A PCR-based genotyping system that detects divergence of IS100 locations within the Yersinia pestis genome was used to characterize a large collection of isolates of different biovars and geographical origins. Using sequences derived from the glycerol-negative biovar orientalis strain CO92, a set of 27 locus-specific primers was designed to amplify fragments between the end of IS100 and its neighboring gene. Geographically diverse members of the orientalis biovar formed a homogeneous group with identical genotype with the exception of strains isolated in Indochina. In contrast, strains belonging to the glycerol-positive biovar antiqua showed a variety of fingerprinting profiles. Moreover, strains of the biovar medievalis (also glycerol positive) clustered together with the antiqua isolates originated from Southeast Asia, suggesting their close phylogenetic relationships. Interestingly, a Manchurian biovar antiqua strain Nicholisk 51 displayed a genotyping pattern typical of biovar orientalis isolates. Analysis of the glycerol pathway in Y. pestis suggested that a 93-bp deletion within the glpD gene encoding aerobic glycerol-3-phosphate dehydrogenase might account for the glycerol-negative phenotype of the orientalis biovar. The glpD gene of strain Nicholisk 51 did not possess this deletion, although it contained two nucleotide substitutions characteristic of the glpD version found exclusively in biovar orientalis strains. To account for this close relationship between biovar orientalis strains and the antiqua Nicholisk 51 isolate, we postulate that the latter represents a variant of this biovar with restored ability to ferment glycerol. The fact that such a genetic lesion might be repaired as part of the natural evolutionary process suggests the existence of genetic exchange between different Yersinia strains in nature. The relevance of this observation on the emergence of epidemic Y. pestis strains is discussed.

Genetic variability of Yersinia pestis isolates as predicted by PCR-based IS100 genotyping and analysis of structural genes encoding glycerol-3-phosphate dehydrogenase (glpD).

Pour tenir compte de cette relation &eacute;troite entre le biovar orientalis souches et l&#39;antiqua Nicholisk 51 isoler, nous postulons que cette derni&egrave;re repr&eacute;sente une variante de ce biovar avec restaur&eacute; aptitude &agrave; fermenter le glyc&eacute;rol. Le fait qu&#39;une telle l&eacute;sion g&eacute;n&eacute;tique pourrait &ecirc;tre r&eacute;par&eacute;e dans le cadre du processus &eacute;volutionnaire naturel sugg&egrave;re l&#39;existence d&#39;&eacute;changes g&eacute;n&eacute;tiques entre diff&eacute;rentes souches de Yersinia dans la nature. Chromosome 19 has the highest gene density of all human chromosomes, more than double the genome-wide average. The large clustered gene families, corresponding high G + C content, CpG islands and density of repetitive DNA indicate a chromosome rich in biological and evolutionary significance. Here we describe 55.8 million base pairs of highly accurate finished sequence representing 99.9% of the euchromatin portion of the chromosome. Manual curation of gene loci reveals 1,461 protein-coding genes and 321 pseudogenes. Among these are genes directly implicated in mendelian disorders, including familial hypercholesterolaemia and insulin-resistant diabetes. Nearly one-quarter of these genes belong to tandemly arranged families, encompassing more than 25% of the chromosome. Comparative analyses show a fascinating picture of conservation and divergence, revealing large blocks of gene orthology with rodents, scattered regions with more recent gene family expansions and deletions, and segments of coding and non-coding conservation with the distant fish species Takifugu.

DNA microarrays encompassing the entire genome of Yersinia pestis were used to characterize global regulatory changes during steady-state vegetative growth occurring after shift from 26 to 37 degrees C in the presence and absence of Ca2+. Transcriptional profiles revealed that 51, 4, and 13 respective genes and open reading frames (ORFs) on pCD, pPCP, and pMT were thermoinduced and that the majority of these genes carried by pCD were downregulated by Ca2+. In contrast, Ca2+ had little effect on chromosomal genes and ORFs, of which 235 were thermally upregulated and 274 were thermally downregulated. The primary consequence of these regulatory events is profligate catabolism of numerous metabolites available in the mammalian host.

Ent&eacute;rovirus enc&eacute;phalite est une neuroinfection rare plus souvent diagnostiqu&eacute;e chez les enfants dans le contexte des flamb&eacute;es &eacute;pid&eacute;miques enteroviral. Il a des caract&eacute;ristiques cliniques pl&eacute;omorphes, s&eacute;v&eacute;rit&eacute; variable et une certaine susceptibles de causer des s&eacute;quelles neuropsychologiques particuli&egrave;rement chez les nourrissons. Certains sous-types sont extr&ecirc;mement s&eacute;v&egrave;res avec un taux de mortalit&eacute; de jusqu&#39;&agrave; 25 % en agissant sur le tronc c&eacute;r&eacute;bral. Ces sous-types viennent habituellement avec des conclusions tr&egrave;s pr&eacute;cises sur la neuro-imagerie. Nous pr&eacute;sentons un cas p&eacute;diatrique d&#39;enc&eacute;phalite diffuse tr&egrave;s probablement enteroviral dans la nature, avec quelques particularit&eacute;s concernant la forme clinique grave, les aspects de la neuro-imagerie et les s&eacute;quelles neuropsychologiques en raison de l&#39;&eacute;volution d&eacute;crite rarement vers enc&eacute;phalomalacie.

