breast cancer. AS601245, AP.24534 and roscovitine were the top three chemotherapeutic agents showing the highest sensitivity differences between the risk groups. A total of 251 DEGs between HER2-negative and -positive groups were screened, which were found to be significantly involved in the Kyoto Encyclopedia of Genes and Genomes pathway of estrogen signaling, PI3K-AKT signaling pathway and chemical carcinogenesis-receptor activation. Eight prognostic gene models were constructed, and it was found that patients in the high-risk group had significantly shorter survival times than those in the low-risk group. A nomogram, incorporating risk groups and clinicopathological features, demonstrated strong predictive ability and high accuracy. The RT-qPCR results indicated that the expression of electron transfer flavoprotein subunit α, rap guanine nucleotide exchange factor-like 1, keratin 7, cluster of differentiation 24, proline rich 15-like, arachidonate 15-lipoxygenase type B, ELOVL fatty acid elongase 2 and C-X-C motif chemokine ligand 9 was consistent with the results of bioinformatic analysis. In conclusion, the HER2-related risk model and nomogram developed in the present study demonstrated high accuracy in predicting patient survival.
