Collaborative care for pulmonary tuberculosis involves a prolonged medication course tailored to drug-susceptible or drug-resistant tuberculosis.
Drug-susceptible tuberculosis refers to strains of tuberculosis that are sensitive to the standard first-line anti-tuberculosis drugs, like Isoniazid, Rifampin, Pyrazinamide, and Ethambutol.
These drugs are administered in combination orally once a day for eight weeks during the initial phase.
The continuation phase of treatment then includes Isoniazid and Rifampin for four or seven months.
In contrast, drug-resistant tuberculosis occurs when the patient becomes resistant to at least one of the first-line drugs, often due to incorrect dosage or premature treatment termination. It requires second-line drugs such as Fluoroquinolones.
Directly Observed Therapy or DOT is the preferred strategy to ensure all patients take their medications promptly, where a designated person observes the patient swalloing the medication to prevent nonadherence and multidrug resistance.

Tuberculosis treatment depends on whether the infection is latent or active, and it also depends on the patient’s overall health. Latent tuberculosis infection means TB bacteria are present in the body but are not causing illness or symptoms. It is not contagious, but preventive treatment is important to keep it from becoming active TB disease.

Latent TB can be treated in a few ways. One option is isoniazid, or INH, taken daily for nine months. Another is INH with rifapentine, or RPT, taken once weekly for three months under direct observation. A third option is rifampin, or RIF, taken daily for four months.

Active TB disease is usually treated in two phases. The initial phase lasts two months and uses four drugs: INH, RIF, ethambutol, or EMB, and pyrazinamide, or PZA. The exact plan can change based on how the patient responds and on the TB strain. The continuation phase lasts at least four months and usually includes INH and RIF.

Some TB strains are drug-resistant. Multidrug-resistant tuberculosis does not respond to at least INH and RIF, which are the two strongest anti-TB drugs. Extensively drug-resistant tuberculosis is an advanced form of multidrug-resistant TB. It is resistant not only to fluoroquinolones, but also to at least one of three injectable second-line medicines.

When first-line drugs do not clear the bacteria, second-line drugs are used. These include fluoroquinolones such as levofloxacin and moxifloxacin. They also include aminoglycosides such as streptomycin, ethambutol, and cycloserine. Treatment for resistant TB must be individualized and usually lasts longer.

TB care also includes strict safety and follow-up steps. In a hospital, patients should be on droplet precautions, and healthcare providers should wear a gown, gloves, an N95 mask, and protective eyewear.