Enteroviral encephalitis is a rare neuroinfection more often diagnosed in children within the context of enteroviral epidemic outbreaks. It has pleomorphic clinical features, variable severity and a definite potential to cause neuropsychological sequelae especially in infants. Some subtypes are extremely severe with a mortality rate of up to 25% by affecting the brain stem. These subtypes usually come with highly specific findings on neuroimaging. We present a paediatric case of diffuse encephalitis most likely enteroviral in nature, with some particular features concerning the severe clinical form, the neuroimaging aspects and the neuropsychological sequelae due to the rarely described evolution towards encephalomalacia.

Encefalite classica &egrave; una rara neuroinfection pi&ugrave; spesso diagnosticata nei bambini all&#39;interno del contesto dei focolai epidemici classica. Ha caratteristiche cliniche pleomorfo, gravit&agrave; variabile e un potenziale definito a provocare sequele neuropsicologiche soprattutto nei neonati. Alcuni sottotipi sono estremamente gravi, con un tasso di mortalit&agrave; del 25% che colpisce il tronco encefalico. Questi sottotipi vengono solitamente con risultati altamente specifici sul neuroimaging. Presentiamo un caso pediatrico di encefalite diffusa probabilmente classica in natura, con alcune caratteristiche particolari riguardanti la forma clinica grave, gli aspetti di neuroimaging e le sequele neuropsicologiche a causa dell&#39;evoluzione raramente descritto verso encefalomalacia.

Whipple disease is a disorder caused by Tropheryma whipplei, a ubiquitous Gram-positive bacillus. In addition to gastrointestinal manifestations, many other systems may be involved in Whipple disease. Pulmonary hypertension (PH) is a rare manifestation of Whipple disease, and its clinical course is not well established. We report a case of a 45-year-old woman who presented with typical gastrointestinal manifestations of Whipple disease, which was diagnosed by duodenal biopsy. She was also noted to have elevated pulmonary arterial pressures on transthoracic echocardiography. There was no evidence of left-sided valvular disease, hypertrophy, or dyskinesis, and there was no evidence of endocarditis. The patient was started on intravenous ceftriaxone for 6 weeks and then transitioned to oral trimethoprim-sulfamethoxazole for a year. The patient demonstrated clinical improvement, endoscopic and histologic improvement, and also resolution of PH. This is the third reported case of PH that is convincingly secondary to Whipple disease that resolved after appropriate antibiotic therapy.

Resolution of Whipple Disease-Induced Pulmonary Hypertension Following Antibiotic Therapy.

En plus des manifestations gastro-intestinales, beaucoup d&#39;autres syst&egrave;mes peut-&ecirc;tre &ecirc;tre impliqu&eacute;s dans la maladie de Whipple. Hypertension pulmonaire (HP) est une manifestation rare de la maladie de Whipple, et son &eacute;volution clinique n&#39;est pas bien &eacute;tablie. Nous rapportons le cas d&#39;une femme de 45 ans qui pr&eacute;sent&eacute;e des manifestations gastro-intestinales typiques de la maladie de Whipple, qui a &eacute;t&eacute; diagnostiqu&eacute; par biopsie duod&eacute;nale. Elle a aussi not&eacute;e &agrave; ont &eacute;lev&eacute; des pressions art&eacute;rielles pulmonaires sur l&#39;&eacute;chocardiographie transthoracique. Il n&#39;y n&#39;avait aucun signe de maladie valvulaire c&ocirc;t&eacute; gauche, hypertrophie ou dyskinesis, et il n&#39;y n&#39;avait aucune preuve d&#39;endocardite. Le patient a commenc&eacute; sur la ceftriaxone par voie intraveineuse pendant 6 semaines et puis est pass&eacute; &agrave; la trim&eacute;thoprime-sulfam&eacute;thoxazole orale pendant un an. Le patient a d&eacute;montr&eacute; d&#39;am&eacute;lioration clinique, am&eacute;lioration endoscopique et histologique et aussi la r&eacute;solution du PH. C&#39;est le troisi&egrave;me cas signal&eacute; de PH qui est convaincante secondaire &agrave; la maladie de Whipple r&eacute;solus apr&egrave;s une antibioth&eacute;rapie appropri&eacute;e.

Malattia di Whipple &egrave; un disturbo causato da molte altre, un bacillo Gram-positivo onnipresente. Oltre alle manifestazioni gastrointestinali, molti altri sistemi possono essere coinvolti nella malattia di Whipple. Ipertensione polmonare (PH) &egrave; una rara manifestazione della malattia di Whipple, e il suo decorso clinico non &egrave; ben definito. Riportiamo un caso di una donna di 45 anni che ha presentato con manifestazioni gastrointestinali tipici della malattia di Whipple, che &egrave; stata diagnosticata con biopsia duodenale. Lei &egrave; stato anche osservata per avere elevate pressioni arteriose polmonare su ecocardiografia transtoracica. Non non c&#39;era nessuna evidenza di malattia valvolare di sinistra-parteggiato, ipertrofia o discinesie, e non non c&#39;era nessuna prova di endocardite. Il paziente &egrave; stato iniziato su ceftriaxone per via endovenosa per 6 settimane e quindi la transizione a orale trimetoprim-sulfametossazolo per un anno. Il paziente ha dimostrato il miglioramento clinico, il miglioramento endoscopico e istologico e anche risoluzione di PH. Questo &egrave; il terzo caso riportato di PH che &egrave; in modo convincente secondaria a malattia di Whipple che ha risolto dopo appropriata terapia antibiotica.
